Does Suture Choice Matter? Vicryl versus Monocryl in Subcutaneous Fat Closure.

Wound complications following lower segment cesarean section (LSCS) contribute significantly to postoperative morbidity, prolonged hospital stay, and patient dissatisfaction. The choice of suture material for subcutaneous fat closure plays a crucial role in wound healing outcomes. Absorbable sutures such as polyglactin 910 (Vicryl) and poliglecaprone 25 (Monocryl) differ in their tensile strength, absorption profile, and tissue reactivity. We report 50 cases of a primigravida undergoing LSCS in whom subcutaneous fat was closed with Vicryl for 25 patients, and Monocryl was used for subcutaneous

Higher postoperative complication rates in patients with systemic lupus erythematosus undergoing surgical and obstetric procedures: a nationwide retrospective matched cohort study.

Population-level evidence on postoperative outcomes in systemic lupus erythematosus (SLE) remains limited. To compare postoperative outcomes between patients with SLE and matched controls and identify factors associated with complications and algorithm-defined disease reactivation. We conducted a nationwide retrospective matched cohort study using DATASUS data. Patients with SLE were matched 1:5 to controls by sex, age group, geographic macro-region, preoperative surgical risk, and surgical category. Postoperative outcomes were assessed over 180 days. Multivariable analyses assessed factors as

Low-lying placenta is associated with increased risk of hemorrhage and severe maternal morbidity: A nationwide analysis.

BACKGROUND: Placenta previa (PP) is a well-recognized obstetric complication with established maternal risks, particularly hemorrhage, that contribute significantly to maternal morbidity. In contrast, low-lying placenta (LLP) has historically been grouped with previa in both coding and clinical practice, limiting our ability to study its imparted associated outcomes as a distinct entity. However, in 2017, coding revisions allowed for a parsed diagnosis of LLP and PP and enabled the current study of differentiated associated outcomes. OBJECTIVES: To describe and compare the independent rates o

Alpine dwarf shrubs on green roofs: species-specific physiological responses to substrate depth and drought intensity.

BACKGROUND AND AIMS: Green roofs can enhance urban biodiversity when local, functionally diverse species are used. However, extreme roof conditions require species selection based on plant eco-physiological characteristics. METHODS: This study evaluated the performance of four Alpine dwarf shrubs (Arctostaphylos uva-ursi, Vaccinium myrtillus, V. vitis-idaea, Calluna vulgaris) in green roof plots with 10 and 30 cm substrate under natural conditions (temperate climate) and compared it with the drought-tolerant shrub Salvia officinalis as a reference. Seasonal and daily variations in leaf temper

Early Palliative Care Consultation for Patients Requiring Extracorporeal Membrane Oxygenation.

OBJECTIVE: Obesity is one of the main concerns for public health and is becoming an increasingly widespread problem worldwide. Women are more likely to require a cesarean section and have a longer hospital stay after delivery. Excess body weight can interfere with ovulation and make it more difficult for embryos to implant in the uterus. A high body mass index (BMI) has controversial effects on the outcomes of medically assisted reproduction treatments (IVF) and, if careful counseling is not performed, medical-legal risks may be incurred. While some researchers argue that obesity does not part

Birth preparedness and complication readiness knowledge, practices and its associated factors among recently delivered women: A cross-sectional study in Bharatpur, Chitwan, Nepal.

BACKGROUND: Birth preparedness and complication readiness (BPCR) is a comprehensive strategy, aimed at ensuring that expectant mothers and their families are for normal delivery and preparing for potential complications that may arise during pregnancy, labor, delivery, and the postpartum period without any delays. BPCR interventions are widely promoted by government and international agencies to reduce maternal and neonatal health risk in developing countries like Nepal. Studying BPCR also helps identify gaps in knowledge, access, and practices, guiding interventions to strengthen health syste

Perceived Safety in Childbirth: Pregnancy Predictors of Women's Experiences During Childbirth.

BACKGROUND: Women's perceived safety during childbirth contributes to their childbirth experience, which can impact mental health and the experience of future pregnancies. Unexpected birth events may predict negative experiences of childbirth, but there is limited evidence about the role of demographic, health, and psychological factors known during pregnancy. The aim of this paper was to model pregnancy predictors of women's perceived safety during childbirth. METHODS: Women (n = 313) < 20 weeks' gestation were recruited from a large maternity hospital in Melbourne, as part of the Mercy Preg

The psychological impact of childbirth: Unscheduled cesarean delivery associates with increased risk for acute stress response.

INTRODUCTION: Cesarean delivery is one of the most common surgical procedures in the United States. Despite their frequency and medical necessity, the acute psychological response associated with unscheduled cesarean delivery is unknown. This study aimed to assess the rates and nature of peritraumatic stress reactions during and shortly after unscheduled cesarean delivery, and whether acute stress predicts later mental health symptoms. METHODS: A total of 1146 patients receiving routine perinatal care at a single tertiary, urban hospital were assessed for peritraumatic stress reactions during

"Every Centimetre Counts" The Association Between Cesarean Scar Size and Acute Postoperative Pain: A Prospective Cohort Study.

