Invasive Infections Caused by Streptococcus pseudoporcinus: A Two-Case Series and Review of the Literature.

Streptococcus pseudoporcinus (S. pseudoporcinus) is a beta-hemolytic, Gram-positive, catalase-negative, nonmotile coccus that primarily colonizes the female genitourinary tract. Although it has historically been considered a colonizing organism, it has increasingly been associated with invasive infections. Its identification may be challenging because of its phenotypic similarity to other beta-hemolytic streptococci. We report two invasive infections caused by S. pseudoporcinus. The first occurred in a 59-year-old woman with cirrhosis who developed spontaneous bacterial peritonitis in the sett

The use of dupilumab in bronchiolitis obliterans after hematopoietic stem cell transplant.

BACKGROUND: Bronchiolitis obliterans is a serious pulmonary manifestation of chronic graft-versus-host disease (cGVHD) following hematopoietic stem cell transplant (HSCT). Bronchiolitis obliterans is typically treated with a combination of inhaled steroids, azithromycin, montelukast and immunosuppressants. There are no curative treatments other than lung transplant, however, and additional therapies remain an unmet need. CASE PRESENTATION: Here we report two patients who developed bronchiolitis obliterans following HSCT. Both developed strong type 2 inflammatory markers including high eosinop

Cost-Effectiveness of HCV Screening in Emergency Departments Using DETECT Hep C Trial Results.

IMPORTANCE: Nontargeted hepatitis C virus (HCV) screening in emergency departments (EDs) has been demonstrated to identify more patients with HCV than targeted screening. Cost-effectiveness is key to strategy selection; however, the cost-effectiveness of these strategies in EDs remains unknown. OBJECTIVE: To compare clinical outcomes and cost-effectiveness of 2 novel ED-based HCV screening strategies with no ED HCV screening. DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation study, a cohort of individuals presenting to the ED in the US was simulated with data from the Determinin

Genotype-phenotype analysis in 333 individuals with heterozygous variants in NFKB1.

Heterozygous germline variants in NFKB1 can be classified into variants with "known protein defect" and "unknown protein effect." However, beyond this, NFKB1 variants may be additionally categorized into at least eight distinct variant classes. We conducted a literature review of 67 papers reporting on NFKB1 variants and included four additional individuals, carrying previously unreported NFKB1 variants. We identified 333 individuals with unique variants in NFKB1. Our compiled data revealed that there is no sex predilection, the penetrance-including all heterozygous carriers-was 78%, and the m

Surveillance Data Use Through the Fleming Fund: Informing Action on Antimicrobial Resistance in Low- and Middle-Income Countries.

BACKGROUND: Drawing on the Fleming Fund's work to strengthen antimicrobial resistance (AMR) surveillance across Africa and Asia during 2018-2026, this article provides one of the first syntheses of AMR surveillance data use in low- and middle-income countries. METHODS: A participatory synthesis for narrative review was conducted to summarize experience across administrative levels (facility, subnational, national, and regional). RESULTS: Data use was most evident at the facility level, in human health systems where resistance patterns, antimicrobial use, and clinical data increasingly inform

Biotic response networks are an important organizer of the transcriptome in wild Arabidopsis thaliana populations.

Extensive laboratory experimentation has revealed conserved molecular pathways controlling growth and stress responses in plants, yet how these programs operate in natural settings remains poorly understood. We have investigated the organization of the transcriptome in wild populations of Arabidopsis thaliana by sampling plants across 60 natural sites in Europe and North America over two seasons. Transcriptomes varied extensively among individuals and were only weakly structured by geography or season. No groups of plants stood out due to clear biotic or abiotic stress signatures known from th

Post-translational modifications in the insoluble fraction of platelet-poor plasma distinguish pre-COVID-19 Postural Orthostatic Tachycardia Syndrome, Long COVID, and Long COVID-POTS: a preliminary study.

BACKGROUND: Pre-COVID-19 Postural Orthostatic Tachycardia Syndrome (PC-POTS), Long COVID with initial orthostatic hypotension (LC-IOH), and Long COVID-POTS (LC-POTS) are primarily chronic post-viral conditions with debilitating symptoms, despite routine laboratory findings often being normal. We previously identified amyloidogenic fibrinaloid microclot complexes (FMCs) in the insoluble fraction of platelet-poor plasma (PPP-IF) in Long COVID, but it remains unclear whether similar abnormalities occur in PC-POTS and LC-POTS, or whether post-translational modifications (PTMs) contribute to diseas

Myroides bacteremia associated with wound myiasis: A case report and focused literature review.

