One hundred and sixty-two Chinese women undergoing emergency Caesarean section were allocated at random on admission to the labour ward to receive one of three regimens for orally administered chemoprophylaxis against acid aspiration: ranitidine 150 mg 6 hourly with sodium citrate at induction of anaesthesia, omeprazole 40 mg 12 hourly with sodium citrate, or omeprazole 40 mg 12 hourly alone. Intragastric pH and volume were measured immediately after induction of anaesthesia. Ten patients (17%) in the omeprazole-only group, three (6%) in the omeprazole and citrate group and one (2%) in the ran

A five-year retrospective study of obstetric admissions to the Surgical Intensive Care Unit (SICU) in the National University Hospital, Singapore was carried out with the aim of determining the incidence, causes and outcome of these admissions. Most of the patients were admitted following emergency caesarean sections. Obstetric complications was the reason for admission in 56.8% with hypertensive disease of pregnancy being the major cause and haemorrhage accounting for the rest. Anaesthetic complications accounted for 21.6% of admissions and these included difficult intubation, aspiration pneu

Conflict exists between satisfying the parturient's desire for oral intake and traditional restrictive standards of obstetric and anesthesia departments. Surveys of institutions providing obstetric services reveal greatly varying oral intake policy. There is neither evidence of benefit in withholding fluids nor evidence of risk in allowing them. Prolonged fasting has potential liabilities. Maternal mortality is rare, and anesthesia-related causes are not among the common etiologies. Aspiration is not a significant factor in the modern era. Higher risk for anesthesia morbidity is associated wit

A postal survey was conducted to gain information on the use of policies on oral intake, selection of mothers and type of intake given during established labour by all maternity units in England and Wales. A response rate of 91.6% (351 out of 383) was achieved; 79.5% of units had a written policy for oral intake, 96.4% of units allowed mothers some form of oral intake and 68.3% of these units selected mothers according to risk categories. Of the 268 units allowing oral intake, 67.2% gave drink only and 32.8% drink and food. Of those allowing food, all but 13.6% had a selection policy. Of the 8

The effectiveness of sodium citrate as a prophylaxis against acid aspiration syndrome was studied in 3 groups of obstetric patients. Group I was the control group which consisted of 20 patients in established labour who were not likely to require caesarean section. No antacid had been given to these patients. Group II consisted of 20 patients who underwent elective caesarean section, while Group III consisted of another 20 patients who underwent emergency caesarean section. Group II and III were given 30ml of 0.3M sodium citrate as soon as they arrived in the operation theatre. The gastric con

Gastric pH and volumes were measured in 84 women who had general anaesthesia for emergency Caesarean section. Forty-eight received only 20 ml 8.4% sodium bicarbonate immediately before induction of anaesthesia and 36 ranitidine 150 mg 6-hourly during labour in combination with NaHCO3. Gastric pH was less than 2.5 in four women who received only bicarbonate. All aspirates from the ranitidine plus bicarbonate group had a pH greater than 2.5. Mean volumes of gastric content aspirated were 87 (SD 87.4) and 60 (SD 46.3) ml for the bicarbonate alone and bicarbonate plus ranitidine series respectivel

During 1987 a confidential survey of all hospitals in Australia providing obstetric services was undertaken to determine the antacid medications used routinely as prophylaxis against acid aspiration pneumonitis. Of the 567 hospitals surveyed, 379 (67%) responded. Of these, 243 hospitals provide an obstetric service which includes caesarean section, and 67% of these perform less than 500 deliveries per annum. Aspiration prophylaxis during labour was used in 22.4% of responding hospitals. Prior to elective caesarean section, 11.5% used no prophylaxis, and 39.4% used particulate antacids such as

Twenty patients who underwent elective Caesarean section received ranitidine 150 mg by mouth 8-14 h, and 50 mg i.m. 90 min, before surgery. Intraoperative gastric aspiration resulted in contents with a pH greater than 2.5 and volume less than 25 ml in all patients (mean pH 6.5 (SD 0.8); mean volume 9.0 (SD 7.2) ml). Sixty patients in labour, who received ranitidine 50 mg i.m. 6-hourly, underwent emergency surgery. Half of this group received, in addition, a single preinduction dose of either 15 or 30 ml of sodium citrate 0.3 mol litre-1. A further 30 patients who remained unmedicated during la

One hundred patients undergoing emergency Caesarean section were treated with cimetidine 200 mg intramuscularly when the decision was made to deliver the patient by Caesarean section and, preceding general anaesthesia, 30 ml of 0.3 M sodium citrate was administered orally. No routine antacid therapy was employed during labour. No patient had a gastric aspirate pH of less than 2.7, and only one of less than 3. The regimen is simple and effective, and treats only those patients at risk of acid aspiration.

