Prevalence of dural puncture headache after caesarean section at a tertiary hospital in the Gambia.

BACKGROUND: Following a caesarean section performed under spinal anaesthesia, a common complication is post-dural puncture headache (PDPH). Spinal anaesthesia has become the most common anaesthetic procedure during caesarean section in our practice. Therefore, knowing the prevalence and risk factors of PDPH will inform practice and add value in our obstetrics practice. METHODOLOGY: A cross-sectional study was conducted at Edward Francis Small Teaching Hospital Banjul and data was effectively collected from August to October 2023. A structured data collection tool was used. The data was entere

Unanticipated Hypoxemia in Late Pregnancy Revealing Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) complicates pregnancy due to pulmonary arteriovenous malformations (PAVMs), which risk rupture from hemodynamic changes. A 26-year-old woman at 32 weeks' gestation with refractory cough, dyspnea, and hypoxemia was diagnosed with HHT based on recurrent epistaxis, telangiectasias, and an ENG deletion. Chest computed tomography revealed multiple PAVMs with a shunt fraction of 26%. Caesarean section at 33 weeks ensured maternal and fetal safety. Persistent postpartum hypoxemia necessitates endovascular embolization and home oxygen therapy. This case unde

Birth Outcomes in the Hispanic Population in the United States: Trends, Variation, and Determinants (2011-2021).

Infants born to mothers who self-identify as Hispanic account for a substantial and growing share of births in the United States, yet limited research has examined disparities in birth outcomes across Hispanic origin subgroups. This study aims to document trends and identify important factors associated with Cesarean section (C-section), low birthweight, and prematurity within the Hispanic population. We use data from the National Vital Statistics System (2011-2021), covering nearly all U.S. births. We compare outcomes across Hispanic, non-Hispanic White, and non-Hispanic Black mothers and fur

Association of cesarean section with breast cancer: A systematic review and meta-analysis.

Breast cancer (BC) remains the most prevalent malignancy among women globally, and its potential associations with reproductive factors have been extensively investigated. Among these, the role of the cesarean section (CS) as a potential risk factor has garnered increasing attention. The present systematic review aimed to evaluate the association between CS and BC risk, as well as its prognostic implications. A comprehensive literature search was conducted following Systematic Reviews and Meta-Analyses (PRISMA) guidelines across multiple databases, including PubMed, Scopus, Web of Science, Coc

Trends and determinants of early neonatal mortality in Ethiopia: evidence from the Ethiopian demographic and health survey data.

BACKGROUND: Early neonatal mortality (ENM) accounts for three-fourths of all neonatal deaths and one-third of overall child mortality. While previous studies have primarily focused on neonatal and under-five mortality, ENM has received comparatively less attention. This study aims to address this gap by analyzing trends and identifying factors associated with ENM in Ethiopia. METHODS: Data from the Ethiopian Demographic and Health Survey (EDHS) conducted between 2005 and 2016 were analyzed, comprising a total weighted sample of 22,310 participants. To account for the hierarchical structure of

Maternal, fetal and neonatal outcomes among pregnant women with arthrogryposis multiplex congenita: a scoping review.

BACKGROUND: The rarity of pregnancies in women with arthrogryposis multiplex congenita (AMC) could lead to healthcare providers having limited exposure to these cases. Consequently, they may be less familiar with the possibilities and challenges associated with pregnancies in women affected by AMC. AMC is an umbrella term for a disorder with multiple contractures at birth, having a broad spectrum of causes, onset and severity of expression. A clinical classification describing the phenotype is Group 1 with primary limb involvement, Group 2 with musculoskeletal involvement plus other system ano

Perinatal outcome of vaginal breech delivery in Orotta National Referral Teaching Hospital, Eritrea, 2024; a case control study.

INTRODUCTION: Delivery of fetus in breech presentation is a controversial topic in obstetrics and it had been the subject of debate since 1980s. As experts with abilities and know-how to perform vaginal breech delivery have decreased, and medico legal concerns increased, some physicians endorse cesarean section for breech presentation at term. The aim of this study was to identify the perinatal outcome in vaginal breech deliveries. METHODOLOGY: This study was a retrospective case control study of mothers delivered vaginally between January 1, 2019 and December 31, 2023 in Orotta National Refe

MOTA syndrome diagnosis following unexpected neonatal death.

The Manitoba Oculo-Tricho-Anal (MOTA) syndrome was first described in 1992. MOTA syndrome is a rare syndrome. MOTA syndrome has a clear genetic autosomal recessive inheritance pattern. The MOTA syndrome is related to FRAS1 (Fraser extracellular matrix complex subunit 1) related extracellular matrix 1 (FREM1) gene mutation, which can also cause genitourinary defects (including renal agenesis), nasal abnormalities and anorectal abnormalities. In our case, a pregnant woman whose fetus had been diagnosed during a routine mid-trimester ultrasound scan with unilateral renal agenesis and no other str

Effectiveness and evidence-based practices in maternal care bundles for labour and birth: a systematic review protocol.

INTRODUCTION: Worldwide, maternal mortality is mostly caused by avoidable or treatable health complications, despite the progress made in maternal health. High-quality care focuses on a standardised and patient-centred approach. In this light, care bundles, which are sets of evidence-based practices, were introduced to improve the quality of care and maternal outcomes. The existing reviews on care bundles focus on specific conditions, and there is, to our knowledge, no comprehensive systematic review on the effectiveness and evidence-based character of care bundles on multiple maternal outcome

Predicting Postpartum Hemorrhage Using Clinical Features Extracted With Large Language Models.

OBJECTIVE: To evaluate whether large language models (LLMs) applied to prenatal clinical notes can predict postpartum hemorrhage (PPH) before the onset of labor and to compare model performance across outcome definitions, including a novel intervention-based definition. METHODS: We conducted a retrospective cohort study within a large regional health network. Two outcome definitions for PPH were used: 1) estimated or quantitative blood loss (EBL-QBL) extracted from clinical notes; and 2) a clinical intervention-based PPH definition (cPPH) designed to capture significant hemorrhage requiring i

Elective single morula transfer achieved better clinical outcomes than two cleavage embryos transfer in fresh IVF-ET cycles.

BACKGROUND: Assisted reproductive technology (ART) effectively treats infertility yet often results in multiple pregnancies. Optimizing clinical pregnancy rates while minimizing multiples remains challenging. Key considerations for ART providers include embryo transfer timing, number, and developmental stage. While double cleavage-stage and single blastocyst transfers are common, limited data exists on the intermediate strategy of single morula transfer. OBJECTIVE: This research aimed to assess whether elective single morula transfer (eSMT) on Day 4 yields clinical outcomes comparable to doub

Understanding the impact of antibiotic treatment on the diversity of gut microbiota species.

INTRODUCTION: The community of microorganisms that colonize the intestine plays a vital role in regulating human metabolism and immune system function. According to translational medicine studies, administering antibiotics disrupts the balance of microorganisms in the gastrointestinal system, increasing the risk of multiple diseases. Dysregulation and reduction of the biodiversity of intestinal microbiota increase the risk of developing unhealthy conditions for the host. In this review, we tried to offer a current vision regarding the use of antibiotics and the modification of the intestinal m
