Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Her pregnancy was complicated by

Early Palliative Care Consultation for Patients Requiring Extracorporeal Membrane Oxygenation.

OBJECTIVE: Obesity is one of the main concerns for public health and is becoming an increasingly widespread problem worldwide. Women are more likely to require a cesarean section and have a longer hospital stay after delivery. Excess body weight can interfere with ovulation and make it more difficult for embryos to implant in the uterus. A high body mass index (BMI) has controversial effects on the outcomes of medically assisted reproduction treatments (IVF) and, if careful counseling is not performed, medical-legal risks may be incurred. While some researchers argue that obesity does not part

Magnesium Toxicity in an Obstetric Patient Due to Preeclampsia-Related Renal Dysfunction Despite Administration of a Standard Pritchard Regimen: A Case Report.

Magnesium toxicity could occur in the treatment of preeclampsia/eclampsia, especially when the patient has coexisting renal impairment. If undetected and promptly treated, hypermagnesemia can cause severe fetomaternal complications and death. We present the case of a 22-year-old pregnant woman at 32 weeks of gestation, who was referred to our hospital with eclampsia, treated with intramuscular magnesium sulfate following a standard Pritchard regimen. An emergency cesarean section was carried out under general anesthesia due to multiorgan dysfunction. Following the cesarean delivery, she was su

Beyond Standard Algorithms: Diagnostic, Procedural, and Analgesic Challenges in Refractory Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) remains a leading cause of preventable maternal morbidity and mortality, with management often complicated by overlapping etiologies, delayed manifestations of hypovolemia, and procedural challenges related to analgesia availability. We report a case of recurrent PPH in a 24-year-old gravida 2, para 2 woman following spontaneous vaginal delivery without neuraxial anesthesia, in whom persistent bleeding refractory to uterotonic therapy was associated with discordant findings of a firm uterine fundus and a boggy lower uterine segment. Hemorrhage control was delayed by

Delayed Subaponeurotic Fluid Collection in an Infant Following Instrumental Delivery: A Case Report.

We report the case of a 70-day-old male infant who presented with a progressively enlarging, soft, fluctuant swelling over the vertex of the scalp, first noticed on day 43 of life. The infant was born via instrumental vaginal delivery following prolonged labor but had no signs of trauma or scalp swelling at birth. The child remained asymptomatic, with no fever, irritability, or feeding difficulties, and demonstrated normal neurological development. Physical examination revealed a non-tender, fluctuant swelling without overlying skin changes or signs of infection. A thorough differential diagno

Recurrent Bronchospasm During Intra-arterial Chemotherapy via IMAX Access for Retinoblastoma: A Case Report.

Intra-arterial chemotherapy (IAC) has become an increasingly popular approach for treating advanced retinoblastoma. It offers targeted drug delivery and reduced systemic toxicity when compared to systemic chemotherapy. Respiratory complications are rare, but this case presents a patient who experienced recurrent episodes of intraoperative bronchospasm possibly linked to an anatomical variation. A six-month-old female with Group D retinoblastoma in her right eye underwent five cycles of IAC within six months of diagnosis. Due to failed visualization of the right ophthalmic artery during angiogr

The Role of Local and General Anaesthesia in Modern Dental Practice: Clinical Decision-Making Frameworks, Patient Selection, Trends, Challenges, and Outcomes.

Anaesthesia is essential in modern dental practice for ensuring patient comfort and enabling safe and effective dental procedures. Increasing procedural complexity, a growing population of medically vulnerable patients, and higher expectations for safe outpatient care intensify the demand for anaesthetic approaches aligned with contemporary clinical standards. Despite substantial advancements in pharmacology and monitoring, inconsistencies persist in clinical decision-making, patient selection criteria, and the integration of safety standards across diverse dental settings, creating variabilit

Cost-effectiveness of prehabilitation for elderly (pre-)frail patients prior to elective surgery compared to standard care - an economic evaluation from a societal perspective.