High-risk pregnancies require specialized obstetric and anesthetic care. A basic understanding of how specific pathophysiology and pharmacologic therapy interact with anesthetic care is essential for both obstetrician and anesthesiologist. This paper selectively focuses on preeclampsia/eclampsia, diabetes mellitus, prematurity, multiple gestations, infectious disease, preexisting neurologic disease, and preexisting cardiac disease, reviewing anesthesia for labor and vaginal and cesarean delivery for each high-risk problem, as practiced at a Level III perinatal unit. Emphasis will be placed, wh

Maternal mortality from aspiration of gastric contents still remains unacceptably high, despite various recommended prophylactic measures. In order to establish which forms of antacid prophylaxis are currently being used, a questionnaire was sent to anaesthetists working in obstetric units in the United Kingdom. Despite its limitations, magnesium trisilicate remains the most popular antacid during labour and before Caesarean section.

Anesthesia in an emergency presents an additional risk to the mother and may have some possibly harmful effects on the stressed fetus. The risk to the mother can be reduced by using regional anesthesia, whenever reasonable, and by taking measures to minimize the risk of aspiration of vomitus. The stressed fetus can be assisted, if not by a rapid delivery, by giving the mother oxygen to oxygenate the fetus and possibly by improving intervillous perfusion with a regional anesthetic.

The efficacy of the non-particulate antacid 0.3 M sodium citrate in single and multiple dose regimes was investigated in 156 obstetric patients, both emergency and elective, who required operative delivery under general anaesthesia. Failure of either of these regimes to elevate the intragastric pH consistently to an acceptable level (greater than 3) led to investigation of a regime combining the histamine H2-receptor antagonist, ranitidine, and a single pre-anaesthetic dose of 0.3 M sodium citrate in a further 170 patients. Satisfactory pH values (greater than 3) were achieved in all patients

In a prospective controlled study 30 parturients provided for elective cesarean section were premedicated either with no specific medication for prophylaxis of aspiration pneumonia or 400 mg cimetidine orally at the evening and 400 mg intramuscularly two hours prior to induction of anesthesia. In the cimetidine treated group only one patient had a gastric pH below 2.5, while in the control group 11 patients had a pH below this limit. The gastric volume in the cimetidine treated group also was significantly reduced. No side effects could be observed in mothers and children. Application of intra

This paper is concerned with the aetiology, prophylaxis and treatment of the Mendelson-syndrome. It serves as a quick reference to anaesthesists and obstetricians who are faced with this problem and therefore helps them to decrease the maternal mortality resulting from regurgitation and aspiration of gastric juice.

Anaesthesia is now an important cause of maternal death. Most deaths which occur in association with anaesthesia are preventable. Mendelson's syndrome and hypoxic cardiac arrest account for almost all maternal anaesthetic deaths. Although routine antacid therapy in labour does not guarantee to prevent Mendelson's syndrome, the use of antacids is nevertheless fully justified. There remains the possibility of pulmonary irritation due to food particles, bile salts and even the antacid itself. The role of difficult or failed tracheal intubation is emphasized in the causation of hypoxia and the pul

Aspiration of gastric contents is the commonest cause of maternal anesthetic death. If the pH of the contents is below 2.5, morbidity and mortality rates are likely to be higher than if the pH is above 2.5. A study of 146 patients in labor shows that one in four patients is "at risk" to acid aspiration. No time interval between last mean and either onset of labor or delivery can guarantee an empty stomach. The use of oral antacids during labor reduces the number at risk to 1 in 35. Other methods by which the obstetrician may contribute to reducing the risk are discussed.
