Universal digital mental health interventions for children and youth: a scoping review.

INTRODUCTION: Digital mental health interventions (DMHIs) are increasingly used to support child and youth mental health, yet the scope and characteristics of universal (Tier 1) DMHIs remain poorly defined. METHODS: This scoping review synthesized peer- reviewed studies of universal DMHIs delivered to children and youth aged 0-18 years. RESULTS: Use of online programs and hybrid delivery were prevalent. Many programs used an independent structure, while facilitation varied across self- led, facilitator-led, and both-led models. Outcomes were commonly assessed across multiple domains, includi

Pathological diagnosis of transposition of the great arteries as a contributor to neonatal mortality.

BACKGROUND: Transposition of the great arteries (TGA) is a rare cyanotic congenital heart defect, classified into complete TGA and corrected TGA. Late presentations are common in the majority of complete TGA patients alongside ventricular septal defect (VSD) or atrial septal defect (ASD). Complete TGA without accompanying cardiac malformations exhibits low morbidity and high mortality rates. CASE PRESENTATION: A 30-year-old woman, primigravida presented with a negative general and obstetric history. The patient was admitted at 38 weeks and 6 days of gestation due to persistent lower abdominal

Racial and Ethnic Disparities in Low-Risk Unplanned Cesarean Birth: Disaggregating Asian Data.

BACKGROUND: Cesarean outcomes are rarely investigated by Asian ethnicities when examining variation among low-risk, first-time birthing parents. We analyzed a clinical birth dataset of Northwestern U.S. hospitals to evaluate disparities in unplanned cesarean births among disaggregated Asian ethnicities. METHODS: This cross-sectional study used chart-abstracted birth data from 2017 through 2021. Analysis restrictions included hospitals reporting for the full timeframe, and patients who were nulliparous, term, singleton, vertex presentation, allowed to labor without a scheduled cesarean birth,

Implementation of the Robson classification for caesarean sections in Israel: a 10-year cross-sectional study.

BACKGROUND: In recent decades, caesarean section (CS) rates have dramatically increased; the reasons for this trend are multifactorial and not fully understood. This continuing trend has raised public health concerns regarding higher maternal and perinatal risks, high costs, healthcare efficiency, and inequality of services. We aimed to characterize and evaluate Israel's CS rate by applying the Robson classification system. METHODS: This was a national retrospective cross-sectional study. Data from 1,061,786 live births were collected from electronic medical records of women admitted for deli

Maternal and Perinatal Outcomes in Oligohydramnios: A Cross-Sectional Analysis of Pregnancies Between 28 to 42 Weeks of Gestation.

Background Oligohydramnios, defined as an amniotic fluid index (AFI) <5 cm or a single deepest pocket <2 cm, is associated with increased risks of intrauterine growth restriction (IUGR), umbilical cord compression, and perinatal morbidity. Despite extensive research, controversy remains regarding the optimal management strategy for pregnancies complicated by oligohydramnios. This study evaluates maternal and perinatal outcomes in oligohydramnios to guide clinical decision-making. Methods A hospital-based cross-sectional study was conducted over two years (October 2022 to September 2024) at a t

Maternal and neonatal outcomes in full-term pregnancies of very advanced maternal age: a Japanese single-center retrospective cohort study.

BACKGROUND: Advanced maternal age (AMA), defined as pregnancy in people aged ≥ 35 years, is associated with an increased risk of adverse perinatal outcomes. Although the outcomes of AMA pregnancies have been reported, detailed information on the specific outcomes in pregnant individuals aged ≥ 45 years, particularly those reaching full term, remains limited. Therefore, this study aimed to investigate the perinatal outcomes of full-term pregnant individuals aged ≥ 45 years. METHODS: We conducted a retrospective cohort study involving full-term pregnant individuals aged ≥ 35 years who attempted

Extra-uterine placental transfusion and intact-cord stabilisation of infants in caesarean sections: an intervention development and pilot-study (INTACT-1).

BACKGROUND: Keeping the umbilical cord intact the first minutes after delivery is beneficial for both term and preterm infants. However, this may be challenging in caesarean sections (CS) due to lack of mobile resuscitation equipment, maintenance of sterility or concern for excessive maternal blood loss. The objective of this study was to develop and pilot-test extra-uterine placental transfusion and intact-cord stabilisation of infants in CS. METHODS: The intervention development process (phase 1) covered: (A) placenta delivery without cord clamping, (B) intact-cord stabilisation of the infa

Ruptured heterotopic pregnancy with successful term intrauterine pregnancy outcome after natural conception: a case report.

BACKGROUND: Heterotopic pregnancy is the simultaneous existence of intrauterine and extrauterine pregnancies. This is a rare obstetric case, with an incidence of 1 in 30,000 in naturally conceived pregnancies and 1 in 100 pregnancies achieved through in vitro fertilization. Early diagnosis of heterotopic pregnancy is challenging in developing countries with no high-resolution ultrasound. CASE PRESENTATION: A 22-year-old multigravida Somali mother presented to our hospital on her tenth week of gestation with the complaint of progressively worsening abdominal pain of 7 days' duration. Infectious morbidity associated with early amniotomy during labor induction.

OBJECTIVE: This study's purpose was to determine if an early amniotomy increased the risk of maternal and neonatal infectious morbidity when compared with late amniotomy. STUDY DESIGN: This was a single-center retrospective cohort study performed between 1/1/2010 and 3/31/2018. It assessed the relationship between eligible term women undergoing labor induction with early amniotomy, defined as artificial rupture of membranes (AROM) performed less than 12 h from start of cervical ripening, compared with women who do not have an early amniotomy. The primary outcome was a composite of chorioamnio

Post-cesarean delivery pain management.

Cesarean delivery is one of the most common surgical procedures worldwide and is associated with moderate-to-severe postoperative pain. This review summarizes current evidence and guidelines for optimizing postoperative pain management while minimizing opioid-related side effects. Neuraxial long-acting opioids remain the gold standard but are limited by side effects such as pruritus, nausea, and urinary retention. Field blocks, including quadratus lumborum and erector spinae blocks, offer promising alternatives, though no single technique has demonstrated clear superiority over the other. Epid
