Ex vivo and computational investigation of corneal iontophoresis to enhance penetration of high-molecular-weight compounds: a study using albumin as a model molecule.

Efficient ocular drug delivery remains a major challenge due to the strong barrier properties of the corneal epithelium and the rapid clearance of topically applied formulations. This limitation is particularly critical for hydrophilic and high-molecular-weight (HMW) therapeutics such as proteins and biologics. In this study, we investigated transcorneal iontophoresis as a strategy to enhance the delivery of HMW compounds using albumin (66 kDa) as a model macromolecule. Ex vivo rabbit corneas were exposed to iontophoretic currents ranging from 0.5 to 7 mA for clinically relevant conditions, wh

Pancreatic tissue engineering: towards a vascularized bioengineered cure for diabetes.

UNLABELLED: Type 1 diabetes results from autoimmune destruction of pancreatic β-cells, and while whole-organ pancreas and islet transplantation can restore endogenous insulin secretion, both are limited by donor scarcity, early inflammatory injury, hypoxia, and the need for lifelong immunosuppression. Pancreatic tissue engineering has therefore emerged as a strategy to generate scalable, transplantable constructs capable of reestablishing physiological glucose regulation. However, as native islets depend on a robust microvascular network for oxygen delivery, nutrients and coordinated hormone s

Successful neonatal outcome after postmortem caesarean section at Sumbawanga Regional Referral Hospital, Rukwa, Tanzania: case report.

BACKGROUND: Cardiac arrest in pregnancy can occur for a variety of reasons. During maternal cardiac arrest or shortly thereafter, the fetus may remain viable. Postmortem caesarean section can potentially save the fetus with or without long-term neurological sequelae; in the absence of timely intervention, however, fetal death is inevitable. Given the limited documentation in developing countries and the possibility of unfavourable outcomes, this report presents one of the few documented cases from resource-limited Sub-Saharan Africa describing a live birth with a sustained favourable neurologi

Local periodontal injection, systemic heart repair: A dual-functional hydrogel for non-invasive concurrent treatment of periodontitis and myocardial infarction.

Periodontitis and myocardial infarction (MI), the leading cause of mortality worldwide, represent globally prevalent inflammatory diseases with bidirectional pathophysiological links. Despite the urgent demand for non-invasive strategies capable of alleviating local periodontal destruction while mitigating associated systemic cardiovascular complications, no integrated treatment modality currently exists. To address this challenge, a protein-loaded antibacterial hydrogel, thiolated chitosan/AMP-PEG-maleimide (C1.5P4/BMP-2), with novel sustained protein release properties was developed. This hy

Application and Potential of Local Drug Delivery Systems for Antibacterial Treatment of Periodontitis.

Periodontitis (PD) is a chronic inflammatory disease characterized by the progressive destruction of periodontal supporting tissues. As one of the most prevalent chronic diseases, PD affects more than 743 million people globally, some with serious systemic health implications. Plaque accumulation constitutes the key driver of periodontitis, initiating host inflammatory cascades and compromising periodontal microbiome equilibrium. Conventional treatment methods, such as scaling and root planing, are limited by a constrained operative field, resulting in blind spots that impede the complete erad

Delayed residual intra-abdominal abscess with threatened preterm labor after laparoscopic appendectomy in the second trimester.

Acute appendicitis is the most common non-obstetric surgical emergency in pregnancy. We report a 30-year-old primigravida at 25 weeks' gestation who underwent laparoscopic appendectomy for appendicitis with localized peritonitis. Thirteen days later, she represented with worsening lower abdominal pain, vomiting, leukocytosis, vaginal bleeding, and uterine contractions. Ultrasound showed a right lower abdominal collection and a viable 27-week fetus. Magnetic resonance imaging demonstrated a peri-cecal abscess adherent to the uterus with associated intraperitoneal fluid suspicious for purulent p

Compartment-specific Zinc Misallocation in Diabetic Foot Ulcers: Mechanistic Coupling Between Macrophage M1 Locking and MMP-9 Hyperactivation.

