Comparative effectiveness of intravenous remifentanil, epidural anesthesia, and a 2-step analgesic approach for external cephalic version: a large prospective single-center cohort study.

BACKGROUND: Among the analgesic strategies used during external cephalic version, neuraxial anesthesia is the only approach that has consistently demonstrated higher vaginal delivery rates. However, neuraxial analgesia encompasses a heterogeneous range of techniques, and higher complication rates have been reported compared with other approaches. OBJECTIVE: This study aimed to compare the success of external cephalic version, modes of delivery, maternal pain, and complications using 3 strategies: intravenous analgesia with remifentanil, epidural anesthesia, and a stepwise approach in which ep

The role of fetal viability in perineal and anal sphincter injury: Insights from a large cohort study.

BACKGROUND: Perineal injury is a common complication of vaginal delivery with implications for maternal morbidity. Injuries range from minor lacerations to severe obstetric anal sphincter injuries (OASI). The role of fetal viability in perineal outcomes remains unclear. METHODS: A retrospective cohort study was conducted at a tertiary center (with approximately 13 000 annual deliveries). Eligible cases were singleton vaginal births ≥24 weeks and ≥ 500 g. Breech, vacuum-assisted deliveries, and episiotomies were excluded. Matching was performed by year of birth, parity, gestational age, birthw

Impact of an e-learning training for interpreting intrapartum fetal heart rate monitoring to avoid perinatal asphyxia: A before-after multicenter observational study.

INTRODUCTION: Perinatal asphyxia, a condition that results from compromised placental or pulmonary gas exchange during the birth process, is rare but can lead to serious neonatal and long-term consequences. The visual analysis of cardiotocography (CTG) is designed to avoid perinatal asphyxia, but its interpretation can be difficult. Our aim was to test the impact of an e-learning training program for interpreting CTG on the rate of avoidable perinatal asphyxia at term. METHOD: We conducted a retrospective multicenter before-after study comparing two periods, before and after the implementatio

Does overweight and obesity have an impact on delivery mode and peripartum outcome in breech presentation? A FRABAT cohort study.

PURPOSE: Obesity is a worldwide and growing issue affecting women in childbearing age, complicating surgical procedures as well as pregnancy. Through a reduction of not necessarily required cesarean deliveries-for instance in pregnancies with breech presentation-obesity mediated and surgery-associated morbidity might be contained. Date on the impact of maternal BMI in vaginally attempted breech delivery is not existing. To give insight into whether an elevated BMI leads to an increased perinatal morbidity in vaginally intended deliveries out of breech presentation, we analyzed delivery outcome

Circumferential Vaginal Tear During the Cesarean Section: A Complication of Vaginal Hand Assistance in a Deeply Impacted Fetal Head.

We present the case of a 29-year-old, G2P1+0 pregnant woman who was unbooked and presented to the emergency room at 36+5 weeks gestation with complaints of leaking liquor, labour pains, vaginal bleeding and raised blood pressure. Her history revealed previous vaginal delivery and index pregnancy complicated with obstetric cholestasis, pre-eclampsia, and fetal growth restriction. During her hospital course, the patient underwent an emergency cesarean section due to uncontrolled blood pressure and pathological cardiotocograph (CTG) revealing a deeply impacted fetal head intraoperatively and nece

Intrapartum Ultrasound Guidance to Make Safer Any Obstetric Intervention: Fetal Head Rotation, Assisted Vaginal Birth, Breech Delivery of the Second Twin.

Intrapartum ultrasound (US) is more reliable than clinical assessment in determining parameters of crucial importance to optimize the management of labor including the position and station of the presenting part. Evidence from the literature supports the role of intrapartum US in predicting the outcome of labor in women diagnosed with slow progress during the first and second stage of labor, and randomized data have demonstrated that transabdominal US is far more accurate than digital examination in assessing fetal position before performing an instrumental delivery. Intrapartum US has also be

Neglected congenital bilateral knee dislocation treated by quadricepsplasty with semitendinosus and sartorius transfer: A case report.

