Management of COVID-19-Associated Guillain-Barré Syndrome in a Full-Term Pregnant Woman: A Case Report.

Guillain-Barré syndrome (GBS) can occur after viral infections. Its occurrence after COVID-19 infection in the peripartum period is a very rare co-occurrence. Therefore, there are no guidelines for the management of these patients. We report the case of a 32-year-old pregnant woman who developed COVID-19-associated GBS with aspiration pneumonia, motor weakness, and ascending paralysis at 39 weeks of gestation. Preoperative plasmatic exchange (plasmapheresis) and oxygen support were very effective and allowed for a rapid recovery within five days. Because of foetal distress during labor, the pa

Aspiration Pneumonitis Causing Respiratory Collapse in a Pregnant Patient Not in Labor.

BACKGROUND: Aspiration is a known risk of obstetric anesthesia; however; it has not been previously described outside of active labor or the setting of anesthesia. CASE: We present the case of a 31-year-old patient with a twin gestation at 33 weeks of gestation, not in labor, with clinically silent aspiration leading to aspiration pneumonitis and respiratory collapse requiring endotracheal intubation and transport to a pulmonary critical care service. After 4 days, she was extubated and eventually underwent a cesarean delivery at 37 weeks of gestation with no long-term pulmonary sequelae. CO

Anesthetic management of parturients with Arnold Chiari malformation-I: a multicenter retrospective study.

BACKGROUND: Consensus regarding the safest mode of delivery and anesthetic management for parturients with Arnold Chiari malformation-I (ACM-I) remains controversial. This study assessed their anesthetic management and reported anesthetic complications during hospitalization for delivery. METHODS: This was a multicenter, retrospective, cohort study of patients with ACM-I undergoing vaginal or cesarean delivery. Data were obtained from the electronic databases of four United States academic institutions using International Classification of Diseases (ICD) codes from 2007-2017 at three sites an

Ultrasonographic evaluation of gastric contents in term pregnant women fasted for six hours.

BACKGROUND: Current fasting guidelines suggest six hours are adequate to minimise the aspiration risk after a light meal consumed by pregnant women undergoing elective caesarean section. We assessed gastric contents in non-labouring pregnant women, using ultrasonographic analysis. METHODS: In a prospective study, pregnant women ≥36 weeks' gestation, without conditions likely to influence gastric emptying, underwent ultrasonographic analysis of their gastric antrum, after six hours of fasting following a standardised light meal. The primary outcome was solid food content in the antrum. Other o

General anesthesia for caesarean section.

PURPOSE OF REVIEW: For most anaesthesiologists, the clinical experience with general anaesthesia for caesarean section is very low. General anaesthesia is mostly performed for emergency grade 1 caesarean section and due to a lack of time to apply a neuraxial anaesthesia technique. Unfortunately, the majority of anaesthesiologists rely on historical and partly outdated approaches in this stressful situation. We propose an evidence-based approach to general anaesthesia for caesarean section. RECENT FINDINGS: Rapid sequence induction using propofol and rocuronium should become the standard for g

Interventions at caesarean section for reducing the risk of aspiration pneumonitis.

BACKGROUND: Various interventions are used as prophylaxis for aspiration pneumonitis in obstetric anaesthesia. This review, based on a Cochrane systematic review currently being updated, examines whether interventions given before caesarean section reduce the risk of aspiration pneumonitis. METHODS: Twenty-two studies, involving 2658 women providing data in a usable format for meta-analysis were identified. RESULTS: Compared to no treatment or placebo, there was a significant reduction in the risk of intra-gastric pH <2.5 with antacids (risk ratio (RR) 0.17, 95% confidence interval (CI) 0.09

Obstetric anesthesia units in Israel: a national questionnaire-based survey.

BACKGROUND: This survey was performed to assess the organization and practice of obstetric anesthesia units in Israel. METHODS: A written questionnaire was mailed at the end of December 2005 to all Israeli anesthesia departments providing labor and delivery services in 2005 (n=25). RESULTS: A response rate of 100% accounted for 125,340 deliveries. All labor and delivery suites had on-site anesthesia department services. Data are presented as mean (range) or frequency. Eleven hospitals performed 2500-4999 deliveries/year, 6 hospitals 5000-7499 deliveries/year, and 4 hospitals 7500-9999 delive

[Preoperative fasting in labour].