Myroides is a gram-negative rod that most commonly affects immunocompromised hosts. Traditional risk factors for infection include soil or water exposure, but one less well-described risk factor is myiasis: the infestation of living tissue by maggots. We present a 50-year-old man with end-stage liver disease and chronic lower-extremity wounds who developed Myroides bacteremia in association with nonsterile maggot infestation. A literature review of PubMed, Ovid (Embase and MEDLINE), Web of Science, and Scopus identified three other cases of Myroides infection in the setting of myiasis. Myroide

Vedolizumab treatment in adults with immunodeficiency-associated enteropathy: a case series.

BACKGROUND: Inflammatory gastrointestinal disease is a common manifestation of inborn errors of immunity (IEI) and may affect any segment of the gastrointestinal tract. Vedolizumab, a gut-selective anti-integrin approved for inflammatory bowel disease, has been scarcely studied in IEI. We describe our experience with vedolizumab in adults with immunodeficiency-associated enteropathy. METHODS: We conducted a retrospective case series of adults with humoral or combined immunodeficiency and inflammatory gastrointestinal disease treated with vedolizumab at a single tertiary referral center. Spatiotemporal trends in leptospirosis in the Philippines: a 10-year national surveillance analysis in a One Health and climate-related context.

BACKGROUND: Leptospirosis remains an important zoonotic disease in the Philippines, yet its long-term national and regional epidemiological patterns and implications for surveillance remain insufficiently characterized. This study aimed to (1) characterize temporal and spatial trends in reported human leptospirosis incidence in the Philippines from 2015 to 2024; (2) situate Philippine trends within the global epidemiological context; (3) interpret national and regional patterns through a One Health-informed perspective; and (4) examine their implications for surveillance and public health poli

Anticipating El Niño-related health risks in Latin America and the Caribbean: from climate information to public health preparedness.

El Niño is a recurrent mode of tropical Pacific climate variability with major public health implications for Latin America and the Caribbean. Its development is partly driven by the Bjerknes feedback, a coupled ocean-atmosphere process in which weakened equatorial trade winds reduce coastal upwelling and allow warm surface waters to shift eastward. Warming of the central and eastern tropical Pacific can reshape regional rainfall and temperature patterns, increasing the likelihood of floods, droughts, wildfires, food-system disruption, and health-service interruption in locations with establis

Characterizing Oseltamivir Use Among Community-Dwelling Patients Diagnosed With Influenza Virus Infection, 2023-2025.

BACKGROUND: Oseltamivir is an antiviral medication for influenza that can reduce the duration of symptoms and may lower the risk of some complications. Recommendations for use of oseltamivir include in the outpatient setting for individuals at higher risk of developing influenza complications. The objectives of the current study were to describe oseltamivir initiation and treatment completion among influenza-positive outpatients and identify factors associated with each. METHODS: In a US outpatient household transmission study, index participants with laboratory-confirmed influenza provided u

Adult Presentation of a Large Congenital Pulmonary Airway Malformation Discovered After Traumatic Rib Fractures.

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung lesion that is usually identified prenatally or in early childhood, and adult presentations are uncommon. A 44-year-old woman presented after a fall with right-sided chest pain and dyspnea. CT chest revealed nondisplaced fractures of the right seventh through ninth ribs and incidentally identified a large multilocular cystic lesion in the right lower lobe consistent with CPAM, unchanged from imaging performed 15 years earlier. Pulmonary function testing demonstrated mild small-airway dysfunction, air trapping, and red

Extracellular vesicles released by Candida albicans yeast cells impact candidiasis lethality in insect and murine models.

Extracellular vesicles (EVs) are nano-sized lipid bilayer structures released by diverse cell types. For pathogenic fungi, EVs play critical roles by modulating immune responses and influencing disease progression. Previously, we demonstrated that vaccination with EVs from Candida albicans promoted protection in immunosuppressed mice subsequently infected with lethal doses of this organism. However, the impact of EVs in the presence of fungi is poorly investigated in vivo. Here, we demonstrated that co-injection of C. albicans yeasts and their EVs into Galleria mellonella larvae led to a rapid
