Delayed diagnosis of mucopolysaccharidosis type I in a patient with spinopelvic instability, short stature, and skeletal dysplasia.

Pathogenic variants in IDUA, encoding α-L-iduronidase (IDUA), cause the autosomal recessive lysosomal storage disorder mucopolysaccharidosis type I (MPS I). Deficiency of α-L-iduronidase leads to the accumulation of partially degraded glycosaminoglycans in various tissues, including the musculoskeletal system. Clinically, typical skeletal manifestations are dysostosis multiplex, joint contractures, and low BMD. Mucopolysaccharidosis type I represents a spectrum of disease severity. Especially in attenuated disease, establishing an accurate diagnosis and initiating disease-specific therapy, inc

Efficacy and Safety of Microvascular Decompression in Patients with Trigeminal Neuralgia Previously Treated with Gamma Knife Stereotactic Radiosurgery.

Background/Objectives: Gamma Knife stereotactic radiosurgery (GKS) controls classical trigeminal neuralgia (TN) in 70-90% of patients, but pain recurs in a substantial minority, and these patients are then considered for microvascular decompression (MVD). Whether radiation-induced arachnoid thickening, vessel-nerve adhesion and nerve atrophy compromise the safety or efficacy of subsequent MVD remains unresolved, and the available series are small. The objective of this study was to determine whether prior GKS alters the efficacy or the safety of subsequent MVD. Methods: We performed a retrospe

Efficacy and safety of electrical cardioversion and catheter ablation during pregnancy: a case review and literature analysis.

BACKGROUND: Despite guideline recommendations, electrical cardioversion and catheter ablation remain underutilized for the management of pregnancy-associated arrhythmias, primarily due to persistent but unfounded safety concerns. However, delayed rhythm control may precipitate hemodynamic compromise, increasing risks of maternal cardiovascular instability and fetal distress. Herein, we report an illustrative case from our institution and provide a structured narrative review of electrical cardioversion and catheter ablation during pregnancy. METHODS: We present a case of recurrent arrhythmia

Ablation of a Para-Hisian Accessory Pathway from the Non-coronary Cusp: A Case Report.

Para-Hisian accessory pathways (PHAPs) remain among the most challenging substrates for catheter ablation because of their close anatomical relationship to the atrioventricular (AV) conduction system and the consequent risk of iatrogenic AV block. The non-coronary cusp (NCC) has emerged as a valuable alternative when right-sided ablation is ineffective or associated with an unacceptable risk of injury to the His bundle. We report the case of a 23-year-old man who presented with pre-excited atrial fibrillation. Electrophysiological study localized an anteroseptal/para-Hisian accessory pathway.

Efficacy and Safety of Omadacycline in Patients with Mycoplasma Pneumoniae Harboring the 23S rRNA A2063G Mutation.

OBJECTIVE: Mycoplasma pneumoniae is a major pathogen of community-acquired bacterial pneumonia (CABP). Macrolide-resistant Mycoplasma pneumoniae (MRMP) harboring the 23S rRNA A2063G mutation poses a global therapeutic challenge. Omadacycline, a novel aminomethylcycline approved for CABP, exhibits activity against MRMP. However, real-world data on omadacycline for A2063G-mutated MRMP pneumonia remain limited. In this study, we present our clinical experience with intravenous omadacycline in patients with genetically confirmed A2063G-mutated MRMP pneumonia. METHODS: We retrospectively analyzed

Nx3 Is a Z-Disk Structural and Signaling Hub That Is Reduced in Heart Failure.

BACKGROUND: Nx3 (novex-3) is an exceptionally small isoform of the giant protein titin, whose structural and functional roles within the sarcomere remain poorly understood. METHODS AND RESULTS: We used a comprehensive, multimodal approach to define the key properties of Nx3 in healthy and failing hearts, including its abundance relative to FLT (full-length titin), sarcomeric localization, protein interactions, and functional relevance in mouse and human cardiomyocytes under physiological and pathological conditions. Using Western blotting, quantitative polymerase chain reaction, total RNA seq

Ischemic Stroke Occurring After Supraventricular Dysrhythmia Conversion With Carotid Massage: A Case Report.

BACKGROUND: Supraventricular tachycardia (SVT) is commonly managed with vagal maneuvers, including carotid sinus massage, as first-line therapy. If unsuccessful, pharmacological treatment with adenosine is recommended. However, concerns regarding the safety and efficacy of carotid sinus massage have been raised after previous case reports have described ischemic stroke following its use. CASE REPORT: A 67-year-old woman presented with acute chest pain, dizziness, and palpitations. She had a heart rate of 234 bpm prehospital but was hemodynamically stable on arrival. SVT was suspected; initial

Zero-fluoroscopy catheter ablation for arrhythmias in pregnancy: validation of a standardized approach with 5-year follow-up.

