The results of a survey of a rural population requiring obstetric care are presented. It was found that 56,6% of this population had significant risk factors which warranted hospital delivery. A plea is made for the provision of more 'waiting mothers' areas', especially by provincial hospital authorities, to overcome the problems of communication experienced in rural areas. 212 consecutive pregnancies are reviewed in previously infertile women. 96 had conceived following treatment, 21 in a "rest cycle" after treatment, 60 during investigations and 35 before investigations were started. This study based on hospital records is an analysis of 7433 deliveries in Adjamé Maternity Hospital, Abidjan, Ivory Coast. The incidence of low birthweight babies is 14.1%. Mean birthweight and height were shown to increase significantly from parity 1 to 9. Lowest mean birthweight and highest incidence of low birthweight babies were observed in primiparae. Incidence of stillbirths and abortions according to parity are described and an estimate of infant mortality rate has been attempted.

Information collected through the Medical Birth Registry of Norway on a seven-year cohort of 457,465 live births for the years 1967--73 were used to determine the factors associated with the risk of respiratory distress syndrome (RDS). A total of 1235 cases were identified and 510 of these died, resulting in an incidence rate of 2.7 and a mortality rate of 1.1 per 1000 live births. Between July 1, 1969, and October 31, 1977, 619 patients were evaluated for infertility. Homologous insemination (AIH) was employed in 53 patients, many of whom had other contributory causes of infertility. There have been 14 single pregnancies in 11 women, including 4 spontaneous abortions, 1 ectopic pregnancy, 8 term deliveries, and 1 pregnancy in progress. Twenty women are currently under treatment. Prophylactic cefazolin was evaluated in high-risk obstetric patients who had invasive fetal monitoring and subsequent cesarean section. A three-dose regimen of either cefazolin or placebo was administered randomly in a double-blind manner to 93 patients, 48 receiving cefazolin and 45 placebo, with the first dose given when the cord was clamped. A prospective trial was designed to study the effect of extradural analgesia on the management of breech delivery. From a study of 51 patients, it was concluded that the duration of labour was not lengthened, the frequency of breech extraction was decreased and the condition of the infant was improved, as shown by foetal blood sampling and Apgar scores. Therefore we recommend strongly that extradural analgesia should be used in the management of all breech deliveries.

The technique of use of Barton's forceps is described. A series of 248 patients in a 4-year period has been assessed to determine the safety of the instrument. A series of 14 triplet pregnancies has been analyzed. The older triplet mother had a better chance of fetal survival than the younger mother. Parity did not appear to have much effect on fetal survival but the maturity of the infants at birth had more influence than did birth weight, on their survival.
