Maternal and neonatal outcomes in pregnant patients with Brugada Syndrome - a multicentre retrospective study (2014-2025).

BACKGROUND: Brugada Syndrome is an inherited channelopathy that predisposes to ventricular arrhythmias. Triggers include fever, enhanced vagal tone, electrolyte abnormalities and certain medications, including local anaesthetics and propofol. Multicentre studies evaluating anaesthetic management in conjunction with maternal and neonatal outcomes in pregnant patients with Brugada Syndrome are lacking. METHODS: In this retrospective cohort study, outcomes from 64 pregnancies in 57 patients with Brugada Syndrome across 12 maternity units in the UK and Ireland were reviewed (2014-2025). Outcomes

Complicated Epidural Analgesia With Undiagnosed Spinal Tumor: A Case Report.

Neuraxial anesthesia is commonly used for labor analgesia, and postpartum back pain is often experienced, with or without an epidural. Although it usually improves with supportive care, back pain coupled with other findings (such as radiculopathy, bowel or bladder dysfunction, or inability to ambulate) warrants further investigation post epidural. Here, we present a 37-year-old patient who experienced inadequate pain relief with multiple epidural blocks during labor and worsened back pain after delivery, and was later determined to have a lumbar myxopapillary ependymoma. Neuraxial anesthesia i

High Neuraxial Blockade in a Parturient After Epidural Placement: A Quality Improvement Simulation.

Background Epidural anesthesia is a commonly used procedure in obstetrics for managing labor pain, with a potential complication of high neuraxial blockade. Proficiency in performing epidural or spinal anesthesia and promptly recognizing and managing high neuraxial blockade are critical components of anesthesiology residency training. Methods A simulated case was designed involving a 21-year-old female in labor receiving epidural anesthesia. After the administration of local anesthetic, she develops symptoms of nausea, hypotension, and respiratory distress, progressing to high neuraxial blocka

A Hospital-Based Quality Improvement Initiative to Reduce Postdural Puncture Headache in Cesarean Deliveries.

BACKGROUND: Postdural puncture headache (PDPH) is a major complication of neuraxial anesthesia in cesarean deliveries. This study aimed to reduce PDPH incidence through a quality improvement initiative focusing on operating room (OR) efficiency and anesthetic management. METHODS: Using Plan-Do-Study-Act (PDSA) cycles, interventions included preoperative catheterization, synchronized scrubbing, and improved surgeon-anesthetist coordination. Spinal anesthesia protocols were refined with atraumatic needle techniques and standardized vasopressor use. Data from the pre-intervention (April to Octob

Labor Analgesia in a Patient With Beals Syndrome: A Case Report of Management Challenges.

Beals syndrome, also known as Beals-Hetch syndrome or congenital contractural arachnodactyly, is an autosomal dominantly inherited, rare connective tissue disorder characterized by flexion contractures, arachnodactyly, camptodactyly, severe kyphoscoliosis, and, less frequently, cardiovascular features. We describe the successful management of labor analgesia in a pregnant woman with Beals syndrome. During labor, a combined spinal/epidural technique was performed uneventfully, and intermittent top-ups were administered as needed at lower-than-usual volumes. Eutocic delivery occurred without com

Neurosurgical Challenges in Recurrent Adhesive Arachnoiditis After Spinal Anesthesia for Cesarean Delivery: Case Report and Literature Review.

Adhesive arachnoiditis (AA) is a rare, chronic inflammatory condition affecting the pia-arachnoid layers, characterized by fibrosis and adhesion formation. It leads to nerve root adhesion and spinal cord compression, often resulting in severe neurological impairment. Commonly associated etiologies include prior spinal surgeries, infections, subarachnoid hemorrhage, epidural anesthesia, myelography, contrast agents, chemical irritation, and, occasionally, idiopathic factors. Following the literature review, 12 cases of AA related to anesthesia and obstetric procedures were identified.  We repor

Chronic, Battery-Free, Fully Implantable Multimodal Spinal Cord Stimulator for Pain Modulation in Small Animal Models.

Spinal cord stimulation (SCS) for chronic pain management is an invasive therapy involving surgical implantation of electrodes into spinal epidural space. While the clinical value and mechanistic action of the therapy is debated considerably in recent years, preclinical chronic studies employing rodent models can provide invaluable insights regarding the balance between efficacy and complications as well as mechanistic understanding of SCS therapy. However, current rodent compatible devices require tethered power delivery or bulky batteries, severely limiting the ability to probe long-term eff

Management of High Spinal Anesthesia for Cesarean Section in the Semi-Fowler's Position.

