Case Report: Image-guided adaptive lattice radiotherapy for a 2,772-cc ulcerative triple-negative breast cancer.

BACKGROUND: Giant ulcerative breast cancers (>2,000 cc) with active hemorrhage present a critical therapeutic challenge, as conventional radiotherapy carries a high risk of severe toxicity to surrounding organs. Lattice radiotherapy (LRT) delivers spatially fractionated high-dose vertices within a low-dose background, offering the potential to overcome dose constraints. However, rapid tumor regression induced by LRT may render fixed plans unsafe, necessitating adaptive replanning. CASE PRESENTATION: A 46-year-old female with stage IV triple-negative breast cancer (TNBC; BRCA1 germline mutatio

Case Report: Atypical cat-scratch disease presenting as referred shoulder pain and deep lymphadenopathy in a pediatric patient.

BACKGROUND: Cat-scratch disease (CSD) is a zoonotic infection attributable to Bartonella henselae. Typical clinical manifestations include regional lymphadenopathy accompanied by primary cutaneous lesions that develop following exposure to cats. Although most patients present with typical regional lymphadenopathy, a subset of patients may develop atypical manifestations, which can complicate diagnosis. Owing to the nonspecific clinical features, CSD is prone to misdiagnosis or diagnostic omission. This report describes a pediatric case of atypical cat-scratch disease characterized by initial s

Progressive Pancytopenia in the Setting of Severe Pneumonia in a Patient With a History of Mantle Cell Lymphoma: A Case Report.

Progressive pancytopenia in a patient with a history of hematologic malignancy presents a diagnostic challenge because active hematologic disease and severe systemic illness can produce overlapping findings. We report a 74-year-old woman with previously treated mantle cell lymphoma who presented with fever, altered mental status, neutropenia, and multifocal pneumonia. During hospitalization, she developed rapidly progressive pancytopenia, prompting consideration of active hematologic disease, infection and critical illness, medication effects, and consumptive processes. Peripheral blood flow c

Exceptionally Durable Response to First-Line S-1 Monotherapy in Metastatic Pancreatic Adenocarcinoma: A Case Report.

Pancreatic adenocarcinoma with distant metastases is associated with a poor prognosis, and exceptionally durable responses to systemic chemotherapy are uncommon. We report a rare case of pancreatic adenocarcinoma with radiologically suspected multiple liver metastases that achieved a prolonged response to first-line tegafur/gimeracil/oteracil (S-1) monotherapy. A 76-year-old man with a history of cerebral infarction and diabetes mellitus was referred to our department because of rapidly worsening diabetes. Imaging revealed an approximately 3-cm pancreatic head tumor and multiple hepatic lesion

When the Cure Turns Into a Culprit: Mesalamine and the Eosinophilic Lung.

We report a case of a 74-year-old man with a one-year history of ulcerative colitis in remission who developed low-grade fever, dry cough, progressive dyspnea, loss of appetite, and an eight-kg weight loss over 12 weeks. Oral mesalamine (2.4 g/day) had been initiated 12 weeks before presentation, with respiratory symptoms developing approximately three weeks after initiation. Despite initial antibiotic therapy, his symptoms progressively worsened, and he developed hypoxemia requiring hospitalization. Investigations revealed leukocytosis with marked peripheral eosinophilia, elevated C-reactive

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Quantifying Diagnostic Misclassification and Inpatient Mortality Determinants in Scrub Typhus: A Systematic Review and Interaction Meta-Analysis of Acute Undifferentiated Febrile Illness (AUFI) Cohorts in India.

Scrub typhus (ST), caused by Orientia tsutsugamushi, is an underrecognized but major etiology of acute undifferentiated febrile illness (AUFI) across India. Prior national syntheses were limited by selection biases arising from heterogeneous screening denominators. To establish an unbiased epidemiological baseline, we conducted a Preferred Reporting Items for Systematic Reviews and Meta‑Analyses‑compliant systematic review and meta‑analysis (registered with the International Prospective Register of Systematic Reviews) of observational studies published between January 2015 and June 2026, enfor

Retroperitoneal necrotizing fasciitis masquerading as perforated viscus: a rare case report.

INTRODUCTION AND IMPORTANCE: Necrotizing fasciitis is a rapidly progressive, life-threatening soft-tissue infection associated with high mortality, particularly when diagnosis is delayed. While most cases involve the extremities or perineum, retroperitoneal necrotizing fasciitis is exceedingly rare. Its deep anatomical location often results in vague clinical presentations, allowing extensive infection before overt signs develop, which poses significant diagnostic and therapeutic challenges. Early recognition and timely intervention are critical to improving outcomes. PRESENTATION OF CASE: We

Integration of surgical and Ayurvedic interventions in the treatment of Fournier's Gangrene: A Case Report.

