OBJECTIVE: To describe an elusive case of blastomycosis involving the thyroid gland, which was ultimately diagnosed by use of ultrasound-guided fine-needle aspiration (FNA).
METHODS: We present a case report, including clinical features, results of laboratory studies, and radiographic, computed tomographic, and ultrasonographic findings. In addition, the treatment and the utility of FNA of the thyroid relative to the diagnosis of blastomycosis are discussed.

Although disseminated blastomycosis is a rare complication in pregnancy, delay in diagnosis and treatment can be fatal. Blastomycosis is an uncommon life-threatening disease caused by the dimorphic fungus Blastomyces dermatitidis. Estimates of the percentage of cases occurring in children range from 2% to 11%, and it is exceedingly rare in individuals less than 1 year of age. We present the case of an otherwise healthy infant with disseminated blastomycosis acquired in Northern Ontario and a brief review of the pediatric literature.

Blastomycosis is an uncommonly recognized disease in pediatric patients. A 26-year-old veiled Saudi-Arabian woman presented with hemoptysis, and multiple nodules and abscesses. A skin biopsy specimen revealed yeast forms consistent with Blastomyces dermatitidis. Fungal cultures from bronchoscopy and skin specimens also grew B dermatitidis. She was treated with oral itraconazole (200 mg twice a day). A 5-year-old German shepherd was evaluated after collapsing at home following a week of lethargy and anorexia. Systemic blastomycosis was diagnosed histologically at necropsy. Blastomycosis is an unusual fungal infection in children. It is often a chronic infection characterized by granulomatous and suppurative lesions. Clinical manifestations include either pulmonary findings or disseminated disease. PURPOSE: To review the presentation and course of choroidal blastomycosis, a rare chorioretinal mycotic infection, which results from disseminated blastomycosis.

Blastomycosis is a fungal disease endemic to the midwestern and southeastern United States. This is a case report of a 29-year-old woman who presented with weight loss, fever, fatigue, and pneumonia. She developed disseminated blastomycosis, adult respiratory distress syndrome (ARDS), and ulcerative skin lesions, requiring mechanical ventilation, amphotericin B, and multiple surgeries. Blastomycosis is endemic to a large portion of the United States. Family physicians should consider fungal infection in the differential diagnosis of an unresolving pneumonia.

Intraocular infection due to Blastomyces dermatitidis is rare, and only 10 cases have previously been reported. Manifestations of ocular blastomycosis can range from keratitis to panophthalmitis, and it is often difficult to diagnose ocular blastomycosis early. We report the case of a 45-year-old man who had disseminated blastomycosis that involved the lungs, skin, and ocular uvea and who was successfully treated with systemic and local antifungal therapy. We also review the literature describing the spectrum of clinical findings due to intraocular blastomycosis.
