Reports of blastomycosis in individuals infected with the human immunodeficiency virus (HIV) are increasing. We report on 3 patients co-infected with blastomycosis and HIV (to add to the previously reported 21), and review important clinical aspects and outcomes in all cases. The percentage of patients co-infected with blastomycosis and HIV who had disseminated blastomycosis (63%) was similar to the blastomycosis patients in the general population (67%); however, as a group the patients with HIV were severely immunosuppressed and fared poorly. Severe immunodeficiency was indicated by CD4 count

Fungi cause serious, often fatal infections in immunocompromised hosts. Recipients of solid organ and bone marrow transplants are predisposed to invasive fungal infections with Candida species, Aspergillus species, and Cryptococcus neoformans. In contrast, infections with Blastomyces dermatitidis have rarely been diagnosed in transplant recipients. We describe a patient who received an orthotopic heart transplant and developed recurrent disseminated blastomycosis. Other reported cases of blastomycosis in transplant recipients are summarized. Clinical presentation, treatment options, and morbid

Two patients were mistakenly treated for several months for tuberculosis. Both had lung and vertebral involvement and one patient had superficial skin lesions as well. Blastomycosis was eventually confirmed by fungal culture, but only after both patients had deteriorated markedly; one died soon after treatment with amphotericin B was started. In Africa, blastomycosis is a relatively rare condition which can mimic tuberculosis; it is one of several infections which should be borne in mind when dealing with apparently non-responsive tuberculosis.

Disseminated blastomycosis, usually a chronic slowly progressive illness, primarily occurs in individuals with a history of exposure to a rural environmental source. We report a case of disseminated blastomycosis leading to acute respiratory failure in a young man confined to prison in an urban area.

Ketoconazole (KTZ) has largely replaced amphotericin B as first-line therapy for blastomycosis. However, KTZ penetrates poorly into the central nervous system (CNS), and therapeutic failure may be caused by initially unrecognized CNS infection. Two patients (22% [2/9] of all culture-proven cases of blastomycosis at Grady Memorial Hospital, Atlanta, over 15 years) developed CNS blastomycosis while receiving KTZ. Neither initially had CNS symptoms; both had cutaneous and pulmonary disease that responded to KTZ. If KTZ or other fungistatic imidazoles are to continue as primary therapy for blastom

Patients with acquired immunodeficiency syndrome (AIDS) are subject to a host of opportunistic infections, but to our knowledge a predisposition to blastomycosis has not previously been established. Autopsies of two patients with AIDS revealed disseminated blastomycosis with massive pulmonary involvement, Blastomyces meningoencephalitis, and widespread dissemination. The massive systemic involvement and rapid terminal course in both cases may reflect the state of acquired immunodeficiency. An analysis of an autopsy series showed that the incidence of blastomycosis was increased in patients wit

A case of disseminated blastomycosis in a male renal transplant recipient is presented. Discontinuation of immunosuppressive therapy and treatment with high-dose ketoconazole was successful in treating the patient's cutaneous and pulmonary disease initially. Ketoconazole was discontinued after 12 months of chronic therapy, but 2 weeks after discontinuation, blastomycosis recurred. High-dose ketoconazole was again effective; the patient remains asymptomatic presently on chronic suppressive therapy with ketoconazole.

The production of interleukin 1 (IL 1) by adherent peripheral blood mononuclear cells (PBMC) was quantitated in 16 individuals infected with Histoplasma capsulatum or Blastomyces dermatitidis and 16 age-matched controls. In parallel, we measured blastogenic responses to phytohemagglutinin (PHA) by PBMC from patients and controls. Of the 16 patients, six had pulmonary histoplasmosis, six had disseminated histoplasmosis, two had pulmonary blastomycosis, and two had disseminated blastomycosis. At the time of study, none of the patients were receiving immunosuppressive agents or had an underlying

