[Delivery of twin pregnancies: Guidelines from the College of French Gynecologists and Obstetricians].

In France, the rate of twin deliveries is estimated at 1.6% of all births. These recommendations address the gestational age at delivery and the mode of delivery for twin pregnancies. They were developed using the GRADE methodology, with questions formulated according to the PICO framework. Regarding gestational age at delivery, in cases of uncomplicated dichorionic diamniotic pregnancies, it is recommended to plan delivery between 37 weeks and 0 days and 38 weeks and 6 days of gestation (weak recommendation, very low-quality evidence). In cases of uncomplicated monochorionic diamniotic pregna

Forceps, vacuum, and breech extraction for difficult delivery of an unengaged fetal head during cesarean birth: a retrospective cohort study.

OBJECTIVES: To compare perinatal outcomes, primarily neonatal transcutaneous bilirubin levels and the need for phototherapy, associated with three fetal extraction techniques used during cesarean birth after difficult or failed manual delivery of an unengaged fetal head: short forceps, vacuum, and breech extraction. METHODS: A retrospective, single-center analytical study was conducted including all cesarean births performed between July 2022 and July 2025 in which fetal extraction was considered difficult and required the use of short forceps, vacuum, or breech extraction. Only singleton, te

Vaginal delivery of the second twin: simulation to improve trainee knowledge and comfort.

BACKGROUND: Vaginal delivery in twins is feasible but challenging. Successful vaginal delivery of a non-vertex second twin depends on knowledge of specific obstetrical maneuvers. Skill acquisition at the patient's bedside is difficult, making simulation training an integral part of obstetrics and gynecology residency programs. METHODS: This prospective, randomized, controlled, single-center study involved obstetrics and gynecology residents. Group A attended a frontal lecture followed by practical simulation; Group B received digital home learning. One month later, both groups underwent a sim

Development of evaluation grids for obstetrical techniques in simulation training using the Delphi method.

BACKGROUND: Simulation-based training has become a cornerstone in obstetrics education, offering valuable opportunities to practice rare and complex procedures in a controlled environment. The importance of simulation in obstetrics and gynecology is well documented internationally, particularly in improving clinical skills and patient safety. In some countries, certain obstetric procedures, such as vaginal breech delivery or shoulder dystocia management, are infrequently performed, which highlights the challenge of ensuring that practitioners are adequately prepared for these rare events. Obstetric Rectal Buttonhole Tear: Case Series, Literature Review, and Management Recommendations.

PURPOSE: Obstetric rectal buttonhole tear (ORBT) is a rare but serious complication of vaginal delivery, associated with rectovaginal fistula and fecal incontinence. Standardized management protocols are lacking. PATIENTS AND METHODS: We conducted a retrospective study of 3 institutional ORBT cases (2018-2024) managed with primary transvaginal layered repair and synthesized data from 40 published cases (1993-2024). RESULTS: Among 43 patients, 77.8% were primiparous; the median neonatal weight was 3570 g. ORBT occurred after spontaneous vaginal delivery (58.1%), operative vaginal delivery (34

Pre-operative ultrasound before cesarean delivery: reviewing findings during the first surgical time-out to improve maternal and infant safety.

Twenty-nine million cesarean deliveries performed every year. Although often regarded as routine, even elective procedures can lead to serious complications for mothers and infants. Modern patient safety emphasizes that no incision should be undertaken without knowing what lies beneath, a principle codified by the World Health Organization in its Surgical Safety Checklist. A preoperative ultrasound represents an extension of this principle to obstetrics. Several complications exemplify face-valid safety signals: incision through an anterior placenta with maternal hemorrhage and neonatal anemia

Impacted fetal head extraction methods at second stage cesarean and subsequent preterm delivery: A multicenter study.

