Prenatal Diagnosis of an Umbilical Cord Cyst Associated With Obstructed Umbilical Artery Flow: Case Report.

BACKGROUND: Umbilical cord cysts are rare and the reported associations with abnormal foetal outcomes present challenges in antenatal diagnosis and management. KEY FINDINGS: We report a case of a large umbilical cord cyst detected in the second trimester, with no communication to the foetal bladder. Third trimester review demonstrated oedematous change in the umbilical cord and significantly reduced umbilical artery flow necessitating delivery by emergency caesarean section. Postnatally, the infant was found to have a patent urachus. DISCUSSION: This case underlines the importance of antenat

Adnexal Torsion in the Third Trimester.

A 41-year-old woman presented with acute, severe left-sided abdominal pain and vomiting at 37 weeks' gestation. Her symptoms were attributed to renal colic, and she was admitted for supportive treatment. During her admission, she went into spontaneous labour. Due to other obstetric indications, the team proceeded with delivery by emergency caesarean section. Intra-operatively, a necrotic left fallopian tube and ovary were identified, and a diagnosis of adnexal torsion (AT) was recognised. There was no return of tissue perfusion on de-torsion, and a left salpingo-oopherectomy was performed. AT

Independent effect of gestational weight gain and prepregnancy obesity on pregnancy outcomes among Saudi women: A sub-cohort analysis from Riyadh mother and baby cohort study (RAHMA).

BACKGROUND: Gestational weight gain (GWG) and prepregnancy obesity are garnering more attention as determining factors of pregnancy outcomes when it comes to the wellbeing of both the mother and her baby. This study was conducted to describe the pattern of GWG among participants of Riyadh Mother and Baby Multicenter Cohort Study (RAHMA) and to investigate the detrimental effects of excessive GWG and prepregnancy obesity on pregnancy outcomes. METHODS: RAHMA is a multicentre cohort study conducted in three hospitals in Riyadh, Saudi Arabia. Participants were categorized according to the Instit

Bruck Syndrome: Beyond the Obvious.

INTRODUCTION: Bruck syndrome is a rare autosomal recessive disease characterized by multiple joint contractures, bone fragility, and fractures. Two genes have been associated with Bruck syndrome, FKBP10 and PLOD2, though they are phenotypically indistinguishable. CASE PRESENTATION: We present a prenatally diagnosed case of Bruck syndrome in a young multiparous woman, with no notable personal, family or obstetric history. A 12-week ultrasound raised the suspicion of short long bones, subsequently confirmed at 16 weeks. In addition, bilateral fixed flexion of the elbow, wrist, and knee joints a

Maternal Prepregnancy Weight and Pregnancy Outcomes in Saudi Women: Subgroup Analysis from Riyadh Mother and Baby Cohort Study (RAHMA).

The objectives of this study were to estimate the prevalence of prepregnancy overweight/obesity and underweight among Saudi mothers and to determine the adverse pregnancy outcomes associated with them. Methods. This is a subgroup analysis from a Riyadh mother and baby cohort study. Participants were divided into four groups according to prepregnancy BMI. Participants with normal BMI were the reference group. Groups were compared in relation to pregnancy-related obstetric, as well as fetal and neonatal complications. A regression model was used to control for covariates, and adjusted odds ratio

Factors associated with acute kidney injury during delivery or the postpartum period: A case control study.

INTRODUCTION: To investigate the incidence of risk factors associated with acute kidney injury(AKI) occurring during the intrapartum or the immediate postpartum period. MATERIAL AND METHODS: A retrospective case control study in a tertiary UK hospital of pregnant women identified with AKI in labour or in the postpartum period between July 2019 and June 2020 was carried out. Women identified with pregnancy related AKI (pr-AKI) in labour or postpartum were compared with a matched cohort of women with no evidence of pr-AKI, using multiple regression analysis. RESULTS: In 4600 births, 71 women w

Group B streptococcal infection in the first 90 days of life in North Queensland.

