Ultrasound-controlled therapeutics for bladder cancer: Mechanisms, recent advances, and future perspectives.

Bladder cancer remains a significant clinical challenge, characterised by a high recurrence rate and substantial healthcare costs. Although advances in surgery, systemic therapy, and intravesical treatment have improved clinical outcomes, treatment-related toxicity, therapeutic resistance, and the persistently poor prognosis of advanced disease highlight the need for more effective therapeutic strategies. Ultrasound-based micro/nanotherapeutics offer a promising approach that combines deep tissue penetration with high focal precision in a non-invasive, externally controllable manner. This revi

An Injectable Hydrogel Sustains Low-Dose Intra-Articular Glucocorticoid Exposure to Protect Cartilage in Osteoarthritis.

Intra-articular glucocorticoid (GC) injections provide rapid symptomatic relief in osteoarthritis (OA). However, repeated administration of conventional triamcinolone acetonide (TA) suspensions can generate recurrent high local drug exposure, which may contribute to cartilage degeneration. Although extended-release TA formulations have been developed to reduce burst exposure, achieving prolonged and stable intra-articular drug retention while preserving cartilage structure remains challenging. Here we report an injectable hyaluronic acid (HA) hydrogel that forms a stable intra-articular depot

Association of ESRα-Pvull and ESRα-Xbal Polymorphisms with Breast Cancer Susceptibility in an Iranian Population: A Hospital-Based Case-Control Study.

The viral vector-transgene soaked gelatin-sponge method has been shown to be successful in mediating transgene expression across an intact round window membrane (RWM) in mouse in vivo. However, there are many confounding factors which make it difficult to evaluate the role of the RWM in gene transfer. We have created an in vitro model to test the feasibility of gene delivery through an intact RWM. The round window including the bony niche of a CD1 mouse was removed under an operating microscope and fixed with adhesive on the base of a petri dish through which a hole had been drilled. A Call for Comprehensive Reform of Military Medical Planning of NATO and Its Allies Based on Lessons From the Ukraine War-Cultural Context and the Human Factor.

The systematic targeting of medical infrastructure, personnel, and casualty evacuation routes by Russia in Ukraine challenges whether the North Atlantic Treaty Organization (NATO) can still rely on the Geneva Conventions for the protection of medical assets and personnel. This necessitates a thorough review of NATO Standardization Agreements (STANAGs) and doctrine used for Medical Planning for future Large Scale Combat Operations (LSCO). However, drawing lessons from the Ukrainian experience requires consideration of cultural context, medical evidence, and human factors. Ukraine's military med

Stereotactic lesioning of cholinergic cells by injection of ME20.4 Saporin in the nucleus basalis of Meynert in a rhesus monkey (Macaca mulatta).

Cholinergic degeneration in the nucleus basalis of Meynert (NBM) is clinically linked to cognitive impairment and gait dysfunction in Alzheimer's disease and Parkinson's disease. Modeling cholinergic degeneration in an animal model may provide powerful opportunities to study the clinical-physiological role of the NBM and lead to new therapies. We describe a procedure to inject ME20.4 Saporin, an immunotoxin that specifically binds to and depletes cholinergic neurons stereotactically into the NBM of a rhesus monkey (Macaca mulatta). A digital non-human primate brain atlas was co-registered to t

Stem Cell-Based Delivery of Immunomodulators for Overcoming Glioblastoma Immune Evasion.

Glioblastoma (GBM) is the most aggressive primary brain tumour in adults, characterised by rapid progression, extensive heterogeneity, and poor outcomes despite surgery, radiotherapy, and temozolomide (TMZ). A subpopulation of glioblastoma stem cells (GSCs) with self-renewal and multi-lineage differentiation capabilities drives tumour initiation, progression, recurrence, and therapeutic resistance. GSCs evade conventional treatments via enhanced DNA repair, multidrug efflux, activation of survival pathways, epigenetic reprogramming, and entry into quiescent states. Moreover, these cells utilis

Adjunctive Therapies in Periodontitis: Current Concepts and the Future.

