Overview of Fungi Classified as Risk Group 3 by the Japanese Society for Medical Mycology.

Fungi are categorized into Risk or Hazard Groups, depending on their potential to harm humans, with groupings ranging from 1 (least risky) to 4 (most risky). Similarly, Biosafety or Containment Levels ranging from 1 to 4 denote the level of control over microorganisms in laboratories. This review focuses on fungi categorized as Risk Group 3 by the Biosafety Committee of the Japanese Society for Medical Mycology. One such fungus is Blastomyces dermatitidis, which is primarily found in North America and causes blastomycosis, a condition that can result in a pulmonary or disseminated disease. It

A case of blastomycosis presenting as a non-healing unifocal ulcerative nodule on the jawline.

Blastomyces dermatitidis is a dimorphic fungus that can disseminate in the immunocompetent and immunodeficient. Many infected patients display heterogenous cutaneous findings, making it one of dermatology's great clinical mimics. Cases presenting as single lesions are often mistaken for neoplasms or other infections. We report a patient with diabetes mellitus who presented to the emergency department with a two-month history of an ulcerated jawline nodule. Treatment with incision/drainage and doxycycline for presumed bacterial abscess was unsuccessful. Upon re-presentation 10 days later, biops

Potentially Preventable? A Case of Recurrent Blastomycosis Involving the Central Nervous System.

BACKGROUND: Blastomycosis is a disease caused by infection with thermally dimorphic fungi belonging to the genus Blastomyces. Central nervous system (CNS) involvement occurs in an estimated 1% to 10% of patients with blastomycosis, and treatment requires at least 12 months of antifungal therapy. METHODS: We describe a 32-year-old male who presented with recurrent blastomycosis involving the CNS. RESULTS: The patient was initially diagnosed with pulmonary blastomycosis with possible involvement of the CNS approximately 1 year prior to his current presentation. At that time, he was treated wit

Review of Cutaneous Blastomycosis Seen in Wisconsin.

INTRODUCTION: Blastomycosis is a fungal infection caused by Blastomyces dermatitidis that is hyperendemic in Wisconsin. It commonly presents as a pulmonary infection and frequently disseminates to the skin. Studies evaluating the presentation and diagnosis of blastomycosis with skin as a presenting sign have not been thoroughly evaluated, and understanding the most accurate way to diagnose this infection is important for earlier therapeutic intervention. METHODS: This is a retrospective chart review study of a single institution. Subjects were identified through a search of ICD-9 (Internation

Disseminated Blastomycosis in an Immunocompetent Patient.

Blastomycoses dermatitidis is a dimorphic fungus that can cause disseminated blastomycosis with varying clinical manifestations and multiorgan involvement. While blastomycosis commonly causes pulmonary disease, extrapulmonary spread can result in skin, bone, and central nervous system involvement. Cutaneous blastomycosis can present as pustular lesions that evolve into ulcerative or verrucous plaques. We present a case of disseminated blastomycosis in an immunocompetent patient with both pulmonary and cutaneous features. The patient developed hypoxic respiratory failure and was subsequently di

Pulmonary Blastomycosis in Two Immunocompetent Patients: The Role of Obesity and Vitamin D Deficiency.

This case report depicts two patients with morbid obesity who presented to the ED with signs and symptoms of community-acquired pneumonia and were treated accordingly. Despite empiric antibiotic therapy, their symptoms did not subside, prompting further evaluation, which revealed pulmonary blastomycosis. Both patients were also found to have severe vitamin D deficiency. Treatment with amphotericin B followed by itraconazole, along with aggressive vitamin D supplementation, led to clinical improvement and resolution of lung lesions in both cases. Although blastomycosis is not rare in immunocomp

Blastomycosis Awareness: A Crucial Reminder Amidst the Fifth Pneumonia Episode.

Blastomycosis is a rare fungal infection endemic to North America and parts of Africa. It can be challenging to diagnose until it reaches a critical stage. We present a blastomycosis case in Alabama, emphasizing the importance of early recognition and management. A 67-year-old man had a month of worsening respiratory symptoms, which included malaise, cough, shortness of breath, and fever. Despite multiple urgent care visits and treatments with antibiotics and steroids, his condition continued to deteriorate. He was hospitalized with persistent fever and hypoxia. The imaging revealed patchy lun

Blastomycosis of the Central Nervous System.

The reported incidence of blastomycosis is increasing in certain regions of the United States. The diagnosis is primarily made via urine antigen testing, culture, or cytology smear. The differential diagnosis for blastomycosis includes pneumonia, tuberculosis, and non-infectious pulmonary disease. Clinical context and epidemiologic exposure play a crucial role in diagnosis. However, the differential can expand significantly if there is disseminated central nervous system involvement, especially if pulmonary manifestations are not seen. Imaging begins to play a vital role when differentiating d

Sex differences in blood pressure, free radicals and plasma cholesterol fractions in Ren-2 transgenic rats of various ages.