BACKGROUND: Congenital knee dislocation (CKD) is a rare condition, which accounts for 1% of congenital hip dislocations. It can present as an isolated condition or coexist with other genetic disorders. Treatment options include serial casting, percutaneous quadriceps recession, and V-Y quadricepsplasty (VYQ). The pathogenesis and hereditary patterns of CKD are not fully understood, with most cases being familial. CKD is usually managed immediately after birth. However, in this report, the patient was neglected for 2 years. CASE SUMMARY: A 2-year-old girl with bilateral CKD after birth present

Sonographic examination at the beginning of the second stage of labor predicts birth outcome in vaginally intended breech deliveries: a blinded prospective study.

PURPOSE: In order to spread competence in vaginal breech deliveries, it is necessary to develop new and easily applicable tools for birth progression and safety evaluation. Ultrasound is a useful and ubiquitously available tool with already documented value for birth progression observation. In deliveries out of breech presentation, an established ultrasound examination is missing. We determined the descent of the fetal buttocks in relation to the maternal pelvic inlet using intrapartum ultrasound. We evaluated these results in comparison to the clinical vaginal examination with the aim to est

Assisted Reproductive Technology and Breech Delivery: A Nationwide Cohort Study in Singleton Pregnancies.

Since essential factors have changed in recent years in assisted reproduction technologies (ARTs), this study reassessed the association between ART and breech presentation. We primarily aimed to estimate the correlation between ART and breech at delivery. Secondary purposes were to evaluate the correlation between other subfertility treatments (OSTs) and breech and to assess possible confounding factors and temporal trends. This study investigated the 31,692,729 live birth certificates from US states and territories in the 2009-2020 period. The inclusion criteria were singleton births reporti

Remifentanil for External Cephalic Version: A Systematic Review and Meta-Analysis.

The purpose of this study was to examine the efficacy of remifentanil on external cephalic version (ECV) in breech presentation. An extensive search was conducted using PubMed, Cochrane Library, and other grey literature. Only randomized controlled trials using remifentanil for ECV were included. Risk ratio (RR) and mean difference (MD) were used to estimate outcomes and quality of evidence was assessed using the Risk of Bias and GRADE system. Five studies consisting of 602 patients were analyzed. Remifentanil resulted in a moderate increase in ECV success rate (RR, 1.19; 95% CI, 1.00 to 1.43;

Role of Topical Anaesthesia in Pain Management of Farm Animals, a Changing Paradigm.

Field evidence indicates that livestock producers are motivated by access to products that readily deliver pain management during husbandry interventions and, more recently, viral epidermal infectious diseases, including FMD. There has been impressive adoption in Australia of a farmer-applied spray-on topical anaesthetic wound formulation (TAF; Tri-Solfen®, Medical Ethics, Australia), initially for managing pain of the breech modification 'mulesing' procedure that reduces susceptibility of sheep to flystrike. Over 120 million lambs have now received pain relief and cattle producers have commen

Brachial Plexus Birth Injuries.

Brachial plexus birth injuries (BPBIs) are typically traction type injuries to the newborn that occur during the delivery process. Although the incidence of these injuries has overall decreased from 1.5 to around 0.9 per 1000 live births in the United States over the past 2 decades, these injuries remain common, with incidence holding fairly steady from 2008 to 2014. Shoulder dystocia is the strongest identified risk factor, imparting a 100-fold greater risk. The newborn's shoulder is caught behind the mother's pubic bone, and traction performed on the child during delivery results in injury t

Influence of conception and delivery mode on stress response marker Oct4B1 and imprinted gene expression related to embryo development: A cohort study.

BACKGROUND: Recent scientific data support that the mode of conception and delivery may influence epigenetic regulation and therefore embryo development. Octamer-binding transcription factor 4-B1 (OCT4B1), a novel variant of OCT4 with yet unknown biological function, is suggested to have a potential role in mediating cellular stress response. Furthermore, Insulinlike Growth Factor 2 (IGF2), Mesoderm-specific Transcript (MEST) and paternally expressed gene 10 (PEG10) are genes known as imprinted and are regulated via means of epigenetic regulation. The influence of delivery mode and conception