Numerous scientific case reports and observational studies, including that of Curtis Mendelson with its great historical relevance, have reported the problem of aspiration pneumonitis during labour. Because of this, strict fasting prior to delivery to reduce pulmonary aspiration has been doctrine since the 1940s. However, maternal mortality from anaesthesia, particularly from aspiration, has dropped dramatically over recent decades. This is due both to far-reaching improved quality of care as well as technical progress in anaesthesia, which increasingly challenge the scientific basis for fasti

Acid aspiration prophylaxis in obstetrics in France: a comparative survey of 1998 vs. 1988 French practice.

BACKGROUND AND OBJECTIVE: To survey French anaesthetic practice regarding acid aspiration prophylaxis and compare it with an earlier survey. METHODS: A confidential questionnaire was sent to all 800 maternity units in France to assess three major topics: (a) drugs used for pharmacological prophylaxis; (b) regional anaesthesia for labour and Caesarean section and (c) techniques used for general anaesthesia and endotracheal intubation. RESULTS: Two-hundred-and-two units responded. Pharmacological prophylaxis was regularly used for labouring women in 78% of the responding units in 1998 (compare

Midwifery dilemma: to fast or feed the labouring woman. Part I: The case for restricting oral intake in labour.

This paper presents a literature review which pursues both sides of the labouring woman's oral intake debate--to fast or to feed the labouring woman. Part 1 provides an exploration of the phenomenon of gastric content aspiration associated with general anaesthesia and its historical and physiological underpinnings. Part 2 provides an examination of the literature concerning the benefits of fasting labouring women.

Pulmonary aspiration risk during emergency department procedural sedation--an examination of the role of fasting and sedation depth.

The assessment of pre-procedure fasting and control of sedation depth are prominent elements of widely disseminated procedural sedation guidelines and of the Joint Commission on Accreditation of Healthcare Organizations' standards. Both exist primarily to minimize the risk of pulmonary aspiration of gastric contents. This paper critically examines the literature on pre-procedure fasting and controlling sedation depth in association with pulmonary aspiration, and interprets this evidence in the context of modern emergency medicine practice. The article reviews the pathophysiology of aspiration

Fe(III)-mediated cellular toxicity.

Drug-induced liver injury is a frequent cause of hepatic dysfunction. Reliably establishing whether the liver disease was caused by a drug requires the exclusion of other plausible causes and the search for a clinical drug signature. The drug signature consists of the pattern of liver test abnormality, the duration of latency to symptomatic presentation, the presence or absence of immune-mediated hypersensitivity and the response to drug withdrawal. Determination of causality also includes an evaluation of individual susceptibility to drug-induced liver injury. This susceptibility is governed

[Prophylaxis against obstetric acid aspiration syndrome in the German Federal Republic in 1997. A review based on results of a federal survey].

A survey of all German hospitals providing obstetric anesthesia was performed in 1997 (n = 1061, recovery rate 82%; comprising about 700,000 deliveries and 115,000 cesareans, resp.) concerning the routine prophylactic measures for acid aspiration syndrome (AAS) in pregnant patients and before obstetric procedures (i.e., prevention of aspiration by positioning, Sellick's manoeuvre, reduction of intragastric volume, or reduction of gastric content acidity). In pregnant patients, any prophylaxis of AAS is performed in 36%. Of the patients expecting delivery in the labour ward, only 7% receive pha

Oral intake policies on labor and delivery: a national survey.

STUDY OBJECTIVE: To examine current policies on oral intake during labor among hospitals throughout the United States. DESIGN AND SETTING: Anonymous questionnaire survey distributed to the directors of anesthesia and obstetrics departments of 740 hospitals. Completed surveys were then grouped by number of deliveries performed each year. MEASUREMENTS AND MAIN RESULTS: A total of 2,265 surveys were distributed. Of that number, 902 (33% response rate) surveys, representing 740 U.S. hospitals, were returned. Of the surveys returned, 419 surveys were received from obstetricians and 401 surveys we

Is antacid treatment necessary in obstetric anesthesia?

All the obstetric units in Jeddah were surveyed regarding the use of antacid prophylaxis and the methods of anesthesia used for emergency and elective cesarian section. The results were compared with the Western practice where marked variation was found but this apparently did not influence mortality from acid aspiration.

Eating and drinking in labor. A literature review.

This literature review questions the routine practice of denying food and fluids to women in labor. Fasting in labor, an established practice throughout the United States since the 1940s, is now under careful scrutiny. Many clinical practices, especially those that offer midwifery services, are currently instituting policies to allow and encourage eating and drinking in normal labor. To date, there have been no reported rises in maternal mortality with this policy change; neither have there been any reports of detrimental outcomes for mother or infant.
