Microfluidic loading of verteporfin into extracellular vesicles for neuroblastoma therapy.

Despite contributing to cancer progression, extracellular vesicles (EVs) could serve as potential drug delivery systems in cancer treatment, having the ability to dissolve water-insoluble drugs and facilitate targeted delivery. However, the clinical translation of EVs is still in its infancy. While traditional methods for EV modifications will remain relevant, microfluidic approaches are expected to replace benchtop methods. Taking advantage of lab-on-chip devices, passive cargo loading through microfluidic mixing and incubation may be an important strategy to produce functional engineered EVs

Incidence of Uterine Cesarean Scar Niche After Cesarean Delivery and Assessment of Its Risk Factors.

Background and Objectives: A cesarean scar defect (CSD), also referred to as a niche or isthmocele, is often detected incidentally during transvaginal sonography (TVS) and is typically asymptomatic. However, the exact prevalence of symptomatic niches remains unclear. This study aimed to evaluate the incidence of cesarean scar niches and identify potential risk factors in a prospectively gathered cohort of Egyptian women. Materials and Methods: The primary endpoint was to determine the incidence of isthmoceles after six months following a cesarean section (CS) and to investigate any associated

Tissue-specific composite hydrogel-loaded BMSCs inhibit apoptosis and promote meniscal repair.

The meniscus, as a weight-bearing tissue, will accelerate cartilage damage after injury and cause osteoarthritis. Currently, there is enthusiasm for using tissue engineering techniques to bionically fabricate grafts to replace meniscus. However, the high toughness and mechanical strength mediated by the specific arrangement of collagen-I fibers in the meniscus is an intractable challenge. Our study utilized decellularized meniscal extracellular matrix (mECM) in combination with GleMA to form a composite hydrogel for delivery of the BMSCs to regenerate the injured meniscus in situ. mECM has the

Does gestational age influence the predictive accuracy of the cerebroplacental ratio for intrapartum fetal compromise?

BACKGROUND: The accuracy of the cerebroplacental ratio (CPR) in predicting cesarean section for intrapartum fetal compromise (CS-IFC) prior to the onset of labor remains controversial. OBJECTIVES: To determine whether advancing gestational age (GA) in the final weeks of pregnancy enhances the predictive performance of CPR and other sonographic parameters for CS-IFC before labor. STUDY DESIGN: This multicentre retrospective study analysed 590 singleton pregnancies across four tertiary centres in Spain, Italy, and the UK. All participants underwent Doppler ultrasound assessment between 35+0 an

Exploring Proteases as Alternative Molecular Targets to Tackle Inflammation in Cystic Fibrosis Respiratory Infections.

Cystic fibrosis (CF) is characterized by chronic respiratory infections and excessive inflammation, driven by both host- and pathogen-derived proteases. The dysregulated activity of proteolytic enzymes such as neutrophil elastase (NE), cathepsin G, and matrix metalloproteases (MMPs) degrades lung tissue, exacerbates airway remodeling, and perpetuates inflammatory cycles. Concurrently, bacterial proteases from pathogens such as Pseudomonas aeruginosa and Staphylococcus aureus contribute to immune evasion and tissue destruction, compounding disease severity. Despite advances in antimicrobial and

Neonate With Necrotic Skin Lesions and Middle Cerebral Artery Stroke in the Context of Fetal Vascular Malperfusion.

There are few published reports of neonates born with necrotic skin lesions and stroke. We report the case of a neonate with an identified placental etiology for necrotic skin lesions at birth and multiple arterial thrombi, including a stroke. This neonate was born to a generally healthy mother with gestational hypertension on labetalol in pregnancy. She was born via emergency Caesarean section at 38 + 1 weeks' gestational age due to biophysical profile 2 of 8 and oligohydramnios and was noted to have multiple necrotic skin lesions on the left forearm. She was found to have numerous arterial t

Nanoparticle platform preferentially targeting liver sinusoidal endothelial cells induces tolerance in CD4+ T cell-mediated disease models.

INTRODUCTION: Treating autoimmune diseases without nonspecific immunosuppression remains challenging. To prevent or treat these conditions through targeted immunotherapy, we developed a clinical-stage nanoparticle platform that leverages the tolerogenic capacity of liver sinusoidal endothelial cells (LSECs) to restore antigen-specific immune tolerance. METHODS: In vivo efficacy was evaluated in various CD4+ T cell-mediated disease models, including preventive and therapeutic models of myelin oligodendrocyte glycoprotein-induced experimental autoimmune encephalomyelitis (EAE), ovalbumin-sensit

Impact of Environmental Determinants of Health on Access to Surgical Care.

