Evaluating maternal mortality, risk factors, and emergency c-section timing in polytrauma pregnant patients.

BACKGROUND: Trauma is a leading non-obstetric cause of maternal morbidity and mortality, yet standardized guidelines for the management of pregnant polytrauma patients remain limited. Physiologic changes of pregnancy may alter trauma response and complication risk, but data guiding risk stratification and time-sensitive interventions are sparse. OBJECTIVE: To evaluate maternal mortality and complications among pregnant polytrauma patients, with particular focus on maternal age, early blood transfusion, and timing of emergency cesarean section. METHODS: This retrospective cohort study analyze

Examining Racial and Ethnic Disparities in Low-Risk Cesarean Section Deliveries in the Colorado Healthy Start Study.

BACKGROUND: Cesarean sections (CS) create unnecessary health risks for mothers and newborns in low-risk contexts. In the United States, low-risk CS prevalence is higher among people of color, though this may vary across regions. We investigated low-risk CS prevalence by race and ethnicity in a Colorado pregnancy cohort comprising primarily Hispanic and non-Hispanic White mother-offspring pairs. METHODS: Pregnant women in Denver, Colorado were enrolled (2009-2014, n = 1,410). 904 mother-offspring pairs met the criteria for low-risk birth and were included in this analysis. Low-risk CS was base

Differential expression of vaginal microenvironment cytokines in relation to delivery modes and their impact on postpartum pelvic floor recovery.

Postpartum pelvic floor dysfunction (PPFD) is a common complication that substantially impairs women's quality of life, and its pathogenesis is closely associated with the physiological and pathological changes induced by different modes of delivery. As a key regulator of local immunity and tissue repair, the vaginal microenvironment undergoes marked alterations during and after childbirth, particularly in cytokine expression patterns. This review synthesizes current evidence on the differential expression of cytokines-including pro-inflammatory, tissue-repair, and anti-fibrotic mediators-acro

Strategies for preventing nerve injuries in obstetric anesthesia: a review.

PURPOSE OF REVIEW: Neuraxial anesthesia is widely used for labor analgesia and cesarean delivery, and although serious complications are uncommon, neurological injury remains a source of patient morbidity and medicolegal concern. Ongoing advances in obstetric anesthesia practice, including evolving imaging techniques and increased recognition of postpartum neurological deficits, make a contemporary review of risk factors and preventive strategies timely and clinically relevant. RECENT FINDINGS: Recent literature indicates that postpartum neurological injuries are multifactorial, involving obs

Deciding the timing of aneurysm coiling and obstetric delivery in pregnant patients presenting with ruptured intracranial aneurysms: a case report and review.

The optimal treatment strategy for intracranial aneurysms (IA) in pregnancy is unclear, particularly in the third trimester of pregnancy and during the intrapartum period. We discuss a case of a ruptured IA at 39 + 3 weeks of gestation, during prelabour and review literature focusing on the order of treatment and delivery. A 4 × 6 × 3 mm aneurysm arising from the left posterior communicating segment of the internal carotid artery was embolised with multiple detachable coils, followed by a caesarean section (CS) with good maternal and foetal outcomes. Our review of the literature identified 29

From medical necessity to private-sector dominance: a two-decade evolution of caesarean section determinants in Bangladesh (2004-2022).

Caesarean section (CS) is a critical life-saving intervention when medically directed, yet its prevalence in Bangladesh has surged impulsively from approximately 4% in 2004 to nearly 45% in 2022, highlighting a distinctive South Asian pattern of widespread surgical overuse. This study examines the shifting socioeconomic and geographic determinants of CS, specifically investigating the private sector's role in widening inequalities and the transition of surgical birth from a targeted medical necessity to a routine practice dominated by private-sector utilization. We analyzed Bangladesh Demograp

Association between cesarean section and maternal body mass index among ever-married women with recent births in Bangladesh: evidence from the 2017-18 BDHS.

BACKGROUND: Cesarean section has increased substantially in Bangladesh over the past decade, raising concerns regarding its potential associations with maternal health outcomes. Evidence suggests that mode of delivery may influence postpartum weight retention and subsequent body mass index (BMI). However, population-based evidence on the association between cesarean section (C-section) and maternal body mass index in low- and middle-income countries remains limited. This study examined the association between cesarean section and maternal BMI among ever-married Bangladeshi women with recent bi

Conjugation of hydrophobic drugs to motile pRNA 4WJ nanoparticles for spontaneous tumor targeting and undetectable toxicity.

Chemotherapeutics are widely used in cancer treatments, but their toxicity, bioavailability and solubility present challenges. RNA nanotechnology has emerged as a promising modality for targeted delivery of chemotherapeutics. Structurally, RNA is thermostable, while conformationally it is dynamic and flexible. RNA's unique deformability and motility lead to rapid spontaneous tumor accumulation and glomerular excretion, thus fast body clearance, while its anionic charge and favorable small size prevent accumulation in vital organs, resulting in undetectable toxicity. We developed branched 4-way

Development and validation of a predictive model for early postoperative pain after cesarean section.

This study aimed to identify independent risk factors for early postoperative pain following cesarean section and to develop a reliable nomogram prediction model to facilitate clinical risk assessment and individualized pain management. We retrospectively collected clinical data from women who underwent cesarean section at our hospital between January 2022 and December 2023. Early postoperative pain was assessed within 24 hours after surgery using the Numerical Rating Scale (NRS), and the highest NRS score (resting or activity-related) during the first 24 hours was used as the primary outcome

Structural models for spreading and scaling digital health initiatives: A scoping review protocol.

BACKGROUND: Healthcare initiatives have a larger impact if effective initiatives are spread (brought from one site to the next) or scaled (infrastructure developed to underpin and support widespread implementation), while sustaining initial benefits. Unfortunately, many initiatives, including digital health initiatives, remain confined to the pilot stage. Of those initiatives that do progress, little is known about how to plan for the equitable spread and scale of effective initiatives. There are many structural "models" of spread and scale, defined here as conceptual representations of how in

Geospatial patterns and multilevel determinants of cesarean section rates in Iran: a Bayesian spatial analysis using INLA.

BACKGROUND: Cesarean section (CS) rates in Iran remain among the highest globally, far exceeding the WHO's recommended 10-15%. While individual determinants are well-studied, provincial spatial heterogeneity and contextual influences are underexplored. This study quantifies multilevel predictors and examines spatial patterns of CS using Bayesian spatial modeling. METHODS: Cross-sectional analysis of 24,982 deliveries from the 2010 Iran Demographic and Health Survey (IrMIDHS). OUTCOME: mode of delivery (cesarean vs. vaginal). PREDICTORS: maternal age, education, economic status, contraceptiv