Fournier's gangrene is a rare, life-threatening form of necrotizing fasciitis affecting the perineum and genital region. In Ayurveda, it correlates with Kotharoga, as both share symptoms such as severe tissue necrosis, pus discharge, and a foul odor. This case report discusses a 50-year-old male who presented with fever, severe scrotal pain, blackish discoloration, and purulent discharge two days after undergoing incision and drainage (I&D) of a perianal abscess. The case was diagnosed based on clinical examination and imaging studies. Broad-spectrum antibiotics, along with herbo-mineral medic

Suspected immune-mediated inflammatory myositis with rhabdomyolysis following yellow fever vaccination: a case report.

BACKGROUND: The yellow fever vaccine is a live-attenuated vaccine with an excellent safety profile; however, rare adverse events have been reported. Musculoskeletal complications, particularly inflammatory myositis with rhabdomyolysis, are exceedingly uncommon and not well characterized in the literature. CASE PRESENTATION: We report a case of a 49-year-old man who developed acute febrile illness, severe myalgia, and progressive proximal muscle weakness 1 week after receiving his first yellow fever vaccination. An extensive diagnostic evaluation was performed. The patient presented with high-

Severe Colchicine Toxicity After Ingestion of a Dose Below Traditionally Reported Lethal Levels: A Case Report.

Colchicine is a widely utilised drug for the management of conditions such as gout, pericarditis and familial Mediterranean fever. It has a narrow therapeutic index, with a significant risk of toxicity in overdose leading to life-threatening consequences. We present a case of severe colchicine toxicity following ingestion of 20 mg (0.26 mg/kg), resulting in multi-organ failure requiring intensive care management. Rapid deterioration with hyperthermia, electrolyte derangement, rhabdomyolysis, coagulopathy and significant bone marrow suppression necessitated IV antibiotics, platelet transfusion

Clinical features and early warning factors of symptomatic pulmonary embolism after total joint arthroplasty: a retrospective cohort study.

BACKGROUND: Symptomatic pulmonary embolism (PE) is a severe and life-threatening complication following total joint arthroplasty (TJA). Despite advances in diagnosis, treatment, and prediction, postoperative PE still occurs. This study aimed to investigate the onset time window, initial clinical presentations, and preoperative comorbidity profiles of patients with symptomatic PE after TJA, thereby providing clinical evidence for early identification and perioperative management of post-TJA PE. METHODS: In this retrospective study, patients who underwent TJA at our institution between January

Peri-procedural and early body temperature trajectories after endovascular thrombectomy and clinical outcomes in intensive care unit patients with acute ischemic stroke: a retrospective cohort study.

BACKGROUND: Body temperature after endovascular thrombectomy (EVT) is often summarized by single measurements or fever thresholds, which do not capture temporal patterns. We examined peri-procedural and early post-EVT temperature trajectories and their associations with 90-day outcomes in ICU patients with acute ischemic stroke. METHODS: This single-center retrospective cohort included 237 ICU patients undergoing EVT. Axillary temperature was recorded at nine prespecified time points from EVT initiation through 72 h. Patient-specific cubic curves were clustered using Gaussian mixture models.

Multiple Neurological Complications and Multidrug-Resistant Bacterial Infections During Prolonged Intensive Care for Undocumented Meningoencephalitis in Côte d'Ivoire: A Case Report.

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

African swine fever virus protein R298L opposes the NF-κB signaling pathway and interferon-mediated pathway.

African swine fever virus (ASFV) is a large DNA virus that is highly pathogenic to domestic pigs and severely threatens the global pig industry. ASFV has evolved a range of strategies to evade or suppress the innate antiviral immunity of the host in order to promote its own infection and proliferation. However, the molecular mechanisms underlying the inhibition of innate antiviral immunity by ASFV-encoded proteins remain unclear. Here, we report the ASFV-encoded protein R298L (p298L) inhibits inflammation and interferon (IFN)-mediated signaling pathways. To verify the function of pR298L in viv

Serologically supported pediatric brucellosis presenting with marked conjugated hyperbilirubinemia and marked systemic inflammation: A case report.

Brucellosis is a multisystem zoonosis that can mimic complicated bacterial infection in children. A 10-year-old girl presented with prolonged high-grade fever, jaundice, abdominal pain, anorexia and weight loss. The initial syndrome was managed empirically as an undifferentiated complicated bacterial illness because enteric fever and a hepatobiliary or intra-abdominal source were considered. CRP was 333.2 mg/L and total/direct bilirubin 7.24/5.21 mg/dL despite only mild aminotransferase elevation; ultrasonography showed hepatomegaly and nonspecific gallbladder-wall edema without biliary dilata

Long noncoding RNA associations with an IFI27-MCEMP1 transcript pattern in Kawasaki disease.

Kawasaki disease (KD) is an acute pediatric vasculitis with risk of coronary artery injury. A two-transcript IFI27-MCEMP1 score has distinguished KD from other febrile illnesses, but its relationship to long noncoding RNAs (lncRNAs) is incompletely defined. We evaluated whether peripheral blood mononuclear cell (PBMC) lncRNA expression differed between KD and febrile controls and was associated with the IFI27-MCEMP1 transcript pattern. Children younger than 8 years were prospectively enrolled at the Children's Hospital of Fudan University during 2024-2025. The newly enrolled cohort included 55
