Antenatal fetal heart rate monitoring of 20 fetuses with major congenital malformations revealed loss of long-term variability in 11 (55%) and an isolated, abrupt-onset fetal heart rate deceleration in 13 (65%). In ten (50%), loss of variability coexisted with periodic fetal heart rate decelerations. These fetal heart rate changes were significantly more prevalent in the malformed group than in a control population. A comparison of the history of hypertension in the mother during pregnancy was made between a group of 136 mentally retarded children and a randomly selected control group of 122 children. The cases and the controls were 9 or 10 years of age at the time of the survey. A number of potentially confounding factors were controlled for in a multiple logistic model. Maternal hypertension during pregnancy was associated with a relative risk of mental retardation in the offspring of 6.1 with 95% confidence interval of 1.3-28.9.

On the basis of our own earlier studies, the progress of 80 twin pregnancies is followed in a prospective investigation. Early diagnosis makes possible numerous measures for prevention of premature birth--the central problem of twin pregnancies. A cerclage was carried out in 51 cases (63.75%). A 1,340-g male neonate was delivered by emergency cesarean section at 28 weeks' gestation following failed therapy with isoxsuprine hydrochloride and fetal distress. During a two-year period, nine cases of cephalocele were evaluated at the authors' perinatal ultrasound unit. There was one false positive and one false negative diagnosis. In five cases, multiple anomalies were present; three as part of the amnion rupture sequence. The obstetric management consisted of elective termination of pregnancy in five cases and cesarean section in four. The value of sonography in the diagnosis of cephalocele and associated syndromes and malformations is discussed. A plan of management and counseling based upon current understanding of prognosis is presented.

A 23 year old primipara with EPH gestosis after caesarean section suffered massive disseminated intravascular coagulation resulting in anuria, respiratory failure, gastrointestinal bleeding and disturbance of liver function. By treatment with antithrombin-III concentrates, fresh blood and fresh frozen plasma we were able to normalise coagulation disorders and to restore organ functions. Causes and therapeutical management of these diseases are discussed.

A radiological method of assessment of Caesarean-section wound healing is described. A total of fifty-eight cervico-hysterosalpingograms were performed, ten as controls and forty-eight on patients each of whom had primary or multiple Caesarean sections. Reported is the analysis of morbidity, mortality, and mode of delivery in 38 cases of ventral wall defects identified from among 128,500 consecutive live births in Maine (January 1975 to December 1982). We reviewed the records of pregnant diabetic women attended at Mayo Clinic during the years 1950-79 and matched each nonaborted diabetic pregnancy with two control pregnancies. Abortions occurred in 49 of 277 diabetic pregnancies (17.7%). The relative risk of other adverse outcomes of pregnancy (+/- 95% confidence interval) in comparison with controls was as follows: perinatal mortality, 7.0 (3.6-13.8); major congenital birth defects, 6.3 (2.8-14.2); and respiratory distress, 9.0 (4.5-17.9). Two hundred thirty-seven autopsy cases of SUA and 1,242 unselected perinatal autopsies from military hospitals were reviewed and statistically analyzed. Fifteen cases were reviewed over a five-year period at a perinatal centre with intrauterine demise of one member of a multiple gestation.