A 36-year-old man developed blurred vision and a cough five months after traveling to an area from where a large outbreak of acute blastomycosis had been reported. Examination revealed a left choroidal lesion in the macular region, skin lesions on the right calf, and a left lung apical infiltrate. Histopathologic examination of the skin lesion demonstrated a granulomatous inflammation and broad based, budding yeast characteristic of Blastomyces dermatitidis. Amphotericin B therapy produced a rapid resolution of both choroidal and pulmonary lesions. During a six-month follow-up, there was no ev

An 18-year-old woman developed disseminated blastomycosis in the 30th week of pregnancy. She was treated with amphotericin-B. Simultaneous maternal-infant blood levels at birth were 1.9 and 1.3 micrograms/mL, respectively. The amphotericin-B level in the amniotic fluid was 0.3 microgram/mL at delivery. The infant was normal and without ill effects from either the blastomycosis or the amphotericin-B. The skin lesions and pulmonary infiltrates in the mother improved rapidly, without unexpected side effects from the therapy. The limited experience with amphotericin-B indicates that it is both wel

This report describes the first well-documented reported case of the recovery of Blastomyces dermatitidis from the blood of a patient with disseminated blastomycosis. The isolate was recovered by using a lysis-centrifugation method and inhibitory mold agar after 6 days of incubation at 30 degrees C. One of six blood samples collected yielded a positive culture.

A patient with disseminated blastomycosis gave birth to an infant who died three weeks later of diffuse pulmonary blastomycosis. The infant most likely acquired the disease through intrauterine transmission. The patient's extensive skin lesions responded dramatically to ketoconazole.

A case of disseminated blastomycosis is reported. The patient presented with lung involvement, lupus vulgaris-like lesions, mainly of the face, and subcutaneous abscesses overlying areas of bone destruction. The diagnosis was confirmed by mycological microscopy and culture of skin material from involved areas. The importance of early diagnosis and the favourable response to treatment are emphasized.

A domestic shorthair cat examined because of dyspnea was found to have ophthalmoscopic and radiographic changes suggestive of systemic mycosis. The cat died despite antifungal therapy. Histologic examination revealed Blastomyces dermatitidis in the eyes, brain, lungs, stomach, liver, kidneys, spleen, pancreas, and adrenal glands. The pathologic changes were similar, but more widespread than those typically seen with canine blastomycosis.

A reproducible model of disseminated blastomycosis was established in C57BL/6 mice by intravenous injection of 10(6) yeast-phase Blastomyces dermatiditis organisms. The infection progressed over 5 weeks to involve lungs, brains, superficial fascia, livers, and spleens of mice. By week 5, there was a greater number of organisms in lungs and brains than in livers and spleens. The tissue response in lungs, brains, and livers progressed from acute neutrophilic invasion before week 1 to pyogranuloma formation by week 5. Lymph nodes and spleens were remarkably spared. By week 5, infected mice became

Disseminated blastomycosis with extensive involvement of the lungs, skin and bones was diagnosed in a 31-year-old woman who was 36 weeks pregnant. No antifungal treatment had been given while she was pregnant, and she gave birth shortly after admission to hospital. The child was healthy and uninfected, and there were no signs of inflammation or infection in the placenta. Post partum the mother was treated with 2 g of amphotericin B, with resolution of her symptoms. A literature review suggested that blastomycosis and other systemic fungal infections are more likely to occur during pregnancy be

An incidental case of cervical blastomycosis discovered by examination of a Papanicolaou smear is reported. Examinations revealed no other focus of blastomycosis in the patient. Since her husband was undergoing treatment for disseminated blastomycosis involving the lungs and prostate at the time the fungus was found, this case may represent venereally transmitted blastomycosis.

Disseminated blastomycosis was diagnosed in a 27-yr-old woman during her twenty-eighth week of pregnancy. Her skin lesions resolved promptly on treatment with amphotericin B. At full-term delivery when the mother had already received 1,536 mg of amphotericin B, the infant was normal and the placenta showed no evidence of blastomycotic infection. After administration of 16 mg of amphotericin B over 140 min, at delivery the concentrations of amphotericin B in serum of blood samples from the mother and the cord were 0.32 and 0.12 micrograms/ml, respectively. The published experience with systemic