Sex-related cardiovascular differences were observed in humans as well as in experimental animals. Our previous study demonstrated a marked sexual dimorphism in blood pressure (BP) of 9-month-old heterozygous transgenic Ren 2 rats (TGR), in which mouse Ren-2 renin gene was inserted into the genome of normotensive Hannover Sprague-Dawley rats (HanSD). We found significantly elevated BP only in male TGR, whereas BP of TGR females was similar to that of HanSD females. The aim of our present study was to compare BP of 3- and 6-month-old heterozygous TGR with age- and sex-matched HanSD under the sa

Scrotal abscess as an unusual presentation of blastomycosis.

Blastomyces dermatitidis is an environmental fungus endemic to parts of Eastern North America that notably causes pulmonary infection in humans and other animals with the potential for extrapulmonary spread, particularly in immunocompromised hosts. However, it rarely presents with genitourinary (GU) tract involvement. Herein, we present a unique case of a 37-year-old immunocompetent male with genitourinary blastomycosis with the initial presentation of a scrotal abscess.

DRESS syndrome: quetiapine associated case report and literature review.

Drug rash with eosinophilia and systemic symptoms (DRESS syndrome) is a rare systemic adverse drug reaction with a high mortality rate. Cases of DRESS syndrome have been reported with almost all classes of psychiatric medications, but data remains limited. We describe the case of a 33-year-old woman who presented with acute respiratory distress syndrome secondary to severe pulmonary blastomycosis. Her hospital course was complicated by severe agitation for which the psychiatry consult team was involved and several medications were trialed including quetiapine. She developed a diffuse erythemat

Traveling Does More Than Widen One's Horizon: Travel History as Key to Diagnosing Blastomycosis.

This case report highlights the importance of a detailed travel history and the need to revisit the differential diagnosis when there is an unexpected clinical course. A previously healthy 15-year-old male presented to a hospital in Florida with a fever, cough, and shortness of breath. He was seen multiple times at urgent care centers and treated with steroids and antibiotics for community-acquired pneumonia (CAP). The patient's chest X-rays and CT showed necrotizing pneumonia with pleural effusion, which required a chest tube. Despite broadening coverage for possible resistant organisms, his

A case series and literature review of blastomycosis during pregnancy amidst the COVID-19 crisis.

Blastomycosis dermatitidis is a rare fungus known for is classic mimicry of pneumonia, lung cancer, and mycobacterial infections. Whilst it is known best for affecting those in the Ohio and Mississippi River basins, several cases have erupted in the Midwest region. Few case reports have focused on blastomycosis and its sequalae in pregnancy. We present a case series of blastomycosis diagnosed during the second and third trimesters in two women amidst the COVID-19 pandemic. Given immunosuppression, complications and treatment can be challenging for clinicians. This case series and discussion ho

Disseminated histoplasmosis in an immunosuppressed patient successfully treated with isavuconazole.

Histoplasmosis is an endemic fungal infection caused by the dimorphic fungus, Histoplasma capsulatum, which is treated with intravenous amphotericin B and oral itraconazole as first-line and second-line therapy. We report a case of a man in his early 70s treated with methotrexate and infliximab for rheumatoid arthritis who developed disseminated histoplasmosis. The patient was unable to absorb itraconazole due to intractable diarrhoea and developed a severe, anaphylactoid reaction or an immune reconstitution inflammatory syndrome when treated with liposomal amphotericin B. He was subsequently

A Case of Blastomyces dermatitidis Diagnosed Following Travel to Colorado: A Case Report and Review of Literature.

The dimorphic fungus Blastomyces dermatitidis, is one of the most frequent causes of endemic fungal infections in the United States as well as various other parts of the world. Clinical presentations vary widely, ranging from asymptomatic to disseminated systemic infections. Blastomycosis usually has a predilection for the lungs, but extra pulmonary manifestations are present in 25-40% of cases, involving the skin, bone, genitourinary tract, and CNS. A fungal culture of tissue specimens and fluids is confirmatory. The mainstay of treatment are the azole antifungals, i.e., itraconazole, and for

Blastomycosis-Associated Hospitalizations, United States, 2010-2020.

BACKGROUND: Blastomycosis is an environmentally acquired fungal disease that can cause severe illness, with approximately 65% of reported cases requiring hospitalization. Recent trends in blastomycosis-associated hospitalizations in the United States have not been described. METHODS: We analyzed hospital discharge data from the Healthcare Cost and Utilization Project (HCUP) National (Nationwide) Inpatient Sample. We calculated hospitalization rates per 100,000 population using U.S. census data and examined factors associated with in-hospital mortality. RESULTS: An estimated 11,776 blastomyco

An Unusual Presentation of Blastomycosis.

INTRODUCTION: Blastomycosis is a rare pyogranulomatous infection that most commonly involves the lungs and sometimes the skin. Other manifestations are much less common. Diagnosis relies on biopsy, histopathology, and culture of suspicious lesions. CASE PRESENTATION: In this case, a healthy 42-year-old male from Wisconsin presented to the emergency department with a chief complaint of 2 weeks of knee pain without a clear mechanism of injury. Upon further examination, he was found to have lesions on his abdomen, which he had first noticed over 3 years prior and had been treated with antibiotic
