Plasma renin activity, plasma renin concentration, plasma renin substrate, plasma angiotensin II and plasma aldosterone were measured in 10 true primiparae, normotensive throughout pregnancy, for 4 days after a spontaneous vaginal delivery. The concentration of all the measured variables, except plasma renin substrate, fell steeply over the first 3 days post partum. Bed rest does not seem to increase the gestational age at the time of delivery but definitely improves the fetal outcome. Among patients with endomyometritis after cesarean section, a double-blind comparison of clindamycin-gentamicin versus cefamandole-placebo therapy was performed. Study criteria were: exclusion--use of prophylactic antibiotics or allergy to the drugs used; entry--uterine tenderness, oral temperature greater than or equal to 101 degrees F, and leukocytosis. Uterine specimens for culture were obtained via a single-lumen transcervical catheter. Most common isolates were Bacteroides bivius, Escherichia coli, and anaerobic cocci. A severe pelvic infection developed in a 17-year-old primigravida after a cesarean section. Multiple antibiotics were administered for presumed mixed aerobic and anaerobic infections, without improvement. Subsequently, total hysterectomy and bilateral salpingo-oophorectomy were performed. Despite negative standard cultures, her condition continued to deteriorate and she required two more exploratory laparotomies for suspected intra-abdominal abscesses. Chlamydia trachomatis and, subsequently, Candida albicans were recovered from cultures of peritoneal fluid obtained after the third operation.

Four hundred and thirty-four twins delivered from 220 women at Notre-Dame Hospital were studied during a period of 11 years (1969-1979). The maternal, fetal, and neonatal outcome was compared before and after 1974, the year ultrasonography and other changes in perinatal care were introduced in our institution. Early diagnosis occurred more frequently after 1974, together with increased antenatal hospitalization. Preeclampsia and hepatic cholestasis occurred in 19.5% and 9.5% of women, respectively. Most of the creatine kinase (CK:EC 2.7.3.2) activity is present in skeletal muscle and myocardium. However, some activity in other organs has been reported. Theoretically, the destruction of organs that contain CK activity should release soluble enzymes into the general circulation. Antepartum dexamethasone administration was associated with a significant lowering of the incidence of respiratory distress syndrome (RDS) and with marked reduction of early neonatal mortality in premature infants. Indications for caesarean section in 213 cases of multiple pregnancy were retrospectively analysed for a period between 1969 and 1980. The average rate of caesarean sections was 6.1 per cent, almost identical with that of monocyesis (5.9 per cent). Perinatal mortality, in particular that of the second twin, may be reduced by expanded indication for caesarean section to cope with malpresentation in cases of multiple pregnancy. Caesarean section is recommended as the primary approach to multiple pregnancy beyond the 34th week with three or more newborns.