BACKGROUND: Group B Streptococcus (GBS) infection is recognised as an important cause for neonatal sepsis. AIMS: To describe the incidence and risk factors for invasive GBS under 90 days of age in North Queensland from January 2002 to December 2011. MATERIAL AND METHODS: Patients were identified with positive blood and cerebrospinal fluid cultures to obtain incidence figures. The Townsville district cohort was further investigated for the presence of maternal and fetal risk factors in a retrospective case-controlled study. RESULTS: Early onset GBS continues to occur at 0.43/1000 live births

Delayed postpartum haemorrhage secondary to a ruptured uterine artery pseudo-aneurysm, successfully treated by transarterial embolisation.

A 29-year-old woman (gravida 1, para 1) had an uneventful first pregnancy and a delivery by emergency caesarean section at term. The caesarean section was complicated by a massive obstetric haemorrhage of 5000 ml. After closure, an immediate re-laparotomy was indicated due to heavy vaginal bleeding. The site of bleeding was identified as an extension of the uterine incision, and was sutured. She was stabilised by transfusion of blood and blood products in the intensive therapy unit, and discharged 5 days later. The patient was re-admitted 6 weeks later with brisk, painless vaginal bleeding, pa

Obesity and mode of delivery in primigravid and multigravid women.

Our objective was to study the effects of maternal body mass index (BMI) on the mode of delivery for primigravid and multigravid women. A retrospective cohort study was conducted at the University College Hospital Galway, Ireland, of 5162 women delivered from 2001 to 2003. BMI at the first antenatal appointment was calculated. Comparisons were made between each of the five BMI categories separately for primigravid and multigravid women in relation to gestation at delivery, age, mode of delivery, and birthweight. There were 5162 deliveries during the time period of the study; 2006 were primigra

Antepartum rupture of the intertwin-dividing membrane in monochorionic diamniotic twins: a case report and review of the literature.

We report a case of monochorionic diamniotic twin gestation confirmed by ultrasound visualization of the thin intertwin-dividing membrane at 32 weeks' gestation. Ultrasound at 36 weeks failed to demonstrate the thin dividing membrane. The pregnancy ended a few days later with spontaneous vaginal delivery of the first twin. The second twin was in transverse lie with no membranes that could be felt around. Severe fetal heart rate deceleration developed, prompting delivery by emergency caesarean section. Cord entanglement was noted at the time of delivery, which resulted in severe perinatal morbi

A population study of birthweight and the risk of caesarean section: Scotland 1980-1996.

OBJECTIVES: 1. To describe the relation between birthweight and risk of emergency caesarean section at term; 2. to determine whether the relation between birthweight and caesarean section differed between male and female babies; and 3. to determine what proportion of the increased rates of caesarean section could be related to greater birthweights. DESIGN: Retrospective population study. SETTING: Data collected from Scottish Morbidity Record 1980 to 1996. POPULATION: All first singleton deliveries by emergency caesarean section and non-elective vaginal birth of live babies at 40 weeks of ge

Pregnancies complicated by placenta praevia: what is appropriate management?

OBJECTIVE: To review the outcome of pregnancies complicated by placenta praevia over a three-year period (1991-1993) and to describe in detail the antenatal course and the events leading to delivery, assessing retrospectively whether there are clinical features predictive of outcome and whether outpatient management would be reasonable. DESIGN: A retrospective review of the case records of women with a pregnancy complicated by placenta praevia. SETTING: A tertiary referral teaching hospital in Edinburgh. RESULTS: There were 15,930 deliveries in the study period. Fifty-eight women (0.4%) had

Delivery and development: to what extent can one associate cause and effect?

Data on 570 mothers and their children whose development was personally assessed from birth to four years were analysed. Strong associations were found between the incidence of fetal distress during labour, birth asphyxia, and delivery by emergency caesarean section. At the age of four years girls and children in the non-manual social classes were more advanced, developmentally, than boys and children in the manual classes. No associations were found between perinatal factors and total developmental scores; but highly significant differences were found between `at risk' pregnancy groups accord