Diabetic foot ulcer (DFU) remains a formidable clinical challenge characterized by a persistent failure of the regenerative process. Affecting approximately 15-25% of diabetic individuals over their lifetime, DFU is associated with a disproportionate risk of lower-limb amputation and mortality. Although zinc is a fundamental requirement for cutaneous repair, conventional supplementation strategies often yield inconsistent therapeutic outcomes. This review addresses this clinical gap by introducing the Zinc Misallocation Hypothesis, which posits that compartment-specific collapse of ionic parti

Advanced primary abdominal pregnancy with a live birth: a case report.

OBJECTIVES: Advanced primary abdominal pregnancy, an extremely rare condition, poses diagnostic challenges and carries a risk of serious maternal and perinatal complications. In the absence of evidence-based protocols, current management approaches are predominantly guided by anecdotal reports and individual clinical judgment. We report a case of primary advanced abdominal pregnancy in a primigravida. CASE PRESENTATION: A 26-year-old asymptomatic woman at 31 + 6/7 weeks' gestation was referred for routine third-trimester ultrasonography, which revealed a non-gravid uterus, a viable extrauteri

Oxidative stress and antioxidant pathways in the pathogenesis of periodontitis and peri-implantitis: mechanistic insights and therapeutic potentials.

BACKGROUND: Periodontitis and peri-implantitis are chronic, immune-mediated inflammatory diseases characterized by progressive destruction of tooth- and implant-supporting tissues. Although microbial dysbiosis initiates these conditions, accumulating evidence indicates that host-derived oxidative stress plays a central role in amplifying inflammation, impairing tissue repair, and driving irreversible bone loss. Excessive production of reactive oxygen species (ROS) disrupts redox homeostasis, induces molecular damage, and activates redox-sensitive signaling pathways that perpetuate tissue destr

Hydrogel-Mediated delivery of Annexin A1 peptide modulates inflammation and tissue remodelling in ligature-induced murine periodontitis.

Periodontitis is characterized by chronic inflammation and progressive alveolar bone loss. This study evaluated whether local delivery of an Annexin A1 N-terminal mimetic peptide (Ac2-26) incorporated into an anionic carboxymethylcellulose hydrogel (Hydrogel/AnxA12-26) modulates inflammation and tissue remodelling in ligature-induced periodontitis in wild-type (WT) and Annexin A1 knockout (AnxA1-/-) mice. Ligature-induced periodontitis was established around the maxillary first molars of C57BL/6 WT and AnxA1-/- mice. Animals received local applications of Hydrogel/AnxA12-26 or vehicle three ti

NMNAT2 Functional Landscape and Localization in Programmed Axon Degeneration.

Why is NMNAT2 (and the NAD-NMNAT2-SARM1 axis) important? Axon loss is a common and early driver of disability across neurodegenerative diseases, peripheral neuropathies, traumatic injury, and neurotoxic chemotherapy. A major conceptual advance from the Wallerian degeneration research field is that axons can execute an intrinsic self-destruction program (programmed axon degeneration) that is actively suppressed in healthy axons. NMNAT2 acts as the central endogenous axon maintenance factor in this system: it is highly labile; it must be continuously delivered from the soma into axons; and when

Macrophage membrane-biomimetic DNA nanocages deliver taurine to reprogram trained immunity for periodontitis therapy.

As one of the most burdensome oral diseases, periodontitis frequently leads to tooth loss and heightens the risk of systemic diseases. A major clinical challenge is that current strategies often fail to resolve chronic inflammation and bone resorption characteristic of its refractory state. Recent research implicates maladaptive trained immunity (TRIM) as a core mechanism, which promotes myeloid cell skewing and amplifies inflammatory responses and bone loss. Single-cell sequencing of peripheral blood mononuclear cells from periodontitis patients identified an elevated inflammatory stress stat

Benign, persistent, and invasive: mechanistic and translational approaches to middle‑ear cholesteatoma.

