Leaflet Modification Technique With UNICORN in Failed Aortic Bioprosthesis: Procedural Step-by-Step, Best Practice, and Troubleshooting.

The UNICORN (undermining iatrogenic coronary obstruction with radiofrequency needle) technique was developed in 2022 to reduce the risk for coronary obstruction during valve-in-valve transcatheter aortic valve replacement. The procedure involves traversing the target leaflet with an electrified wire, followed by serial dilation with coronary and peripheral balloons. A balloon-expandable valve is then delivered into the de novo fenestration for intraleaflet deployment. As the balloon-expandable valve expands, the target leaflet is opened and displaced away from the at-risk coronary ostium, resu

Multiple Modelling Methods Suggest That Grain-Free Diets Are Not Predictive of New Owner-Reported Cardiac Diagnoses Over Two Years in the Dog Aging Project, Despite Owner Misclassification of Diet.

BACKGROUND: Grain-free diets have been potentially linked to dilated cardiomyopathy (DCM) in several studies in dogs, although the underlying mechanisms for this potential link, if present, are unclear. Additionally, some studies have found no association between grain-free diets and DCM in dogs. Unfortunately, many of these studies were limited by the validity of owner-reported diet information. Furthermore, the effect of grain-free diets on other cardiac conditions in dogs is even less studied. OBJECTIVES: To assess the validity of owner-reported grain-free diet status information, and to m

Sub-glottic Diameter Measured by Ultrasonography Predicts Outer Diameter of the Clinically Best Fit Endotracheal Tube in Infants Undergoing Pediatric Cardiac Surgery.

BACKGROUND: Tracheal intubation with an oversized endotracheal tube (ETT) in a pediatric patient can lead to serious complications following extubation, including stridor, hoarseness, difficulty in breathing, increased work of breathing, respiratory failure, cardiac failure, and reintubation. Whereas undersized ETT may lead to erratic delivery of preset tidal volume during mechanical ventilation and increase the risk of aspiration of gastric contents. This study investigates the correlation between ultrasound measured subglottic airway diameter and ETT size selected by the anesthesiologists in

A translational cross-modal control-cost framework for executive breakdown.

Across neurological and neuropsychiatric conditions, patients often know what they should do yet cannot bring themselves to do it. This dissociation between intact knowledge and failed implementation, which we call the knowing-doing gap, is clinically important but lacks a unified, measurable account. We propose that converting knowledge into action carries a cost, termed executive friction (Ψ f ), and that behavior breaks down when this cost exceeds the control resources available to pay it. The proposal is specified at two levels: a measurable factor estimated from observable indicators, and

Removal of proximally migrated pancreatic stents: Ten-year experience from a large endoscopy center and literature review.

BACKGROUND AND STUDY AIMS: With widespread application of pancreatic stent in managing pancreatic diseases and preventing post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis, incidence of stent-related complications such as proximal migration has substantially increased. This study aimed to provide strategic insights for retrieving proximally migrated pancreatic stents by analyzing a large endoscopy center experience and synthesizing novel techniques. PATIENTS AND METHODS: A retrospective analysis was conducted over 10 years, including 43 cases of proximally migrated pancr

Bronchoscopic Internal Traction for Airway Foreign Body Removal via Tracheotomy Under ECMO.

We present a technique utilizing internal traction-assisted mobilization to effectively retrieve objects, such as pen caps, that lack an external grasping surface and resist conventional rigid bronchoscopy. In this case, tracheotomy served as an alternative extraction route for impacted foreign bodies that failed to pass across the glottis or subglottic region. Concurrently, ECMO maintains oxygenation during complex airway manipulation and reduces the risk of hypoxia.

Progressive Extension of a Post-intubation Tracheal Laceration Presenting As Massive Pneumomediastinum: A Case Report.

Post-intubation tracheal laceration is rare and is usually managed conservatively, provided the tear is protected from positive pressure, whether by spontaneous ventilation, by a cuff sited beyond the lesion, or by ventilation delivered without loss of tidal volume. We describe a woman in her early 70s intubated at a district hospital for progressive impairment of consciousness. Imaging before intubation already showed a hiatal hernia, right bronchial mucous plugging, lobar consolidation and bilateral effusions, and initial ventilation delivered 500 mL at a rate of 18 per minute, approximately

Iatrogenic Femoral Neuropathy Mimicking Meralgia Paresthetica Following Groin Flap Surgery: A Case Report.

BACKGROUND: Groin flap surgery carries a risk of iatrogenic nerve injury, most commonly involving the lateral femoral cutaneous nerve and presenting as meralgia paresthetica. In contrast, femoral nerve injury is rare and typically associated with motor deficits. Pure sensory femoral neuropathy is exceptionally uncommon and may closely mimic meralgia paresthetica, creating a diagnostic challenge. CASE REPORT: We report a 24-year-old man who developed severe neuropathic pain over the anterolateral thigh following groin flap donor-site closure. Initial clinical suspicion was meralgia paresthetic

Redo-TAVR (TAV-in-TAV) Best Practices Part 1: Short-in-Short and Short-in-Tall: A Heart and Valve Collaboratory Document.

