INTRODUCTION: Clinical practice guidelines indicate general anesthesia should be avoided for cesarean deliveries, when possible. In the U.S. Military Health System, understanding multifaceted predictors of general anesthesia for cesarean deliveries can be used to enhance value-based obstetric care efforts.
MATERIALS AND METHODS: This retrospective observational cohort study included adult patients who experienced a cesarean delivery in a military treatment facility between January 2018 and October 2024. Potential covariates included patient characteristics, diagnoses, and pregnancy care infor

We report a case of a pregnant patient with dopamine-responsive dystonia (DRD; Segawa syndrome) who underwent cesarean delivery under epidural anesthesia. The patient continued levodopa therapy throughout pregnancy and the perioperative period without interruption. Anesthetic management was carefully tailored to prevent exacerbation of dystonic symptoms. Hemodynamic stability was maintained throughout surgery, and no perioperative neurological complications were observed. This case highlights the feasibility and safety of neuraxial anesthesia with continued dopaminergic therapy and underscores

Dysfibrinogenemia is a rare qualitative fibrinogen disorder associated with both bleeding and thrombotic risks, presenting unique diagnostic and management challenges in pregnancy. Physiological increases in fibrinogen during pregnancy may obscure underlying abnormalities, and reliance on functional assays alone can result in misinterpretation and inappropriate management. We present a case series of three pregnant patients in whom dysfibrinogenemia was either incidentally identified during antenatal care or recognised through prior family screening. All cases demonstrated persistently low fun

BACKGROUND: Maternal hypothermia frequently occurs after cesarean delivery under spinal anesthesia. We assessed whether active fluid warming would decrease the incidence of postoperative hypothermia as compared with no active warming in this context.
METHODS: In this randomized double-blind controlled trial, patients undergoing scheduled cesarean delivery with spinal anesthesia and intrathecal morphine were randomized to active intravenous fluid warming (intervention group, n = 35) or no active warming (control group, n = 35). Maternal temperature was continuously monitored with a zero-heat-f

Systemic mastocytosis (SM) is a rare clonal mast cell disorder in which physical, pharmacological, or emotional triggers can provoke massive mediator release and life-threatening anaphylactoid reactions. Pregnancy and delivery are recognized as high-risk periods that demand careful multidisciplinary planning. Cardiovascular manifestations of mast cell activation disease are increasingly recognized; however, their interplay with primary conduction abnormalities is poorly characterized, and the available evidence is largely limited to isolated case reports. We describe the elective cesarean deli

BACKGROUND: In resource-limited settings with few obstetric anesthesiology specialists, cardiac anesthesiologists may serve as alternative consultants for high-risk pregnancy with cardiovascular disease. We compared maternal outcomes in women with congenital or acquired cardiac disease undergoing cesarean delivery under the care of cardiac versus non-cardiac anesthesiologists.
METHODS: We conducted a retrospective cohort analysis of 283 pregnant women with cardiovascular disease who underwent cesarean delivery at a tertiary-care university hospital. Patients were managed by either cardiac ane

BACKGROUND: Brugada Syndrome is an inherited channelopathy that predisposes to ventricular arrhythmias. Triggers include fever, enhanced vagal tone, electrolyte abnormalities and certain medications, including local anaesthetics and propofol. Multicentre studies evaluating anaesthetic management in conjunction with maternal and neonatal outcomes in pregnant patients with Brugada Syndrome are lacking.
METHODS: In this retrospective cohort study, outcomes from 64 pregnancies in 57 patients with Brugada Syndrome across 12 maternity units in the UK and Ireland were reviewed (2014-2025). Outcomes

Cesarean delivery is commonly performed under spinal anesthesia because of its rapid onset, effective analgesia, and favorable maternal-fetal safety profile. Postspinal hypotension, however, remains a frequent complication and may result in maternal symptoms and impaired uteroplacental perfusion. Vasopressors such as phenylephrine, norepinephrine, and ephedrine are routinely used for prophylaxis, yet the optimal agent in low-risk parturients remains uncertain. This study compared the effectiveness and safety of commonly used prophylactic vasopressors in low-risk women undergoing elective cesar

Massive obstetric hemorrhage is a leading cause of maternal morbidity, especially in placenta accreta spectrum (PAS) cases. We report the anesthetic management of a 38-year-old woman undergoing emergency cesarean section complicated by hemorrhage exceeding 13 liters. Preoperative planning included risk assessment, blood product preparation, and a multidisciplinary approach. Intraoperatively, anesthesia was managed with combined neuraxial and general techniques, continuous hemodynamic monitoring, and a guided massive transfusion protocol. Despite the need for hysterectomy and complex surgical i

Euglycemic diabetic ketoacidosis (EDKA) is an uncommon but serious metabolic complication in pregnancy, where physiologic changes promote ketone production even without marked hyperglycemia. We report the case of an 18-year-old woman at 35 weeks and 4 days of gestation with type 1 diabetes mellitus and severe preeclampsia who developed intraoperative cardiac arrest during an emergent cesarean delivery. She had been nonadherent with insulin therapy for more than seven days before admission. Initial laboratory evaluation demonstrated metabolic abnormalities despite normoglycemia. During cesarean