Periodontitis is a chronic, host-mediated inflammatory disease in which microbial dysbiosis and dysregulated immune responses drive the destruction of tooth-supporting tissues. Although conventional therapy remains centered on mechanical biofilm control, persistent inflammation and alveolar bone loss in susceptible individuals underscore the need for adjunctive strategies. Translating preclinical discoveries into predictable clinical outcomes, however, remains a major challenge in periodontal research. This narrative review integrates two interrelated themes, translational research methodology

Ultrasound-Responsive Macrophage Biomimetic Phase-Change Nanoparticles for Delivering miR-126 Ameliorate Murine Heart Transplant Rejection.

Rejection following heart transplantation remains a significant challenge in the medical science. In addition to the immune response, the inflammatory response and microvascular endothelial damage can exacerbate rejection; however, current therapies focus primarily on the immune response. Numerous studies have confirmed that miR-126 regulates vascular inflammation and promotes angiogenesis. To achieve the precise targeted delivery of miR-126, macrophage biomimetic ultrasound phase-change cationic nanoparticles carrying miR-126 (miR-126-MCNPs) are synthesized. Functional roles of microRNAs in pancreatic islet autoimmunity: what do we know and where do we target?

INTRODUCTION: Type 1 diabetes (T1D) results from a destructive dialog between stressed pancreatic beta-cells and immune system. While current disease-modifying approaches targeting these processes are being developed and tested, microRNAs have emerged as a molecular interface connecting both sides of islet autoimmunity. AREAS COVERED: Specific miRNAs orchestrate beta-cell stress adaptation, immune activation, and intercellular communication, thus shaping disease trajectory and progression across stages. Recent discoveries identified distinct miRNA networks as ER-stress modulators and/or immun

Gut Microbiome Strategies for Enhancing ICI Delivery Across the BBB in Glioblastoma.

Glioblastoma (GB) is highly malignant with a median survival of 14 months despite conventional treatments like surgery, radiotherapy, and temozolomide. Resistance to these therapies necessitates innovative approaches, such as immune checkpoint inhibitors (ICIs) targeting cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), programmed cell death protein 1 (PD-1), and programmed death ligand 1 (PD-L1) to enhance T-cell-mediated tumor destruction. However, clinical trials have shown limited ICI efficacy in GB due to its immunosuppressive microenvironment and the blood-brain tumor barrier (BBTB),

Advances in stem cell-derived beta-cell therapy: A new frontier in type 1 diabetes treatment.

Type 1 diabetes (T1D) is a chronic autoimmune disease characterized by the destruction of pancreatic beta cells (β-cell), leading to lifelong dependence on exogenous insulin. Despite advances in insulin delivery systems and glucose monitoring, achieving tight glycemic control remains a challenge for many patients. In recent years, stem cell-derived β-cell therapy has emerged as a promising strategy for restoring endogenous insulin production. This review explores the progress in differentiating human pluripotent stem cells, such as embryonic stem cells and induced pluripotent stem cells (iPSCs

Type 1 Diabetes: A Review.

IMPORTANCE: Type 1 diabetes is defined by hyperglycemia due to autoimmune destruction of the insulin-producing beta cells in the pancreas, leading to insulin deficiency, and accounts for 5% to 10% of all cases of diabetes. Type 1 diabetes affects approximately 2 million people in the US and 8.4 million people worldwide and is associated with microvascular and macrovascular complications such as retinopathy, nephropathy, neuropathy, and cardiovascular disease. OBSERVATIONS: Ninety percent to 95% of people with type 1 diabetes have at least 1 autoantibody when they are diagnosed with diabetes.

Messenger RNA delivery to islet β cells using conjugated lipid nanoparticles.

