A novel carotid artery catheterization device for intracranial drug delivery in rats and mice.

BACKGROUND: Intracarotid drug delivery in rodents is important for investigating neurological disorders and evaluating intra-arterial therapies. However, existing carotid catheterization tools are limited by large catheter size, complicated preparation, catheter instability, vascular injury, and catheter-related mortality, particularly in mice. NEW METHOD: We developed a simple, cost-effective carotid artery catheterization device for rats and mice. The device consists of ultra-fine polypropylene tubing, a 34 G needle, tissue-biocompatible adhesive, and a 1-mL syringe, and is compatible with

Hippocampal endoplasmic reticulum stress drives a pro-inflammatory microglial bias and neuroinflammation in aged mice with perioperative neurocognitive disorder.

BACKGROUND: Perioperative neurocognitive disorder (PND) is a frequent postoperative complication in older individuals and is commonly linked to microglial activation. Endoplasmic reticulum (ER) stress has been implicated in neuroinflammation and postoperative cognitive decline; however, whether hippocampal ER stress acts upstream to bias microglia toward a pro-inflammatory M1 phenotype, rather than serving only as a trigger of cytokine cascades, remains insufficiently defined in PND. To determine whether hippocampal ER stress promotes an M1-like microglial shift and amplifies neuroinflammation

Chronic, Battery-Free, Fully Implantable Multimodal Spinal Cord Stimulator for Pain Modulation in Small Animal Models.

Spinal cord stimulation (SCS) for chronic pain management is an invasive therapy involving surgical implantation of electrodes into spinal epidural space. While the clinical value and mechanistic action of the therapy is debated considerably in recent years, preclinical chronic studies employing rodent models can provide invaluable insights regarding the balance between efficacy and complications as well as mechanistic understanding of SCS therapy. However, current rodent compatible devices require tethered power delivery or bulky batteries, severely limiting the ability to probe long-term eff

Target anesthesia and surgery induced senescent cell via composite microsphere hydrogels as nose-to-brain amplifiers of platelet factor 4 delivery for neurocognitive dysfunction repair.

Perioperative neurocognitive disorder (PND) is a common complication in older surgical patients, leading to increased neurodegenerative and death risk, augment socioeconomic burdens. Despite its prevalence, the reasons of why this complication occurs highly in older, underlying pathogenesis mechanisms, and effective treatments remain unclear. Senescence associated secretory phenotype (SASP), resulting from cellular senescence, drives inflammaging and cognitive decline. However, the association between cellular senescent and poor cognitive outcome is seldomly defined in PND. Herein, we showed t

Blood-brain barrier injury and neuroinflammation in pre-eclampsia and eclampsia.

BACKGROUND: Cerebral complications of pre-eclampsia are a leading cause of maternal mortality. Better understanding of the pathophysiology may enable the development of novel strategies to protect the maternal brain. We aimed to investigate blood-brain barrier injury and neuroinflammatory pathways in women with eclampsia and pre-eclampsia compared to normotensive pregnancies. METHODS: This observational cross-sectional study conducted between March 2021 and June 2023, included women with eclampsia, pre-eclampsia, and normotensive pregnancies admitted to Tygerberg Hospital, Cape Town, South Af

Hippocampal Nogo66-NgR1 signaling activation restricts postsynaptic assembly in aged mice with postoperative neurocognitive disorders.

Postoperative neurocognitive disorders (pNCD) are a common neurological complication, especially in elderly following anesthesia and surgery. Yet, the underlying mechanisms of pNCD remain elusive. This study aimed to investigate the molecular mechanisms that compromise synaptic metaplasticity in pNCD development with a focus on the involvement of Nogo-66 receptor 1 (NgR1) in the pathogenesis of pNCD in aged mice. Aged mice subjected to anesthesia and laparotomy surgery exhibited anxiety-like behavior and contextual fear memory impairment. Moreover, the procedure significantly increased NogoA a

Anesthetic approach to pregnant patients with malaria: a narrative review of the literature.

INTRODUCTION: Anesthesiologists play an important role in the management of labor and delivery during acute malaria infection. The peripartum anesthesia considerations for such cases remain unclear. FINDINGS: Important peripartum considerations include the severity of thrombocytopenia and coagulopathy, hemodynamic status and cardiac disease, and the likelihood of central nervous system (CNS) involvement. Several antimalarial drugs may interact with perioperative medications, causing hypoglycemia, methemoglobinemia, or QT prolongation. Labor should usually not be induced. Patient volume status

Intrathecal catheter placement after inadvertent dural puncture in the obstetric population: management for labour and operative delivery. Guidelines from the Obstetric Anaesthetists' Association.