Acquired middle-ear cholesteatoma is a histologically benign keratinizing squamous epithelial lesion that paradoxically exhibits locally destructive, recurrent, and invasive behavior, often resulting in ossicular erosion, hearing loss, labyrinthine fistula, and, rarely, intracranial complications. Surgical excision remains the primary management strategy; however, recurrence is common due to persistent microenvironmental drivers. Recent mechanistic studies-including single-cell transcriptomics, spatial proteomics, and epigenetic profiling-reveal a multifactorial pathogenesis orchestrated by ch

The inflammatory microenvironment repurposes BMP-2 signaling to drive pathological osteophyte formation in osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is increasingly recognized as a low-grade inflammatory disease, yet how the inflammatory milieu interacts with regenerative signals remains poorly understood. Bone morphogenetic protein 2 (BMP-2) typically functions as an anabolic factor for cartilage; however, its levels are paradoxically elevated in degenerating joints. This study investigates whether the inflammatory microenvironment acts as a molecular switch that alters the functional outcome of BMP-2 signaling from repair to pathology. METHODS: BMP-2 levels were quantified in human OA cartilage and synovi

Oncobiotics in urinary bladder cancer. A narrative review of living cancer therapeutics.

Urinary bladder cancer (UBC) remains a major global health burden, with high recurrence rates and limited therapeutic options for patients who fail standard intravesical and systemic treatments. In recent years, Living Cancer Therapeutics (LCTs)-including bacteria-, virus-, and microbiome-based oncobiotics-emerged as innovative biological strategies capable of overcoming key limitations of conventional cancer therapies. This article is a narrative review aimed at mapping the mechanistic landscape, historical development, and translational progress of LCTs in UBC. Five interrelated mechanisms w

Controversy over caesarean delivery in late 18th- and early 19th-century France: Jean-François Sacombe and the Anticaesarean School.

BACKGROUND: . In late 18th-century France, caesarean delivery on living women became the focus of an unusually sharp medical and public controversy. This article examines that debate through Jean-François Sacombe (1750-1822), founder of the Anticaesarean School, and his principal opponent, Jean-Louis Baudelocque (1745-1810). METHODS: . We conducted a narrative historical review of primary printed sources by Sacombe and Baudelocque, contemporary institutional publications, and secondary historical literature. Sources were identified in medical-library holdings, the French National Library Gall

Elution kinetics and antimicrobial efficacy of high-dose gram-negative antibiotic-loaded acrylic cement: an in vitro and in vivo study.

BACKGROUND: High-dose antibiotic-loaded acrylic cement is routinely employed for the management of skeletal infections when implant fixation or local structural support is required. However, no established consensus or clinical guidelines currently exist regarding the selection of antibiotics in acrylic cement, particularly those targeting Gram-negative bacteria. This study aimed to systematically evaluate, through both in vitro and animal experiments, the efficacy of high-dose antibiotic-loaded acrylic cement in controlling periprosthetic skeletal infections caused by Gram-negative bacteria f

Dihydroartemisinin-Loaded ZIF-8 Nanoparticles for Antibacterial and Osteogenic Dual Therapy in Brucella-Infected Bone Defects.

Brucella infection frequently causes multijoint cartilage loss and bone destruction, posing significant challenges for clinical treatment. Conventional two-stage therapy is time-consuming and has a high relapse rate. Dihydroartemisinin (DHA) exhibits significant antibacterial activity against Brucella melitensis 16M in vitro. However, its cytotoxicity to mammalian cells and poor water solubility limit its application. In this study, DHA@ZIF-8/HA nanoparticles were synthesized for targeted delivery to infected macrophages. The nanoparticles consisted of a pH-responsive zeolitic imidazolate fram