High spinal anesthesia, a rare but serious complication of neuraxial techniques, occurs when local anesthetic spreads through the cerebrospinal fluid, blocking spinal segments above the intended level. This can lead to significant cardiovascular, respiratory, and neurological effects. These effects include vasodilation, bradycardia, and potential respiratory failure if the phrenic nerve or brainstem is affected. High spinal blocks are particularly concerning in obstetric anesthesia, where they are used for labor analgesia and cesarean deliveries. The ability to quickly and effectively manage t

Anesthetic Management of Parturients With Achondroplasia During Labor and Delivery: A Narrative Review.

Achondroplasia accounts for approximately 70% of all forms of dwarfism. Cesarean delivery is often required in parturients with achondroplasia due to cephalopelvic disproportion. There is no consensus on the optimal management for cesarean delivery considering the difficulties in both general and regional anesthesia in patients with achondroplasia. The aim of this study was to explore the literature for prior case reports and series to determine the optimum anesthetic management for cesarean delivery in achondroplastic patients. We conducted a review of the literature using Embase, Medline, Sc

Safe Anesthesia for Brugada Syndrome in Pregnancy: Insights From a Retrospective Case Series.

Introduction Brugada syndrome (BrS) is a cardiac channelopathy associated with sudden cardiac death and presents unique anesthetic challenges during pregnancy due to autonomic fluctuations and sensitivity to sodium channel-blocking agents. This study examines anesthetic strategies and outcomes in pregnant women with BrS. Methods We conducted a retrospective review of pregnant patients with confirmed or suspected BrS who delivered at our institution between 2012 and 2023. Data collected included ECG patterns, family history, anesthetic technique and agents, delivery details, and maternal and ne

Intraoperative Hypotension After Neuraxial Anesthesia: A Systematic Review and Meta-Analysis of Randomized Controlled Trials of Vasopressors for Cesarean Birth Stratified by Maternal Risk.

Postspinal hypotension is a frequent complication during cesarean delivery, particularly in high-risk pregnancies. Although vasopressors are routinely used for its management, their relative efficacy and safety after the onset of hypotension remain uncertain. Following PROSPERO registration (CRD420251050321) and in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines, this systematic review and meta-analysis evaluated the efficacy and safety of norepinephrine, phenylephrine, and ephedrine administered for treatment, rather than prophyl

Use of a Temporary Pacemaker During Cesarean Section in a Pregnant Patient With Neurally Mediated Syncope: A Case Report.

A 39-year-old pregnant woman with a history of neurally mediated syncope (NMS) and a documented eight-second sinus arrest on her Holter electrocardiography (ECG) was scheduled for cesarean delivery. A temporary pacemaker (PM) was inserted preoperatively to prevent severe bradycardia. Combined epidural and spinal anesthesia (CESA) was carried out. Intraoperatively, severe bradycardia triggered PM activation. Prompt administration of ephedrine and atropine sulfate stabilized her vital signs, and the PM was deactivated within two minutes. The cesarean section proceeded without further complicatio

Peripartum Anesthetic Care of a Parturient With Arnold-Chiari Malformation and Factor XI Deficiency: A Case Report.

Arnold-Chiari malformation (ACM) and Factor XI (FXI) deficiency are rare conditions that pose distinct anesthetic challenges in the obstetric population. While each condition has been managed individually with varying anesthetic approaches, their coexistence in a single patient significantly complicates peripartum decision-making, particularly regarding the safety of neuraxial anesthesia and the risk of hemorrhage. We report the case of a 26-year-old G2P1011 woman at 39 weeks of gestation with ACM type I and FXI deficiency who presented for induction of labor. Multidisciplinary planning involv

Anesthetic Management of a Pregnant Patient With Mixed Aortic Valve Disease Undergoing Cesarean Section.

Aortic stenosis is a condition with multiple etiologies that can remain clinically silent for years. The enhanced cardiovascular demands of pregnancy can unmask a previously undetected disease or worsen existing symptoms. Managing anesthesia in pregnant patients with cardiac valvular disease is a complex task that requires a coordinated, multidisciplinary approach. This case report presents a 28-year-old pregnant Hispanic patient with known severe aortic stenosis who was admitted to our hospital at 30 weeks of gestation for multidisciplinary management and delivery planning. Her only symptom w

The Delayed Presentation of Bilateral Subdural Hematoma Secondary to Epidural Anesthesia for Elective Vaginal Delivery: A Case Report and Review of the Literature.

Epidural anesthesia (EA) involves reaching the spinal epidural space with an anesthetic drug injection. This procedure provides pain relief during labor. Although EA can lead to some complications, subdural hemorrhage (SDH) is a rare adverse event associated with it. We report the case of a 25-year-old female patient who presented to our emergency department with a one-month history of headaches and associated blurred vision following a normal vaginal delivery with EA. She was initially treated as a case of post-dural puncture headache (PDPH), with no improvement. Finally, the diagnosis of bil