Transcatheter aortic valve replacement (TAVR) has created a growing population of patients who require redo-TAVR (TAV-in-TAV) for structural valve deterioration. Short-frame balloon-expandable valves implanted within failed short- or tall-frame transcatheter valves ("short-in-short" and "short-in-tall") pose specific challenges, including coronary risk assessment, valve sizing, anchoring, and preservation of coronary access. Under the auspices of the Heart and Valve Collaboratory, an international multidisciplinary panel of experts reviewed bench and clinical data and synthesized contemporary

Diagnostic Criteria for Convergence Excess: Diagnostic Validity of Clinical Signs Associated with Near Esophoria.

To propose which tests may be used for diagnosing convergence excess. A prospective study of a consecutive clinical sample was performed. Patients (18-35 years) attending optometric care underwent subjective refraction, cover test, and Symptom Questionnaire for Visual Dysfunctions (SQVD). Based on cover test and SQVD scores, two groups were recruited: 64 symptomatic subjects with near esophoria and 64 asymptomatic with normal binocular vision. Accommodative and binocular tests were assessed, identifying those with significant statistical differences between groups. Diagnostic validity was anal

When Lifelong Anosmia Reveals Hypogonadism: A Case of Kallmann Syndrome with Primary Amenorrhea.

BACKGROUND: Kallmann syndrome (KS) is a rare genetic disorder characterized by hypogonadotropic hypogonadism and anosmia. It is often underdiagnosed in females due to subtle clinical presentation. This can lead to delays in accurate diagnosis and proper long-term management, particularly in women presenting with amenorrhea and uterine hypoplasia. CASE ILLUSTRATION: A 25-year-old nulligravid woman presented with lifelong absence of spontaneous menstruation, anosmia, and partial pubertal development. She was initially misdiagnosed with secondary amenorrhea after a positive progesterone withdraw

A rescue assay for genetic diagnosis of oculocutaneous albinism using melanocytic MNT1 knock-out cells.

The molecular diagnosis of albinism is hampered by a significant number of genetic variants of unknown significance (VUS) including a majority of missense and in-frame insertion deletion variants. This contributes to the high rate of unresolved genetic diagnosis for this disease. We designed a straightforward test of missense VUS in albinism genes based on functional rescue. As a proof of concept, the assay was set up for testing variants in the TYR gene associated with oculocutaneous albinism type 1. The TYR gene was knocked-out in the human melanogenic MNT1 cell line and the resulting unpigm

Ectopic Pleomorphic Adenoma of the External Nose.

Pleomorphic adenomas (PAs) are the most common benign salivary gland tumors but rarely present in ectopic locations. When ectopic PAs occur in the head and neck, they most often arise from minor salivary gland tissue of the oral or nasal cavities. PAs of the external nose are exceedingly rare, and this is the first report of a PA arising from the nasal bridge. Here, we report a case of an African American man in his mid-20s with no pertinent medical history who presented with a slowly enlarging left nasal bridge mass that had grown over the past two to three years. Initially attributed to prio

Sexual Dimorphism Is Associated With Corneal Nerve and Epithelial Alterations and Tear-Film Defects in Pax6 Haploinsufficiency Mouse Model.

PURPOSE: The purpose of this study was to determine how Pax6 haploinsufficiency alters epithelial maturation, immune balance, sensory innervation, and lacrimal function across the ocular surface, and how these defects affect corneal repair. METHODS: Pax6tm1Ued mice crossed with CMV-Cre were used to generate Pax6+/- animals. Ocular-surface maturation, epithelial organization, immune infiltration, and sensory innervation were assessed by whole-cornea immunostaining. Standardized corneal abrasion was used to evaluate epithelial and neural repair. Corneal RNA sequencing and tear-film proteomics w

Helix-fixation atrial leadless pacing for rhythm-mediated cardiogenic shock after acute myocardial infarction.

Cardiogenic shock complicating inferior acute myocardial infarction is usually attributed to pump failure, but rhythm-mediated mechanisms may predominate when ischemia involving the sinoatrial nodal artery produces sinus node dysfunction, chronotropic incompetence, and junctional rhythm-induced atrioventricular dyssynchrony. We report a 79-year-old man who developed recurrent cardiogenic shock several hours after successful revascularization for inferior ST-elevation myocardial infarction despite only mildly reduced left ventricular systolic function. Invasive hemodynamic assessment demonstrat

Metastatic malignant melanoma presenting as bilateral acute mastitis in a lactating woman: a case report and literature review.

Malignant melanoma, an aggressive neoplasm with high metastatic potential, rarely metastasizes to the breast, with bilateral involvement being exceptionally uncommon. This case report details a 36-year-old postpartum woman who presented with bilateral, symmetrical, rock-hard breast swelling and erythema, initially misdiagnosed as severe lactational mastitis. Comprehensive evaluation ultimately revealed diffusely metastatic malignant melanoma. The primary site was identified as a previously excised cutaneous lesion on the chest wall, originally reported as a benign compound nevus. The patient e

Long-Term Outcomes in Patients with Dilated Left Ventricular Non-Compaction Versus Dilated Cardiomyopathy: A Propensity Score Matching Study.

The long-term prognosis of left ventricular noncompaction (LVNC) compared to dilated cardiomyopathy (DCM) remains inconclusive, as prior studies have frequently failed to adequately adjust for heart failure severity. We conducted a single-center, retrospective, comparative study enrolling 96 patients with dilated LVNC (dLVNC) and 437 patients with DCM diagnosed at Peking Union Medical College Hospital between January 2010 and December 2022. dLVNC was defined as meeting the Jenni criteria, with left ventricular ejection fraction (LVEF) ≤45% and increased left ventricular end-diastolic diameter
