Cesarean Myomectomy: A Retrospective Observational Study at a Tertiary Care Center in Bihar, India.

Introduction Fibroids during pregnancy are frequently encountered in our routine obstetric practice. Whether cesarean myomectomy is a safe and practical treatment for women with uterine myomas or if it is completely contraindicated is a topic of much discussion. Due to the apprehension of increased blood loss, there remains a dilemma in obstetricians' decisions on whether to do a cesarean myomectomy or not. Our article may help obstetricians to make an informed decision. Method We did a single-center, uncontrolled, retrospective observational study on 25 cases who were enrolled over two years.

Potential Value of Novel Lipid-Derived Indicator as a Biomarker for Nonarteritic Anterior Ischemic Optic Neuropathy.

IMPORTANCE: Overuse of surgical procedures is increasing around the world and harms both individuals and health care systems by using resources that could otherwise be allocated to addressing the underuse of effective health care interventions. In low- and middle-income countries (LMICs), there is some limited country-specific evidence showing that overuse of surgical procedures is increasing, at least for certain procedures. OBJECTIVES: To assess factors associated with, extent and consequences of, and potential solutions for low-value surgical procedures in LMICs. EVIDENCE REVIEW: We searc

Long-term neurological health of the term offspring born via cesarean section for non-reassuring fetal monitoring.

PURPOSE: While some evidence suggests that intrapartum fetal heart rate (FHR) monitoring is associated with a reduction in intrapartum death, a reduction in long-term neurological impairment has not been proven. In this study, we sought to evaluate the offspring long-term neurological morbidity of children born via cesarean delivery (CD) for non-reassuring FHR (NRFHR) indication. METHODS: A population-based cohort analysis was performed comparing long-term neurological morbidity of term children born via CD for NRFHR as compared with children born via CD for non-progressive labor (NPL), at a

Assessing Postpartum Readmission Rates and Associated Risk Factors Using the Robson Classification: A Single-Center Experience.

Objective: Postpartum maternal readmission is a significant burden for patients as well as the health system. Postpartum readmission rate is a known factor in evaluating quality of care and in guiding potential beneficial interventions. Use of the Robson Group (RG) classification, initially used for analysis of cesarean section (CS) rates, has been recently expanded to evaluate other obstetrical outcomes. We aimed to describe the rates of postpartum maternal readmission across RG classification and to identify risk factors among the different maternity groups. Study Design: We carried out a re

An interventional pilot study protocol on the effect of extra virgin olive oil on women with preeclampsia risk.

BACKGROUND: Preeclampsia (PE), a multisystem and complex disorder diagnosed when maternal hypertension manifests after 20 weeks of gestation with proteinuria, is one of the direct causes of maternal morbidity and mortality, in addition to bleeding and infection. Despite its critical impact, effective preventive dietary interventions are scarce. Oxidative stress and microvascular damage are central to PE pathophysiology. Extra virgin olive oil (EVOO), particularly early harvested EVOO (EVOOEH), is rich in antioxidant compounds and may mitigate these issues. METHODS: This randomized, single-mas

Risk factors for combined vaginal-cesarean delivery in the vertex-vertex twins: a case-control study.

PURPOSE: Twin pregnancies pose unique challenges during labor, particularly the risk of cesarean section for the second twin after vaginal delivery of the first-termed a combined vaginal-cesarean delivery. Although nonvertex presentation of the second twin is a recognized risk factor, predictors in vertex-vertex twin pregnancies remain unclear. The objective of this study was to identify maternal, pregnancy-related, intrapartum, and fetal risk factors associated with combined vaginal-cesarean delivery in twin pregnancies with vertex-vertex presentation. METHODS: A retrospective single-center

Impact of Gestational Weight Gain on Delivery Outcomes in Women Undergoing Trial of Labour After Caesarean Section: A Cross-Sectional Study .