OBJECTIVES: To determine the relationship between cesarean delivery (CD) incision size, and acute postoperative pain and longer-term outcomes. METHODS: This prospective cohort study was conducted at a tertiary hospital in Ontario, Canada from 2021 to 2023. Participants were women aged 18-45 years who delivered via low transverse cesarean incision. CD incision size was reported as a continuous exposure per centimetre, and further dichotomized into CD incision size <15 cm and CD incision size ≥15 cm. The primary outcome was pain via numeric rating scale (NRS) on postoperative day (POD) 1, with

Obstetric Sutures Gone Astray: A Bladder Erosion of a Cerclage Suture Leading to Recurrent Urinary Tract Infections.

Cervical cerclage is generally considered a safe procedure; however, rare complications, such as suture erosion and migration into adjacent organs, can occur. We present a case of a 31-year-old multiparous female who presented with a three-year history of recurrent urinary tract infections (UTIs), dysuria, urinary frequency, intermittent gross hematuria, and pelvic pain. She had a history of two prior cervical cerclages. Multiple courses of antibiotics failed to resolve her symptoms. CT imaging revealed a curvilinear hyperdense foreign body within the bladder. A cystoscopy was performed. Intra

"My heart was in my stomach": Texas-based obstetricians' and gynecologists' experiences post anti-abortion legislation.

BACKGROUND: The purpose of this study is to investigate the downstream effects of anti-abortion legislation on the practice of obstetrics and gynecology in urban Texas. METHODS: We conducted semi-structured interviews until saturation with eleven obstetricians/gynecologists (OBGYNs) to explore the effects of Texas’s abortion laws. Analysis consisted of inductive and deductive approaches. RESULTS: We identified three main themes regarding the downstream effects of Texas abortion laws: the conflicting role of colleagues as a support system and a source of frustration and distrust for OBGYNs, the

Impact of repeat cesarean sections on surgical site infections, microbiological patterns, and surgical outcomes: a prospective multicenter cohort study in South Ethiopia.

BACKGROUND: Cesarean section (CS) is a lifesaving procedure, yet its rising global use raises concern about adverse maternal outcomes. Although a substantial body of evidence suggests that prior cesarean delivery is associated with an increased risk of intraoperative and postoperative complications, including surgical site infections (SSI), data from prospective comparative cohort studies remain limited. This study was therefore conducted to examine this association and generate evidence to inform context-appropriate preventive strategies and maternal health interventions in low-resource setti

Satisfaction and experience of care among women with abortion-related complications from Africa and Latin America and the Caribbean.

BACKGROUND: Although abortion is a frequent event in women’s reproductive life and abortion-related complications have negative consequences for women’s health and wellbeing, little is known about the quality of abortion care. To improve the quality, women’s experience of care and satisfaction, need to be further standardized and studied. This study aims to assess experience and satisfaction with post-abortion care in Africa, and Latin America and the Caribbean (LAC). METHODS: We performed a secondary analysis from the World Health Organization Multi-Country Survey on Abortion. This analysis u

Race, ethnicity, and nativity differences in pregnancy and birth outcomes among Medicaid-enrolled women with disabilities.

BACKGROUND: Women with disabilities have worse maternal health outcomes than women without disabilities, but we have a limited understanding of other disparities they face. OBJECTIVE: Investigate whether differences in pregnancy and birth outcomes vary by race, ethnicity, and nativity among low-income women with disabilities. METHODS: We conducted a retrospective cohort study of 24,631 live births to Medicaid-enrolled women with disabilities in Texas. We used linked administrative claims with birth certificate data to examine outcomes, including cesarean delivery, severe maternal morbidity (

Intranasal low-dose dexmedetomidine alleviates propofol-associated postoperative sleep disturbance in female hysteroscopy patients: a randomized controlled trial.

BACKGROUND: Postoperative sleep disturbance (PSD) significantly affects early recovery and is commonly associated with general anesthetics. This study investigated the incidence of PSD after propofol exposure and the preventive effect of low-dose dexmedetomidine. METHODS: In this randomized, controlled trial, female patients undergoing hysteroscopy were allocated to receive intranasal dexmedetomidine (0.2 µg/kg or 0.5 µg/kg) or saline before propofol-based intravenous anesthesia. The primary outcome was the incidence of PSD via the brief version of the Pittsburgh Sleep Quality Index (B-PSQI) o

Ultrasound assessment of true fetal head descent for optimizing operative vaginal delivery.

BACKGROUND: Accurate assessment of fetal head descent during labor is fundamental to the safe performance of operative vaginal delivery. However, conventional evaluation by digital vaginal examination is subjective and prone to interobserver variability. Although intrapartum transperineal ultrasound has improved objectivity by quantifying the presenting part of the fetal head, these assessments may not reliably reflect the vertical level of the largest fetal head diameter, particularly in the presence of fetal head molding and caput succedaneum. The suprapubic descent angle was recently introd
