Single-Application Biobrane Versus Nanocrystalline Silver Dressings for Bilateral Mid-Dermal Petrol Flash Burns: An Intra-Patient Split-Limb Case Report.

Biobrane (Smith & Nephew, Hull, UK) is a biosynthetic bilaminate dressing used for superficial partial-thickness burns, where it provides single-application coverage until re-epithelialisation. Comparative data in adults with mixed-depth burns are limited, and within-patient comparisons are rare. We report a man in his early 50s with type 2 diabetes mellitus who sustained 21% total body surface area (TBSA) circumferential mixed superficial and mid-dermal burns to both lower limbs in a petrol flash explosion. At primary debridement the left limb was covered with a single application of Biobrane

Beyond the Classic Triad: A Case of GQ1b Antibody-Positive Bickerstaff Brainstem Encephalitis Overlap Syndrome.

Anti-GQ1b antibody syndrome encompasses related postinfectious neurologic disorders, including Miller Fisher syndrome, Guillain-Barré syndrome, and Bickerstaff brainstem encephalitis. Recognizing overlap among these phenotypes can be difficult in the intensive care unit because profound unresponsiveness and loss of brainstem reflexes may be attributed to sedation or structural brain injury. A middle-aged man developed perioral numbness, blurred vision, ataxia, extremity paresthesias, and facial weakness after an upper respiratory infection. His condition progressed rapidly to respiratory failu

The association between social capital and mental health among older people living with HIV: a systematic review.

BACKGROUND: Older people living with human immunodeficiency virus (HIV) constitute a vulnerable group in society and are at high risk of developing mental health problems. Although strengthening social capital is a common recommendation to reduce such mental health risks, no systematic review has examined the link between social capital and mental health among older people living with HIV. METHODS: We conducted a systematic review of peer-reviewed studies examining the association between social capital and mental health among adults aged ≥50 years living with HIV. Seven databases were search

Molecular epidemiology of carbapenemase-producing gram-negative bacilli in diabetic foot infections: association with glycemic control.

BACKGROUND: Carbapenem-resistant Gram-negative bacilli are an increasing threat in diabetic foot ulcer (DFU) infections, limiting therapeutic options and worsening outcomes. However, data linking specific carbapenemase genes to clinical and metabolic factors remain limited. OBJECTIVES: To isolate Gram-negative bacilli from deep tissue biopsies of DFUs, determine carbapenem susceptibility by minimum inhibitory concentration (MIC) testing, detect major carbapenemase genes by multiplex qPCR, and evaluate their association with glycemic control and Wagner grade. METHODS: This cross-sectional stu

Cavitary Lung Lesion as the Initial Presentation of Diffuse Large B-Cell Lymphoma: A Case Report With Review of the Literature.

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma and may rarely present with pulmonary involvement, creating significant diagnostic challenges. We report the case of a 69-year-old man who presented with a persistent cavitary right lower lobe lung lesion initially treated as recurrent pneumonia. Despite multiple courses of antimicrobial therapy, his symptoms persisted, and extensive infectious etiology evaluation was unrevealing. Bronchoscopic biopsy ultimately demonstrated aggressive DLBCL. Subsequent evaluation revealed small bowel involvement, establis

Assessment of endothelial glycocalyx degradation in sepsis using direct visualization with sublingual microscopy.

BACKGROUND: The endothelial glycocalyx plays a crucial role in the pathophysiology of sepsis, but assessment of glycocalyx injury in clinical settings remains challenging. Perfused boundary region (PBR) is derived from sublingual intravital video microscopy and has been studied as a surrogate of endothelial glycocalyx barrier function. The goal of this study was to determine whether PBR is associated with circulating heparan sulfate (a biochemical marker of glycocalyx degradation), sepsis status, in-hospital mortality, and intravenous (IV) fluid administration. METHODS: This was an ancillary

Matrix metalloproteinase-guided targeting of vesicular stomatitis virus to tumors following systemic administration.

The lack of tumor-specific targeting by oncolytic viruses (OVs) restricts their therapeutic potential. Here, we report a "cap-linker" strategy to modify the vesicular stomatitis virus (VSV) glycoprotein (G protein, VSV-GIN). The modified VSV (Pro-VSVIN) loses its ability to infect cells until present in the tumor microenvironment, where the linker is cleaved by tumor-enriched matrix metalloproteinases (MMPs). When Pro-VSVIN is armed with a single-chain IL-12 (Pro-VSVIN-IL-12) and systemically injected into MC38 tumor-bearing mice, Pro-VSVIN-IL-12 eradicates established tumors while avoiding sy

Pembrolizumab-Associated Adrenal Insufficiency Presenting as Adrenal Crisis: A Case Report.

Immunotherapy regimens containing the programmed cell death protein 1 (PD-1) inhibitor pembrolizumab are widely utilised in the management of non-small cell lung cancer (NSCLC). However, the administration of pembrolizumab is associated with unique immune-related adverse events. Adrenal insufficiency is a rare, potentially life-threatening endocrinopathy that can manifest even after the cessation of planned treatment. A male in his 50s with metastatic pulmonary adenocarcinoma developed an acute adrenal crisis 12 days after completing a 24-month course of pembrolizumab immunotherapy. Routine ou

Patient-Centered Transition to Clean Intermittent Catheterization After a Sacrectomy for Sacral Chordoma: A Case Report and Narrative Review.

