Comparative study of two protocols of induction of labor with DILAPAN-S devices in a situation of intra-uterine fetal death or medical termination of pregnancy.

BACKGROUND: Practices in induction of labor for medical termination of pregnancy or intra uterine fetal death are controverted. DILAPAN-S dilators are often used as an adjunction of oral prostaglandins to reduce the time of delivery, but protocols may differ between practicians. At the University Hospital of Saint-Etienne, the DILAPAN-S were applied the evening before induction until 2020, and the same day as misoprostol intake since 2022. OBJECTIVE: To evaluate these two protocols of induction of labor with DILAPAN-S, on the day before and on the same day as prostaglandins, on the reduction

LIFT: Lifting the Impacted fetal head; the Fetal Pillow® and Tydeman® Tube trial: protocol for a prospective observational study to evaluate two disimpaction devices.

BACKGROUND: Impacted fetal head (IFH) at full dilatation caesarean section (CS) is an increasingly recognised obstetric emergency associated with significant maternal and neonatal morbidity. Despite its importance, there is limited evidence to guide management, particularly the use of disimpaction devices. METHODS: LIFT is a prospective observational trial comparing the use of two commercially available disimpaction devices-the Fetal Pillow® and the Tydeman® Tube -for the management of IFH at full dilatation caesarean section. DISCUSSION: The LIFT study will compare the performance of the Fe

Increased Primary Cesarean Births Among People with Epilepsy: A Population-Based Cohort Study.

OBJECTIVE: Pregnant people with epilepsy are at increased risk of severe maternal morbidity (SMM). Cesarean birth is a known contributor to SMM; however, the factors driving cesarean rates in this population remain poorly understood. We evaluated whether primary cesarean births were increased among nulliparous, term, singleton, vertex (NTSV) live births in people with epilepsy and whether differences were explained by sociodemographic, clinical, or hospital factors. MATERIALS AND METHODS: This cohort study used linked data from NTSV live births in California from 2007 to 2018. International C

Transradial access in interventional gynecology and obstetrics: Current evidence, clinical applications, and future directions.

Transradial access (TRA) has emerged as a transformative approach in interventional cardiology and is increasingly being adopted in peripheral vascular interventions, including gynecological and obstetric procedures. Traditionally performed via transfemoral access (TFA), uterine artery embolization (UAE) and other pelvic interventions require prolonged bed rest and carry inherent access-site complications. This review synthesizes current evidence on the feasibility, safety, and clinical efficacy of TRA in interventional gynecology and obstetrics, covering UAE for fibroids, management of obstet

A Swift Solution: Early Removal of the JADA Device in the Management of Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) continues to be a major threat to maternal health worldwide. Current management strategies include uterotonic agents, intrauterine tamponade devices, and surgical interventions. These methods have variable efficacy, carry potential complications, and contribute to significant healthcare costs. The JADA device is an emerging and promising tool for managing PPH. We report the case of a 19-year-old G2P1011 at 39w3d who experienced uterine atony following a spontaneous vaginal delivery. Despite administration of Pitocin and tranexamic acid, the patient continued to have

Perineal body length as a predictor of severe perineal lacerations in a Turkish obstetric cohort.

This study aimed to investigate the association between perineal body length (PBL) and the occurrence of severe perineal trauma, defined as third- and fourth-degree lacerations, in a Turkish obstetric population. A prospective observational study was conducted at a tertiary maternity hospital between April 2024 and March 2025. A total of 495 women meeting predefined eligibility criteria were included. PBL was measured once during the first stage of labor at 4 cm cervical dilation using a sterile paper tape. Maternal demographic, obstetric, and neonatal characteristics were prospectively collec

Relationship between continuous maternal intrapartum and postpartum glucose monitoring metrics and neonatal outcomes and postpartum glucose metabolism in individuals with gestational diabetes.

BACKGROUND: Continuous glucose monitoring (CGM) improves maternal and neonatal outcomes in pregnancies complicated by preconception diabetes. CGM measurements may help optimize intrapartum and postpartum glycemic management in pregnant women with GDM. We investigated the use of blinded CGM-derived metrics during labor, delivery, and postpartum supplementary to normally monitored glucose measures in women with GDM to study neonatal outcomes and postpartum glucose metabolism. METHODS: This prospective observational study evaluated 60 pregnant women with gestational diabetes mellitus (GDM) during

The amniotic fluid lactate level as a possible diagnostic tool for uterine fatigue, leading to prolonged labour.

OBJECTIVES: Arrested labour is a common and dangerous complication of delivery leading to adverse outcomes for both the mother and child. Early identification of slow labour is essential for managing the condition. Currently, clinical assessment consists of vaginal examination, abdominal palpation, and the use of a partogram, but there are no well-described, objective and quantifiable markers. One possible cause of labour arrest is uterine fatigue. The aim of the study was to assess the feasibility of using lactic acid concentration in amniotic fluid as a marker of uterine fatigue contributing

Contemporary Approach to Postpartum Hemorrhage: Early Diagnosis and Evidence-Based Therapeutic Management.

Postpartum hemorrhage remains a major obstetric emergency and one of the most important contributors to maternal morbidity and mortality worldwide. Effective management depends on early recognition, systematic evaluation, and rapid implementation of evidence-based therapeutic strategies. A structured clinical approach facilitates identification of the underlying cause and supports timely intervention to prevent progression to severe hypovolemia and organ dysfunction. Risk assessment should begin during the antenatal period, as several maternal and obstetric conditions increase the likelihood o

Selective episiotomy guided by a perineal evaluation form for forceps-assisted vaginal delivery: a retrospective cohort study on maternal and neonatal outcomes.

BACKGROUND: Forceps-assisted vaginal delivery is prone to perineal laceration and other complications. Episiotomy decisions are often experience-dependent, leading to excessive intervention. The Perineal Evaluation Form (PEF) provides an objective basis for episiotomy, but its clinical efficacy in forceps-assisted vaginal delivery has not been fully validated. METHODS: A single-center retrospective cohort study was conducted on 313 parturients who underwent forceps-assisted vaginal delivery at the Third Affiliated Hospital of Zhengzhou University from January 2017 to December 2024, divided int