Environmental determinants significantly impact the health of individuals and populations contributing to increased rates of traumatic injuries, higher cancer incidence, and disruptions in access to healthcare systems and surgical services. Additionally, the healthcare industry contributes significantly to climate change, negatively impacting the earth's environment in both developed and developing nations. Natural disasters such as hurricanes and wildfires cause destruction of hospitals, clinics, pharmacies, and even supply chain facilities, hindering patient access to surgical care. These im

Allosteric targeted drug delivery for enhanced blood-brain barrier penetration via mimicking transmembrane domain interactions.

Current strategies for active targeting in the brain are entirely based on the effective interaction of the ligand with the orthosteric sites of specific receptors on the blood-brain barrier (BBB), which is highly susceptible to various pathophysiological factors and limits the efficacy of drug delivery. Here, we propose an allosteric targeted drug delivery strategy that targets classical BBB transmembrane receptors by designing peptide ligands that specifically bind to their transmembrane domains. This strategy prevents competitive interference from endogenous ligands and antibodies by using

Evaluating digital nudge interventions for the promotion of cancer screening behavior: a systematic review and meta-analysis.

BACKGROUND: Public adherence to cancer screening remains low and is influenced by both rational and non-rational factors, including decision biases that underestimate screening benefits. Digital nudge interventions have shown promise in promoting screening behaviors among at-risk populations, but systematic evidence is still lacking. This study aims to synthesize the effects of digital nudge interventions on promoting cancer screening behaviors in high-risk individuals. METHODS: A systematic search of 10 electronic databases was conducted, and studies published before April 1, 2024, were incl

Emo@KP MBs Modulates the TGF-β1/Smad Signaling Pathway by in situ Micro-Nano Conversion to Reduce Renal Inflammation and Fibrosis Caused by Unilateral Ureteral Obstruction.

INTRODUCTION: Emodin alleviates renal interstitial fibrosis (RIF) and reduces inflammation by inhibiting the TGF-β1/Smad pathway, thus improving CKD outcomes. However, its clinical use is limited due to poor solubility and side effects. This study developed a targeted drug delivery system using αKIM-1 modified microbubbles carrying Emodin to enhance accumulation in renal tissues with high KIM-1 expression. METHODS: Emo@KP MBs were characterized by TEM and DLS, and their drug loading and encapsulation rates were measured by UV-VIS-NIR spectroscopy. Biocompatibility was assessed in vitro with H

Race and sex differences in placental lipid metabolism are associated with spontaneous early preterm birth†.

Placental dysfunction is implicated in the pathogenesis of spontaneous preterm birth (SPTB). We investigated race (self-identified maternal race) and fetal sex differences in the placental metabolome and transcriptome associated with early SPTB (<32 weeks). Long-chain polyunsaturated fatty acids, acylcarnitines, acylglycerols, plasmalogens, and lysophospholipids were remarkably different between SPTB and Term placentas. These alterations were much more profound in Black than in White SPTB placentas. Mode of delivery and body mass index (BMI) had no effect on these differences. The lipid metabo

Artificial Mitochondrial Nanorobots Deliver Energy In Vivo by Oral Administration.

Delivering energy in vivo is essential for treating mitochondrial damage-related diseases. Current methods, including natural mitochondrial transplantation and artificial energy delivery systems, lack non-destructive, external energy-free, and clinically viable potential solutions. Here, artificial mitochondrial nanorobots (AMNs) carrying high-energy phosphate bonds rebuild the in vivo energy supply system to provide energy. Using ischemic heart disease (IHD) as an energy-deficient disease model and the oral route, which has high patient compliance and facilitates long-term administration, to

Evidence of reduced birthweight in Ukraine following the Russian invasion.

The impact of armed conflict on birth weight has been shown in various studies, but the effects of the full-scale Russian invasion of Ukraine in 2022 have been overlooked until now. This study investigates the influence of war on birth-weight using data and follow-up of pregnancies among women having prenatal screening. Maternal factors, outcomes, and birth weights were analyzed for 706 screened women who delivered between 2020 and 2023, excluding cases of spontaneous abortion, termination, and multiple pregnancies. Of these, 330 deliveries occurred before the invasion and 376 afterward. The o
