Ninety-three vertex-breech and 42 vertex-transverse twin gestations were managed at Yale-New Haven Medical Center during a 5-year period. Antepartum diagnosis of twin gestation occurred in 93% of the cases, and diagnosis was made before delivery of the second twin in 97% of the cases. The efficacy of the non-particulate antacid 0.3 M sodium citrate in single and multiple dose regimes was investigated in 156 obstetric patients, both emergency and elective, who required operative delivery under general anaesthesia. Failure of either of these regimes to elevate the intragastric pH consistently to an acceptable level (greater than 3) led to investigation of a regime combining the histamine H2-receptor antagonist, ranitidine, and a single pre-anaesthetic dose of 0.3 M sodium citrate in a further 170 patients. Eleven hepatic resections were performed by means of a divergent Nd:YAG laser. The beam was transmitted through a fiberoptic delivery system without any handpiece. The shortest resection time and most limited ischemic damage (4 mm) were obtained with 80-W power shoots and a low divergent beam (4 degrees 2 for full angle). Failure of the shoulders to follow readily after delivery of the head is a life-threatening emergency for the mother and baby which is occasionally encountered by most obstetricians, and receives scant attention in current obstetric textbooks. A case of post-partum cardiomyopathy probably due to salbutamol is described. The patient was a young primipara without any cardiovascular disease. She presented a sudden left ventricular failure after Caesarean section for twins: salbutamol had not been given for a week. After 10 days' treatment with diuretics and vasodilators, the patient was cured, no sequellae remaining. It seemed the mechanism was multifactorial: direct myocardial injury, a reduction in the number of receptors, peripheral haemodynamic changes.

A case is reported of acute fatty liver occurring in an Addisonian woman with a twin pregnancy. It is pointed out that Sheehan's syndrome or acute fatty liver of pregnancy is an exceptional cause of jaundice in pregnancy. Its severity is due to the syndrome, associating hepatic failure and renal, pancreatic and haemorrhagic complications. The history of the pregnancy and the past medical history are of less import for the outcome. A survey of triplet pregnancies in Linköping, Sweden, 1973-1981, showed a low perinatal mortality (4%) and low morbidity possibly due to prophylactic bed rest and tocolytic treatment. All children were followed at child health clinics. In one child, a persistent growth retardation was recorded at 4 years of age. No psychomotor deviations were found in the group. An interview concerning social and economic factors after delivery was carried out. The answers revealed considerable strain in daily life for all families.

A 23-year-old primigravida admitted to the labor room one week prior to term because of bleeding was known to have a congenital asymmetry, but a heart defect, which decisively affected the course of delivery, had been undiagnosed up to that time. Secondary caesarean section was necessary for fetal and maternal indications. Many obstetricians feel that obesity is associated with a general increase in risks during pregnancy. However, there is some suggestion that obese patients may actually have a decreased risk of premature delivery and delivering small-for-gestational-age infants.