OBJECTIVE: Second-stage cesarean delivery (CD) is associated with subsequent preterm birth (PTB). It has been suggested that an increased risk of PTB after second-stage cesarean delivery could be linked to a higher chance of cervical injury due to the extension of the uterine incision. Previous studies have shown that reverse breech extraction is associated with lower rates of uterine incision extensions compared to the "push" method. We aimed to investigate the association between the method of fetal extraction during second-stage CD and the rate of spontaneous PTB (sPTB), as well as other ma

Management of impacted fetal head at cesarean birth: A systematic review and meta-analysis.

INTRODUCTION: Despite increasing incidence of impacted fetal head at cesarean birth and associated injury, it is unclear which techniques are most effective for prevention and management. A high quality evidence review in accordance with international reporting standards is currently lacking. To address this gap, we aimed to identify, assess, and synthesize studies comparing techniques to prevent or manage impacted fetal head at cesarean birth prior to or at full cervical dilatation. MATERIAL AND METHODS: We searched MEDLINE, Emcare, Embase and Cochrane databases up to 1 January 2023 (PROSPER

Difficult fetal extraction in cesarean section: Number 8 - 2024.

The main causes of difficult fetal extraction during cesarean section are deeply impacted fetal head and floating presentation of the fetus. Studies of management techniques for difficult fetal extraction during cesarean section and the maternal and neonatal results lack scientific evidence, as these predominantly come from case reports, small case series and expert opinions. The deeply impacted fetal head is usually associated with prolongation of the expulsion period and/or unsuccessful attempts at operative vaginal delivery. The main maternal complications associated with the management of

Evidence-based cesarean delivery: intraoperative management from skin incision until placental delivery (Part 8).

The goal of standardizing the technique of the routine, uncomplicated cesarean delivery (CD) is to decrease maternal morbidity while optimizing neonatal outcomes. During the procedure, a family-oriented CD is recommended. The low transverse cesarean skin incision (created with either scalpel or diathermy) is preferred with either the Joel-Cohen or Pfannenstiel methods being acceptable. For patients with obesity (BMI > 35kg/m2), surgeons may also elect either the Cohen (including supraumbilical) or Pfannenstiel (infraumbilical or infrapannus) technique as there are similar outcomes, however the

Management of impacted fetal head at cesarean delivery.

Globally, more than 1 in 5 women give birth by cesarean delivery, and at least 5% of these births are at full cervical dilatation. In these circumstances, and when labor has been prolonged in the first stage of labor, the fetal head can become low and wedged deep in the woman's pelvis, making it difficult to deliver the baby. This emergency is known as impacted fetal head. These are technically challenging births associated with serious risks to both the woman and the baby. The difficulty in disimpacting the fetal head increases maternal risks of hemorrhage and injury to adjacent organs and ma

Management of a rare case of placenta trophotropism.

There are several variations of placental shape or implantation. Multilobed placentas are thought to arise due to implantation in areas of decreased uterine perfusion. An example is represented by lateral implantation in between the anterior and posterior walls of the uterus. Other local factors leading to multilobation are implantation over leiomyomas, in areas of previous surgery, in the cornu, or over the cervical os. After implantation, there is preferential growth in areas of superior perfusion and atrophy in areas of poor perfusion. This is called trophotropism. We described a singular c

Trial of labor of vertex-nonvertex twins following a previous cesarean delivery.

BACKGROUND: Maternal and neonatal outcomes of trial of labor after cesarean delivery of twins are similar to those of singleton trials of labor after cesarean delivery. However, previous studies did not stratify outcomes by second-twin presentation on admission to labor. OBJECTIVE: To examine maternal and neonatal outcomes following trial of labor after cesarean delivery in twins with vertex-nonvertex presentation. Internal podalic version of second twin: Improving feet identification using a simulation model.

BACKGROUND: Podalic version and breech extraction require high obstetrical expertise. Identifying fetal extremities is the first crucial step for trainees. When this skill is not polished enough, it increases the inter-twin delivery interval and can even jeopardize the whole manoeuver. MATERIAL AND METHODS: We present a model for simulating and training this specific skill, with obstetrical mannequin, and 3D printed hands and feet. Five feet and five hands (five rights and five lefts of each one) were printed in 3D after initial ultrasound acquisition of a near term fetus. Each foot and hand,
