Blastomycosis is a fungal infection caused by Blastomyces dermatitidis, which is hyperendemic to areas of Northwestern Ontario, Canada. Delayed diagnosis is common due to the condition's nonspecific clinical presentation, often resembling malignancy or other dermatological and infectious diseases. A patient presented with persistent pustular, violaceous nodules and a tense bulla that was initially diagnosed as bacterial folliculitis. Despite four courses of oral antibiotics, there was no clinical improvement. The patient's cutaneous symptoms progressed, and he developed systemic symptoms. A punch biopsy, blood studies and a chest CT confirmed a diagnosis of disseminated blastomycosis. The patient improved with itraconazole. Blastomycosis is a systemic endemic fungal infection caused by Blastomyces dermatitidis, a dimorphic fungus, that predominantly affects the lungs but can disseminate to other organs, mimicking many diseases, making diagnosis challenging. We present a case of disseminated blastomycosis in a male, initially misdiagnosed as syphilis due to a false positive biopsy result for Treponema pallidum. This case highlights the importance of considering atypical presentations of fungal infections, even in the presence of laboratory findings and outside of the normal geographic regions they may be found.

Blastomycosis is endemic in central and southern North America but rare in China. It can mimic community-acquired pneumonia, tuberculosis, or cancer. We describe a patient who initially had tuberculosis diagnosed and later had blastomycosis diagnosed through metagenomic detection, which aided diagnosis and treatment. Clinicians should consider blastomycosis in differential diagnoses for respiratory diseases.

Blastomyces infections are classified as dimorphic fungal infections commonly seen geographically throughout the Mississippi and Ohio River Valleys, St. Laurence Seaways, as well as the Canadian providences of Ontario Manitoba, and Saskatchewan. Blastomycosis is a fungal infection known to mimic many disease processes, including malignancy. A 25 year old male presented with several weeks of fevers and testicular pain. Workup demonstrated scrotal and prostatic abscesses. Fluid from these following surgical drainage revealed Blastomyces dermatitidis. He was treated with 12 months of oral anti-fungal therapy and repeat Blastomyces urine antigen was negative at follow up. While disseminated blastomycosis most commonly presents with pulmonary and cutaneous manifestations, genitourinary symptoms are rarely seen, but important to consider.