Sacral chordoma may cause progressive neurological and pelvic organ dysfunction, while definitive surgery can require sacrifice of sacral nerve roots and result in permanent neurogenic bladder. We report a 57-year-old woman with an S2-S4 sacral chordoma presenting with severe lumbosacral pain and voiding dysfunction. Initial imaging demonstrated a 70 × 50 × 42 mm lesion extending into the sacral canal and presacral space. On day three, she underwent a limited decompressive resection of the intraspinal component to relieve root compression and obtain tissue for diagnosis. Histopathology and imm

Syphilis Screening in Pregnancy During Emergency Department Gonorrhea and Chlamydia Testing.

INTRODUCTION: Congenital syphilis is rising in the United States, especially among infants born to women with limited prenatal care. Emergency departments (ED) offer opportunities for early detection, particularly among patients evaluated for urogenital sexually transmitted infection (STI) concerns. In October 2019, our health system implemented an ED STI order set that promoted HIV co-testing and included a preselected option for syphilis testing. In this study, we evaluated change in syphilis testing during the post-implementation period among ED encounters with both pregnancy and gonorrhea/

"It's Like a Whole Set of Things That We Do": A Qualitative Study of the Roles of Journal Editors-in-Chief.

INTRODUCTION: Journal editors-in-chief (EICs) play a central role in shaping scholarly publishing, yet little is known about how they understand and enact their roles. We explored how medical education journal EICs describe their roles in shaping the publication process. METHODS: We conducted a qualitative study using thematic analysis of interviews with nine medical education journal EICs. The interviews were part of a broader investigation of publication timelines and explored editorial leadership and practices, including experiences during the COVID-19 pandemic. Sex-divergent trends in cardiorespiratory endurance after the COVID-19 pandemic: a 21-year interrupted time series analysis of Chinese university students.

INTRODUCTION: Studies of COVID-19 and physical fitness in Chinese university students rarely extend beyond 2022, and none has formally tested whether post-pandemic trends differ by sex. METHODS: Using 21 years of routinely collected fitness data (2005-2025), we examined indicator-specific change and sex differences in population-level trends in a repeated cross-sectional design. The cardiorespiratory analytic sample comprised 297,565 valid records from 91,686 full-time undergraduates at one comprehensive university in Zhejiang Province, China. Six indicators were classified by data availabili

Comparative histopathological and immunohistochemical features of central nervous system and pulmonary tuberculosis: an exploratory retrospective study.

Central nervous system tuberculosis (CNS-TB) is a rare and particularly severe form of extrapulmonary tuberculosis (EPTB). Early diagnosis and precise treatment are crucial in managing this condition. Pathological and immunohistochemical features may differ between CNS-TB and pulmonary tuberculosis (PTB). This study included 9 patients of CNS-TB, and analyzed their clinical manifestations, pathological features, and the immunohistochemical(IHC) features. The findings were then compared with PTB to explore the similarities and differences in how tuberculosis manifests in the brain versus the lu

Investigation of the effect of simulated gastrointestinal digestive fluids on the infectivity of influenza A virus H1N1.

The recent emergence of highly pathogenic avian influenza (HPAI) in dairy cattle has raised concerns about potential non-respiratory exposure routes including ingestion of contaminated raw milk. In this study, we assessed the survival of influenza A virus (IAV) in milk during simulated human digestion and estimated infection risk using quantitative microbial risk assessment (QMRA). Whole milk artificially inoculated with IAV A(H1N1) was exposed to human saliva, fed-state simulated gastric fluids (pH 3-6) containing pepsin, and fed-state simulated intestinal fluids containing pancreatin, in the

Giant Mixed-Material Gastric Bezoar With a Synchronous Jejunal Bezoar in a Young Woman With Pica and Autism Spectrum Disorder: A Case Report.

Bezoars are conglomerates of indigestible material retained within the gastrointestinal tract and may produce obstruction, ulceration, bleeding, or perforation. Mixed-material bezoars containing nonfood foreign bodies are uncommon and may be particularly challenging in patients with persistent pica. We report a 21-year-old woman with autism spectrum disorder (ASD), mild intellectual disability, previously diagnosed pica, and a seizure disorder treated with carbamazepine who presented after nearly one month of recurrent fever, colicky abdominal pain, progressive distension, early satiety, const

Are both hemolysis and impaired myocyte integrity associated with glucose-6-phosphate dehydrogenase deficiency?

Glucose-6-phosphate dehydrogenase (G6PD) is a potent antioxidant enzyme. G6PD deficiency, predominant among males, is a neonatal "neurotoxic" risk, characterized by episodic hemolysis that is sometimes infection triggered. Clinical risk among heterozygous G6PD-deficient females is often under-recognized. Recent reports of rhabdomyolysis among G6PD-deficient individuals are reviewed here. In one case, a male athlete with G6PD deficiency (<10% activity) experienced multiple, acute episodes of rhabdomyolysis, painful muscle symptomatology, injury and hemolysis after strenuous exertion. Both his m

Construction of an infectious clone of tomato leaf curl New Delhi virus and development of a duplex assay through recombinase polymerase amplification-lateral flow dipstick to detect the virus and tomato yellow leaf curl virus simultaneously.

The co-infection of tomato yellow leaf curl virus (TYLCV) and tomato leaf curl New Delhi virus (ToLCNDV) in the same host complicates disease diagnosis and management, not to mention that each virus alone can cause devastating losses to its respective host crops across many tomato- and cucurbit-growing areas. However, diagnosis of mixed viral infections in the same host plants can be tedious, costly and challenging. This study aims to establish a rapid, sensitive, and specific duplex recombinase polymerase amplification-lateral flow dipstick (RPA-LFD) assay to simultaneously detect TYLCV and/o