Repeat cesarean deliveries are the primary driver of the global increase in caesarean section (CS) rates. Promoting trial of labour after caesarean (TOLAC) offers an opportunity to reduce unnecessary CS rates. While gestational weight gain (GWG) is a modifiable risk factor influencing maternal outcomes, its impact on TOLAC remains unclear.  Objective: To assess the association between abnormal GWG and the risk of failed trial of labour after CS.  Methods: A prospective case-control study was conducted at a tertiary care centre in India from January 2021 to December 2022. Women with a prior sin

Evaluating Closed Incision Negative Pressure Therapy Use Following High-Risk Caesarean Section in a Middle East Population.

Surgical site complications (SSCs) following caesarean section impact maternal morbidity and mortality. To mitigate SSC development, incision management strategies such as closed incision negative pressure therapy (ciNPT) can be utilised. The effect of ciNPT on the management of closed incisions following caesarean section was examined in the present study. Patients underwent caesarean section between 2022 and 2024 at a single acute care hospital in the United Arab Emirates (UAE). All 82 patients underwent vaginal cleansing and received antibiotics prior to surgery. A Pfannenstiel incision was

Prevalence of High-Risk Pregnancies in an Urban Field Practice Area of Mumbai, India: A Community-Based Analytical Cross-Sectional Study.

Introduction A high-risk pregnancy (HRP) is defined as a condition in which the mother, foetus, or both are at increased risk of adverse outcomes compared to a normal pregnancy or the general population. However, there is limited data available from urban slum settings for effective health planning and resource allocation for the management of HRPs. The current study aims to estimate the prevalence of HRPs in an urban slum of Mumbai and to identify the sociodemographic and obstetric factors and delivery characteristics associated with HRPs. Methodology An analytical cross-sectional study was c

Isolating the effect of prior cesarean delivery on operative vaginal delivery in subsequent labor among women with prior vaginal birth: A matched cohort study.

OBJECTIVE: TOLAC (trial of labor after cesarean) is associated with higher rates of operative vaginal delivery (OVD) compared with deliveries without a prior cesarean. However, it remains unclear whether this reflects the independent effect of prior cesarean delivery or differences in pelvic characteristics and labor dynamics. We aimed to isolate the association between prior cesarean delivery and OVD risk while minimizing confounding related to parity and previous vaginal births. DESIGN: Retrospective matched cohort study. SETTING: Single university-affiliated tertiary center. POPULATION:

Identifying the risk of spontaneous preterm birth after term singleton cephalic birth, by mode of birth, cervical dilatation, and incision extensions at cesarean section: a cohort study.

BACKGROUND: Increasing evidence links cesarean section, especially at full dilatation, to subsequent spontaneous preterm birth. However, many studies to date lack sufficient detail to determine the impact of advancing cervical dilatation in labor, the effect on spontaneous preterm birth at gestations under 32 weeks, and data to support a mechanism of effect. OBJECTIVE: To confirm an independent association between mode of birth and subsequent spontaneous preterm birth at gestations <37, <32, and <28 weeks, including by cervical dilatation, and to explore any impact of unintended uterine incis

Evaluating pregnancy and neonatal outcomes in mothers with genetic disease using electronic health care records.

PURPOSE: The effect of Mendelian disorders on pregnancy and neonatal outcomes is poorly understood because of their rarity and the challenge of compiling complete prenatal and postnatal records. METHODS: Using electronic health records from a single academic center, we developed a retrospective cohort of maternal-infant dyads. Cases were mothers with molecularly confirmed Mendelian disorders paired with live-born infants; controls had no documented genetic disease. Outcomes were evaluated overall, by organ system, and by individual disorder. RESULTS: The cohort included 58,912 dyads, 241 wit

Development and clinical application of an intrapartum ultrasound-based predictive model for cesarean section conversion in nulliparous women following unsuccessful vaginal delivery attempts: A single-center retrospective study.

Ultrasound is potentially more accurate for identifying women who are likely to have a spontaneous vaginal birth than conventional labor progression assessments. However, research on the application of intrapartum ultrasound measurements in predicting cesarean section (CS) during vaginal trials of labor in full-term low-risk nulliparous women is lacking. We aimed to develop a predictive model using intrapartum ultrasound data for CS conversion in nulliparous women who experienced a failed vaginal delivery trial and integrate this model into clinical practice. Low-risk nulliparous women carryin
