Psoriasis is a chronic inflammatory skin condition mediated by T-helper 1 (Th1) and T-helper 17 (Th17) cytokines, including interleukin (IL)-12, IL-23, IL-17, and tumor necrosis factor alpha (TNF-α). Ustekinumab, a human monoclonal antibody against the p40 subunit of IL-12/23, has revolutionized treatment of moderate to severe psoriasis but carries a risk of opportunistic infections due to impaired cell-mediated immunity. We describe a 54-year-old man from Kentucky with type 2 diabetes and chronic sinusitis, receiving ustekinumab for 1 year for plaque psoriasis. He presented with recurrent pne

BACKGROUND: The dimorphic mycoses (DMs) histoplasmosis, blastomycosis, and coccidioidomycosis are classically thought to be geographically restricted to specific regions of the United States. The current scientific consensus is that these geographic boundaries are expanding, but the extent of this expansion remains uncertain.
METHODS: This study is a retrospective analysis of >21 million (annually) commercially insured individuals within the United States during 2007-2016. Diagnosis of histoplasmosis, coccidioidomycosis, and blastomycosis were identified using International Classification of

Myocardial infarction from coronary ischemia in pediatric patients with Fontan circulation is a rare diagnosis with scarce data reported on it. We describe a case of a teenage male who initially presented with an acute occlusive right middle cerebral artery (MCA) stroke with a history of nonadherence to aspirin. He underwent a thrombectomy shortly after presentation and was not restarted on anticoagulation due to hemorrhagic transformation of the stroke postprocedure. He had no residual neurologic deficits. On the sixth day of his admission and without prior symptoms, he had a sudden cardiac a

Blastomycosis is the clinical manifestation of infection caused by the yeast phase of the thermally dimorphic fungus Blastomyces dermatitidis, an environmentally poorly understood endemic North American mycosis that can progress to severe pulmonary disease in immunocompromised patients. We present a case of a 77-year-old male with a past medical history of cutaneous follicular non-Hodgkin lymphoma, renal cell carcinoma requiring total right nephrectomy, insulin dependent type 2 diabetes, and pulmonary chromoblastomycosis with left upper lobe lobectomy who presented to the emergency room for a

BACKGROUND: Dimorphic mycoses are often neglected or underestimated. This study aimed to evaluate the demographic and clinical characteristics of seven dimorphic mycoses.
METHODS: This retrospective study used TriNetX, a global database of multicenter electronic medical records, to evaluate the clinical characteristics of dimorphic mycoses between 2015 and 2025. The mycoses included were blastomycosis, histoplasmosis, coccidioidomycosis, paracoccidioidomycosis, talaromycosis, sporotrichosis, and lobomycosis.
RESULTS: The number of identified cases was as follows: blastomycosis 3711, histopla

We present the case of a 67-year-old man undergoing immunosuppressive therapy with known pulmonary blastomycosis who presented with knee swelling and fever. FDG-PET/CT performed to exclude septic arthritis revealed increased uptake within the right epididymis and multiple osteoarticular, cutaneous, and subcutaneous sites. Cultures obtained from bronchoalveolar lavage, urine, right knee arthrocentesis, and subcutaneous tissue all demonstrated Blastomyces dermatitidis , compatible with disseminated blastomycosis (DB). To the best of our knowledge, this is the first case of DB in a human subject

We presented a case of disseminated blastomycosis in an immunocompetent host, detailing the disease's clinical course, the challenges of diagnosis, and the therapeutic management. A systematic review of the literature further clarified the epidemiological patterns, clinical manifestations, diagnostic criteria, and treatment strategies for blastomycosis. This report highlighted the importance of heightened clinical suspicion among physicians in non-endemic regions, to enable prompt diagnosis and mitigate the risks of adverse outcomes associated with delayed recognition.