BACKGROUND: Frailty syndrome is highly prevalent among elderly surgical patients, increasing their risk of experiencing perioperative complications and developing long-term disability. Prehabilitation may reduce these risks by optimising patients' physiological reserves prior to surgery. The PRAEP-GO trial, a multicentre randomised controlled outcome assessor-blinded trial conducted in Germany, evaluated the (cost-)effectiveness of prehabilitation for older (pre-)frail patients undergoing elective surgery compared to standard care. METHODS: The economic evaluation comprised (i) a cost-utility

High-Flow Nasal Cannula Oxygen Therapy in Perioperative Respiratory Care: Application and Prospects.

High-flow nasal cannula (HFNC) oxygen therapy is an oxygen delivery technique that provides warmed and humidified high-flow oxygen through a non-sealing nasal cannula. With multiple physiological advantages, HFNC had been increasingly used in perioperative respiratory management. Evidence indicates that HFNC significantly reduces the incidence of hypoxemia and decreases procedural interruptions during procedural sedation. As an updated HFNC technique, transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) had been proved to extend the safe apneic time and promote safety during

A Swift Solution: Early Removal of the JADA Device in the Management of Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) continues to be a major threat to maternal health worldwide. Current management strategies include uterotonic agents, intrauterine tamponade devices, and surgical interventions. These methods have variable efficacy, carry potential complications, and contribute to significant healthcare costs. The JADA device is an emerging and promising tool for managing PPH. We report the case of a 19-year-old G2P1011 at 39w3d who experienced uterine atony following a spontaneous vaginal delivery. Despite administration of Pitocin and tranexamic acid, the patient continued to have

Impact of an anaesthetist-led acute pain service for caesarean delivery as part of an enhanced recovery program: a retrospective before-and-after single-centre cohort study (2021-2023).

BACKGROUND: Postoperative pain after caesarean delivery is frequently moderate to severe, and insufficient analgesia may lead to chronic pain, prolonged opioid use, delayed functional recovery, and postpartum psychological distress. Structured postoperative pain management programmes may improve outcomes; however, they are rarely implemented in obstetric practice. This study evaluated the impact of an anaesthesialed acute pain service designed to standardise multimodal postoperative analgesia and early recovery processes. METHODS: We conducted a single-centre retrospective before-after cohort

Anaesthetic Management of Neurofibromatosis of an Unknown Subtype for Emergency Caesarean Section.

Neurofibromatosis is a heterogeneous group of inherited neurocutaneous disorders with variable multisystem involvement and important implications for obstetric anaesthesia. Anaesthetic concerns include potential airway involvement, occult spinal or intracranial tumours, cardiovascular instability, and the association with catecholamine-secreting tumours. These factors often lead to a preference for general anaesthesia, despite well-recognised maternal risks associated with general anaesthesia in obstetric practice. Decision-making is further complicated when neurofibromatosis is previously und

Maternal cardiac arrest: a simulation-based review of anesthetic protocols and perimortem cesarean delivery.

PURPOSE OF THE REVIEW: This review summarizes the current state of the art in maternal cardiac arrest and cesarean delivery and the rationale for and use of simulation to facilitate training in this rare but critical event. RECENT FINDINGS: Stabilization of the mother and consideration of the '4P's' and multidisciplinary management postarrest remain critical. Some key updates on management of maternal cardiac arrest include manual left uterine displacement and early/immediate preparation for resuscitative hysterotomy, with less focus on a 4-min window for intervention. Investigating the association between pain and psychological vulnerabilities and postpartum depression.

Pain and psychological vulnerabilities may be associated with the development of postpartum depression (PPD) in women undergoing Cesarean delivery. However, there is limited information regarding women undergoing labor and delivery. We investigated whether pre-delivery pain and psychological vulnerabilities, and other risk factors were associated with an increased risk of PPD among laboring women. This was a secondary analysis of a clinical trial conducted between June 2017 and July 2021. The risk of PPD was assessed using an online survey on Edinburgh Postnatal Depression Scale (EPDS) adminis

Recognition of intrapartum uterine rupture, anesthetic management, and maternal-fetal outcomes: strategies for rapid response.

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes. RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T