Effective therapies for type 1 diabetes (T1D) must both restrain immune hyperactivity and reduce β cell susceptibility to destruction. We describe a lipid nanoparticle (LNP) platform for β cell-enriched mRNA delivery that can be further augmented by conjugation to enhanced glucagon-like peptide-1 (eGLP-1). Both unconjugated and eGLP-conjugated LNPs deliver mRNA efficiently to mouse and human β cells in vitro. Biodistribution studies in C57BL/6J mice in vivo demonstrate pancreatic enrichment of LNPs, with greater β cell enrichment achieved by eGLP-LNPs compared with unconjugated LNPs specifical

Icaritin eliminates tumor-associated macrophages via STX16-dependent extracellular vesicle delivery of autophagosomes from hepatocellular carcinoma cells.

BACKGROUND: To elucidate the mechanism by which icaritin—a novel agent for hepatocellular carcinoma (HCC)—remodels the tumor microenvironment (TME) by inhibiting HCC cell metabolism-mediated M2 polarization of tumor-associated macrophages (TAMs). METHODS: Integrative approaches spanning in vitro Transwell cocultures, RNA-seq, LC3-based autophagy tracing, STX16 gene edition, and orthotopic xenografts mechanistically dissected the affective and mechanism of icaritin in remodeling TME. RESULTS: Our results indicate that icaritin transcriptionally suppresses ALDOB in HCC cells to reduce lactate pr

DPSC-EVs Drive Functional Recovery in Sjögren's Disease by Systemic Immunomodulation.

Sjögren's disease (SjD) is a debilitating autoimmune disorder characterized by progressive salivary gland destruction, for which current therapies are merely palliative. Novel therapeutic approaches focusing on immunomodulation are therefore urgently needed. Extracellular vesicles from dental pulp stem cells (DPSC-EVs) have emerged as a promising cell-free platform for systemic immunomodulation. Here, we investigated the therapeutic efficacy of DPSC-EVs in a nonobese diabetic mouse model of SjD to elucidate the mechanistic basis for their superior immunomodulatory potential compared with bone-

Ultrasound-targeted delivery of miR-513a-5p inhibits ferroptosis and restores vascular function in preeclampsia.

Preeclampsia (PE) is a hypertensive pregnancy disorder with limited therapeutic options. This study explores the role of miR-513a-5p in modulating ferroptosis via TXNIP and evaluates its therapeutic delivery using ultrasound-targeted microbubble destruction (UTMD). miR-513a-5p was identified as significantly downregulated in PE through GEO analysis (GSE84260). To assess its function, miR-513a-5p was overexpressed in HTR-8/SVneo and TEV-1 trophoblast cells, and its effects on proliferation, apoptosis, migration, and ferroptosis were evaluated. TXNIP was investigated as a mechanistic target usin

Host modulation therapy in periodontitis: from established therapies to emerging technologies.

Periodontitis is a biofilm-induced chronic inflammatory disease, characterized by gingival inflammation and alveolar bone loss. According to a national survey, approximately half of the U.S. adults are affected by periodontal disease. To effectively prevent and treat periodontitis, it is essential to address its underlying causes. The primary etiological factors include polymicrobial synergy and dysbiosis of the oral microbiota, and a dysregulated immune response. The standard therapeutic approach, mechanical removal of biofilm through debridement, sometimes demonstrates limited efficacy, part

Therapeutic effects of hypoxia-preconditioned cartilage progenitor cell-exosomes on osteoarthritis through autophagy activation and macrophage polarization modulation.

Osteoarthritis (OA) is a prevalent degenerative joint disease characterized by progressive cartilage loss, pain, and functional disability. While tissue engineering holds promise for cartilage repair, obtaining high-quality seed cells with optimized functional activity remains a bottleneck. Exosomes derived from cartilage progenitor cells (CPCs) have emerged as potent cell-free therapeutic candidates. Here, we demonstrate that exosomes from hypoxia-preconditioned CPCs (H-Exos) possess superior regenerative capabilities compared to their normoxic counterparts. specifically by promoting prolifer