A 67-year-old male patient presented with anorexia, weight loss, skin masses, and flank pain over the past three months. Imaging revealed a lung mass and a cystic lesion in the pancreatic head. After initial diagnostic work-up and clinical deterioration, broad based budding, seen on pathology, led to a diagnosis of disseminated blastomycosis. After receiving a 14-day course of liposomal amphotericin B followed by itraconazole, the patient showed significant clinical improvement. We reviewed all disseminated blastomycosis cases in the medical literature involving gastrointestinal organs. Althou

BACKGROUND Blastomycosis is a fungal infection caused by Blastomyces spp., a dimorphic fungus hyperendemic in Wisconsin, USA. CASE REPORT A 54-year-old previously healthy man presented with right foot and thigh pain, erythema, and a non-healing purulent ulcer on the right buttock. Initial treatment with oral doxycycline and cephalexin was unsuccessful, and subsequent imaging revealed a thigh hematoma and acute osteomyelitis of the forefoot with an adjacent abscess. The patient underwent 2 incision-and-drainage procedures, which uncovered a disarticulated second toe and persistent purulence. Hi

Blastomycosis is an endemic fungal infection caused by Blastomyces dermatitidis. Immunosuppressed individuals, as well as those with diabetes mellitus or obesity, are more likely to develop severe disease. It is often referred to as "the great imitator" because it can closely mimic tuberculosis. Early diagnosis is critical to prevent delays in appropriate treatment and avoid complications. We present the case of a 51-year-old Haitian immigrant with poorly controlled diabetes mellitus who was initially diagnosed and treated for tuberculosis based on a positive interferon-gamma release assay (IG

Blastomyces dermatitidis, the fungus responsible for blastomycosis, can cause systemic disease with clinical manifestations ranging from asymptomatic infection to acute or chronic pneumonia and extrapulmonary diseases. Here, we present a case of sacral osteomyelitis with multiple abscesses and a fistulous tract, which failed to improve after multiple courses of antibacterial therapy for presumed bacterial causes, leading to a significantly delayed diagnosis. Blastomycosis was diagnosed by potassium hydroxide (KOH) microscopy, fungal culture, and broad-range polymerase chain reaction (PCR). The

BACKGROUND: Histoplasmosis capsulatum and Blastomyces dermatitidis are fungi that are uniquely distributed across the United States. Co-infection with both fungi remains uncommon. The literature review of coinfection was limited to three cases. Only two cases were recorded in humans. Infection can yield symptoms ranging from asymptomatic to life-threatening. On radiographic imaging, diffuse reticulonodular infiltrates with hilar or mediastinal lymphadenopathy can be seen. Management depends on the severity of the disease and the patient's immune state.

Urban cases of blastomycosis are extremely rare and usually involve an individual or individuals with recent travel to, and outdoor exposure in, an endemic area. We report a case of severe cutaneous blastomycosis in a New York City resident. He presented to our hospital in 2023 with multiple cavitary lung nodules and a large area of fungating lesions on his back that had been progressing for a month. Cultures and pathology from the skin lesions confirmed a diagnosis of cutaneous blastomycosis, and the patient was successfully treated with a prolonged course of oral itraconazole. Given no trave

Blastomycosis is a fungal infection caused by Blastomyces dermatitidis, which is hyperendemic to areas of Northwestern Ontario, Canada. Delayed diagnosis is common due to the condition's nonspecific clinical presentation, often resembling malignancy or other dermatological and infectious diseases. We report the case of a 27-year-old otherwise healthy male with an early presentation of disseminated cutaneous blastomycosis. Early recognition of cutaneous blastomycosis can significantly reduce morbidity, mortality, and the overall treatment burden by facilitating timely antifungal therapy. This c

Blastomyces dermatitidis, a dimorphic fungal infection endemic to North America, is typically associated with pulmonary disease, but may also disseminate. We report the case of a 54-year-old female liver transplant recipient who developed blastomycosis with cutaneous and likely pulmonary involvement. She was successfully treated with posaconazole, an alternative to itraconazole with a favorable safety and interaction profile, demonstrating the therapeutic potential of posaconazole as a primary agent for blastomycosis in transplant recipients.

A patient presented with persistent pustular, violaceous nodules and a tense bulla that was initially diagnosed as bacterial folliculitis. Despite four courses of oral antibiotics, there was no clinical improvement. The patient's cutaneous symptoms progressed, and he developed systemic symptoms. A punch biopsy, blood studies and a chest CT confirmed a diagnosis of disseminated blastomycosis. The patient improved with itraconazole. This case highlights the diagnostic challenges of atypical cutaneous blastomycosis, which can mimic bacterial folliculitis and lead to delays in treatment. Clinician