BACKGROUND: Increasing experience with zero- (ZF) or near-zero fluoroscopy catheter ablation (CA) supports the implementation of an early fluoroless approach for recurrent, symptomatic arrhythmias during pregnancy. AIM: This study evaluated the feasibility, efficacy, and safety of CA using a standardized ZF approach during pregnancy. METHODS: Data were derived from a large, prospective multicenter registry (ELEKTRO-RARE-A-CAREgistry). Between 2012 and 2019, more than 2,655 CA procedures were performed in women using an intention-to-treat ZF approach. The procedures were performed using (1) f

Acute Management of Guillain-Barré Syndrome: A Narrative Review.

Guillain-Barré syndrome (GBS) is a rapidly progressive immune-mediated polyradiculoneuropathy that can cause respiratory failure, autonomic dysfunction, and significant long-term disability. This narrative review examines the acute management of GBS, including supportive care, established immunotherapies, and emerging treatment strategies. Early recognition and close monitoring of respiratory and autonomic function remain essential, particularly during the progressive phase of disease. Intravenous immunoglobulin (IVIg) and plasma exchange remain the primary disease-modifying treatments and hav

ODC1-Mediated Ornithine Metabolism Exacerbates Obesity by Disrupting AMPK/ACC Pathway.

Ornithine (OR) is a key intermediate metabolite; however, its molecular role in obesity remains unclear. This study aimed to investigate the effects of OR and its rate-limiting enzyme, ornithine decarboxylase 1 (ODC1), on lipid metabolism using high-fat diet (HFD)-induced obese C57BL/6 mice and C3H10T1/2 cell models. The results showed that OR supplementation and ODC1 overexpression exerted similar effects, including significantly aggravated HFD-induced obesity, elevated serum polyamine levels, impaired glucose tolerance, reduced oxygen consumption, and hepatic steatosis. Transcriptomic analys

Delayed Diagnosis of Native-Valve Infective Endocarditis Presenting as Progressive Aortic Regurgitation in Recurrent Methicillin-Sensitive Staphylococcus aureus Bacteraemia With Initially Negative TTE and FDG PET-CT.

Infective endocarditis (IE) remains a life-threatening condition in which early diagnosis is essential to prevent valvular destruction and systemic complications. Persistent Staphylococcus aureus bacteraemia (SAB) is strongly associated with endocardial infection, yet diagnosis may be challenging when initial imaging is inconclusive. We describe a case of a 55-year-old woman who presented with fever, vomiting, and transient loss of consciousness. Blood cultures grew methicillin-sensitive Staphylococcus aureus (MSSA). Initial transthoracic echocardiography (TTE) and positron emission tomography

Giant Pancreatic Duct Calculi in the Common Channel Two Decades Post Choledochojejunostomy: The Synergistic Impact of Persistent Anomalous Pancreaticobiliary Union and an Elongated Blind Stump.

Anomalous pancreaticobiliary ductal union (APBDU) represents a congenital anomaly predisposing patients to biliary dilatation. This aberrant architecture facilitates the intermingling of pancreatic and biliary fluids, thereby elevating susceptibility to cholelithiasis and malignancies of the biliary tract. While radical cholangiojejunostomy constitutes the standard therapeutic intervention, it frequently fails to rectify the underlying APBDU. Retention of an excessively elongated common bile duct (CBD) remnant can generate a blind loop, precipitating stone formation within the common channel y

A new oculomotor model demystifies "Remarkable Saccades".

Hering's Law of binocular eye movement control guides most oculomotor research and supports diagnosis and treatment of clinical eye misalignment (strabismus). It states that all eye movements are controlled by a unitary conjugate signal and a unitary vergence signal that sum. Recent evidence of temporally asynchronous eye rotations during vergence (Chandna et al., 2021) challenges the viability of a unitary vergence signal. Helmholtz proposed an explanation of binocular control that does not require a vergence system, that the eyes are controlled independently. Yet independent control fails to

Progressive unilobar cavitating lung disease despite medical therapy: Diagnostic limitations and definitive surgical management.

Cavitating lung lesions are a common radiological finding with a broad differential diagnosis, including infectious, malignant and inflammatory causes. Management of infective lesion is often challenging, particularly when disease progresses despite appropriate medical therapy. We report a case of progressive, unilobar cavitating lung disease in a 59-year-old man related to Aspergillus colonization and Streptococcus constellatus infection, despite prolonged optimal antibacterial and antifungal treatment. Initial bronchoscopic investigations identified Aspergillus hyphae, but repeated conventio

Superior Mesenteric Artery Syndrome After Previous Nuss Procedure: A Case Report of Anatomical Realignment With Unintended Consequences.

BACKGROUND: Superior mesenteric artery (SMA) syndrome is a rare condition resulting from compression of the third segment of the duodenum. The condition is associated with a reduction of the aortomesenteric angle and distance between the SMA and the abdominal aorta, often due to weight loss or anatomical changes. The Nuss procedure, a minimally invasive corrective procedure for pectus excavatum, may inadvertently alter this anatomy. CASE PRESENTATION: We report a case of a 22-year-old male with a history of pectus excavatum who presented with two weeks of abdominal pain and vomiting. Abdomina