Neuraxial anesthesia is commonly used for labor analgesia, and postpartum back pain is often experienced, with or without an epidural. Although it usually improves with supportive care, back pain coupled with other findings (such as radiculopathy, bowel or bladder dysfunction, or inability to ambulate) warrants further investigation post epidural. Here, we present a 37-year-old patient who experienced inadequate pain relief with multiple epidural blocks during labor and worsened back pain after delivery, and was later determined to have a lumbar myxopapillary ependymoma. Neuraxial anesthesia i

Spinal anaesthesia is the most commonly used anaesthetic technique for caesarean section due to its rapid onset, reliability, and favourable safety profile. Despite these advantages, spinal-induced hypotension remains a common complication with potential adverse effects on maternal and fetal well-being, including nausea, vomiting, dizziness, reduced cardiac output, and impaired uteroplacental perfusion. Phenylephrine remains the guideline-supported first-line vasopressor for spinal-induced hypotension during caesarean delivery because of its effectiveness in maintaining maternal blood pressure

Acute pancreatitis during pregnancy is an uncommon but serious condition associated with significant maternal and fetal morbidity. Hypertriglyceridemia-induced acute pancreatitis (HTG-AP) is a severe etiology that presents unique diagnostic and management challenges, particularly when complicated by preeclampsia. Diagnosis may be difficult because clinical manifestations can overlap with those of other obstetric emergencies. We report the case of a 30-year-old woman at 33 weeks' gestation with a history of type 2 diabetes mellitus and chronic hypertension who presented with severe abdominal pa

The coexistence of glucose-6-phosphate dehydrogenase (G6PD) deficiency and methemoglobinemia may produce misleading pulse oximetry readings, creating a significant diagnostic challenge during the perioperative assessment of oxygenation. Methemoglobinemia results from the oxidation of hemoglobin iron to the ferric state, impairing oxygen delivery and altering light absorption, often leading to falsely low oxygen saturation values. A 20-year-old man with confirmed G6PD deficiency and suspected chronic methemoglobinemia underwent an elective electrophysiologic study for recurrent supraventricular

BACKGROUND: Tissue contact optimizes pulsed field ablation (PFA), but existing methods, based on impedance, thermal response, or contact force, remain indirect. An endoscope-enabled PFA balloon catheter allows direct, real-time visualization of the electrode-tissue interface.
OBJECTIVES: The goal of this study was to evaluate the safety, lesion durability, and clinical outcomes of the novel, ultra-compliant PFA balloon catheter with direct tissue visualization.
METHODS: VISION AF (Pulmonary Vein Isolation With the Single Shot Endoscopic Ultra-Compliant Pulsed Field Ablation Balloon for the T

PURPOSE: Normal coagulation is essential for maternal safety during pregnancy and delivery. Thalassemia may influence coagulation parameters, however, its effects during pregnancy remain incompletely characterized. This study aimed to evaluate maternal coagulation profiles in pregnant women with thalassemia in South China.
PATIENTS AND METHODS: This retrospective observational study included 53 pregnant women with thalassemia and 352 pregnant women without thalassemia who delivered at a tertiary medical center in South China. Singleton pregnancies with gestational age of ≥ 37 weeks were analy

BACKGROUND: One in four surgical patients carries a drug allergy label, of which an estimated 90% are incorrect. Avoidance of first-choice drug therapies can lead to worse postoperative outcomes. We sought to determine the nature and extent of any association between drug allergy labels and postoperative complications.
METHODS: A multicentre observational study was performed in 21 UK National Health Service hospitals. Eligible patients were ≥18 yr of age and undergoing common surgical procedures: primary hip or knee replacement, internal fixation of closed long bone fracture, colorectal resec

Takayasu arteritis is a chronic inflammatory vasculitis primarily affecting the aorta and its major branches, such as the subclavian and common carotid arteries (CCAs). Because Takayasu arteritis predominantly affects young women of reproductive age, clinicians may encounter pregnant patients requiring complex perinatal management. In these patients, anesthesia for cesarean section is particularly challenging because peripheral blood pressure often fails to accurately reflect cerebral perfusion pressure due to extensive arterial stenosis. We report the anesthetic management of a 39-year-old pr

Serous retinal detachment (SRD) is an uncommon but vision-threatening manifestation of hypertensive disorders of pregnancy, particularly in the context of severe preeclampsia and hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome. We report two cases of young primigravid women who developed bilateral SRD associated with HELLP syndrome. Both patients presented with acute bilateral vision loss and were diagnosed using optical coherence tomography. Management included antihypertensive therapy, magnesium sulfate, and delivery without any ophthalmological intervention. Both

BACKGROUD: Management of pregnant patients with cardiovascular disease is a challenge, and limited experiences exist to date in the literature. Particularly, valve surgery is still at high risk, with a consistent rate of preterm birth, postnatal physical or mental disabilities, and fetal mortality.
CASE PRESENTATION: A 38-year-old woman, who was 37 weeks pregnant, was admitted to the emergency department with severe dyspnea and hypertension. A transthoracic echocardiography revealed a 4 cm mobile mass in the left atrium, attached to the interatrial septum by a stalk; the mass protruded into t