BACKGROUND: Anaesthetists of all grades who work on a labour ward are likely to be involved in the insertion or management of an intrathecal catheter after inadvertent dural puncture at some point in their careers. Although the use of intrathecal catheters after inadvertent dural puncture in labour has increased in popularity over recent decades, robust evidence on best practice has been lacking. METHODS: The Obstetric Anaesthetists' Association set up an expert working party to review the literature. A modified Delphi approach was used to produce statements and recommendations on insertion a

Extensive epidural spread of cerebrospinal fluid displacing the spinal cord after an inadvertent dural puncture in an obstetric patient.

We report a rare complication of an inadvertent dural puncture in an obstetric patient. A 24-year-old healthy primipara had a difficult neuraxial labor analgesia insertion. Subsequently she developed severe back pain and started having 'electric shock'-like sensations radiating from the spine to the lower extremities, raising a suspicion of a vertebral canal hematoma. Topping up the epidural for emergency cesarean section was unsuccessful and the surgery was done under general anesthesia. Subsequent emergency magnetic resonance imaging (MRI) of the spine showed no signs of bleeding but her sym

Neuraxial anesthesia in a parturient with common variable immunodeficiency: a case report.

Choosing whether or not to initiate neuraxial anesthesia in pregnant women with immune system defects may be challenging. Anesthesiologists have the responsibility of making the best decision in terms of anesthesia management for both mother and baby during the labor and delivery process. Whether neuraxial anesthesia is associated with an increased risk of central nervous system infection in immunocompromised compared with healthy patients is unknown. It is also unclear if maternal immune modulation required for fetal tolerance makes pregnant women susceptible to pathogens and causes an altere

Effect of neuraxial technique after inadvertent dural puncture on obstetric outcomes and anesthetic complications.

BACKGROUND: The aim of this study was to evaluate labor and delivery outcomes in parturients with inadvertent dural puncture managed by either insertion of an intrathecal catheter or a resited epidural catheter. METHODS: This was a retrospective cohort review of 235 parturients who had an inadvertent dural puncture during epidural placement over a six-year period. The primary outcome was the proportion of women with a delayed second stage of labor. Secondary outcomes were the proportion of cesarean deliveries, the proportion of cases resulting in post-dural puncture headache, and the incidenc

Transient Coma Due To Epidural Anesthesia: The Role of Loss of Sensory Input.

BACKGROUND: Epidural anesthesia is the most commonly used method of pain relief during labor in the USA. It is not classically associated with alterations in level of alertness. Coma during the procedure is rare, with a reported incidence of 0.1-0.3%. CASE REPORT: An otherwise healthy patient experienced almost complete loss of brainstem function following routine epidural anesthesia during delivery. The episode lasted for less than 3 hours and the patient made a full recovery. To our knowledge, this is the most detailed clinical observation to date of this condition. CONCLUSIONS: Clinicians

Prophylactic vs therapeutic blood patch for obstetric patients with accidental dural puncture--a randomised controlled trial.

Epidural blood patch is a standard treatment for obstetric patients experiencing a severe post-dural puncture headache. Patients who sustained an accidental dural puncture during establishment of epidural analgesia during labour or at caesarean delivery were randomly assigned to receive a prophylactic epidural blood patch or conservative treatment with a therapeutic epidural blood patch if required. Eleven of 60 (18.3%) patients in the prophylactic epidural blood patch group developed a post-dural puncture headache compared with 39 of 49 (79.6%) in the therapeutic epidural blood patch group (p

Measurement of maternal cerebral tissue hemoglobin on near-infrared time-resolved spectroscopy in the peripartum period.

AIM: To measure cerebral tissue hemoglobin in uncomplicated and complicated pregnant women during the peripartum period. METHODS: Time-resolved spectroscopy (TRS-20) can measure absolute concentration of oxygenated, deoxygenated, and total tissue hemoglobin based on the transit time of individual photons. Therefore, we used TRS-20 to measured tissue hemoglobin in the hemi-prefrontal lobes of normotensive pregnant women with (n = 51) or without (n = 19) epidural anesthesia, hypertensive pregnant women with pre-eclampsia (n = 10), a pregnant woman with acute onset of hypertension soon after del

Quality of labour neuraxial analgesia and maternal satisfaction at a tertiary care teaching hospital: a prospective observational study.

PURPOSE: Current labour analgesia practices are evidence-based; however, such evidence often originates in controlled trials, the results of which may not be readily applicable in the context of day-to-day clinical practice. The objective of this study was to evaluate the effectiveness of and maternal satisfaction with the neuraxial labour analgesia regimen provided at a tertiary care teaching hospital. METHODS: All women with a viable pregnancy who requested neuraxial analgesia for labour during November 2011 at our institution were approached to participate in this prospective study. Patien
