Burden and determinants of puerperal sepsis in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Puerperal sepsis is a leading cause of maternal morbidity and is responsible for nearly one-fifth of maternal deaths worldwide. In Ethiopia, it remains a significant contributor to maternal mortality; however, a comprehensive understanding of its determinants is still lacking. Therefore, the aim of this systematic review and meta-analysis is to estimate the pooled prevalence of puerperal sepsis and to identify its determinant factors in Ethiopia, thereby providing comprehensive and up-to-date evidence to inform maternal health interventions. METHODS: The review was conducted in ac

Causal effect of tea consumption on the increased risk of puerperal sepsis and the mediation effect of CD25 on IgD- CD38-B cell: A Mendelian randomization analysis.

Puerperal sepsis is a special type of sepsis characterized by a poor prognosis. The relationship between tea consumption and puerperal sepsis remains controversial. This study aimed to investigate the causal relationship between tea consumption and the risk of puerperal sepsis, as well as to explore the mediating effects of immune traits. In this study, the Mendelian randomization (MR) analysis was used to explore the causal effect of tea intake on postnatal sepsis risk and mediators. Multiple methods were used used for verification. The 2-sample MR analysis showed a positive causal relationsh

BACKGROUND: Existing evidence indicates an increasing burden of puerperal sepsis in sub-Saharan Africa. Across the world, due to a lack of self-care practice and less attention to puerperal sepsis, postpartum women are exposed to major disorders and life-threatening conditions, leading to a lower quality of life. Therefore, this study aimed to assess the knowledge of puerperal sepsis and self-care practices related to the prevention of puerperal sepsis. METHODS: A facility-based cross-sectional study was conducted among 422 postpartum women in Gurage zone public hospitals from July to August

Maternal near miss and maternal mortality and their determinants among pregnant women at a rural tertiary centre in the Eastern Cape Province, South Africa: A cross-sectional study.

BACKGROUND: Maternal near miss (MNM) is a significant entity in the assessment of the quality of obstetric care, particularly at institutional level. It renders possible the assessment of events surrounding a woman surviving what would otherwise have been a fatal outcome during childbirth or within 42 days post termination of pregnancy. The World Health Organization (WHO) introduced a tool that eases the identification of such cases and the interventions that were offered during their care. There have been several assessments of maternal deaths within South Africa (SA), but no audit of materna

A Death From Necrotizing Wound Infection After Cesarean Section: Resurgence of Group A Streptococcal Puerperal Sepsis.

Necrotizing wound infections are potentially lethal complications of surgeries, including cesarean deliveries. A 32-year-old female with obesity and hidradenitis suppurativa (HS) underwent uncomplicated cesarean section. Four days later, she developed abdominal pain and imaging showed ascites; she was treated with antibiotics. She progressed to septic shock and expired the following day from cardiopulmonary arrest. Hospital blood cultures grew no microorganisms. Autopsy revealed soft tissue purulence around the incision and copious purulent ascites without evidence of tubo-ovarian abscess, ret

Group A Streptococcus: A Re-Emerging Pathogen? Two Autopsy Cases of Deadly Infections in Previously Undiagnosed Young Adult Males.

Streptococcus pyogenes [group A strep (GAS)] is not a novel pathogen, but it is a newly emerging deadly threat. Recent data suggests that GAS, like many other contagions, saw a rise in infections in 2022, likely due to the withdrawal of COVID-19 precautions. GAS can present as a localized infection, namely tonsillopharyngitis, impetigo, cellulitis, and scarlet fever, or as an invasive infection such as meningitis, pneumonia, necrotizing fasciitis, and puerperal sepsis. Even though GAS tonsillopharyngitis is not typically fatal, it can be complicated by acute rheumatic fever if antibiotics are

Outcomes of hysterectomy for puerperal sepsis at an Academic Hospital: A retrospective study.

BACKGROUND: Pregnancy-related sepsis contributes significantly to maternal mortality. While there is substantial information on postpartum hysterectomy, information on outcomes of hysterectomy as source control for puerperal sepsis is limited. Knowledge of the common causative organisms and their antimicrobial sensitivity may assist with targeted antibiotic therapy to improve patient outcomes. This study described the indications and outcomes of surgery in patients following hysterectomy as source control for puerperal sepsis. METHODS: In a retrospective study, we analysed the intra-operative

"Why has this new vaccine come and for what reasons?" key antecedents and questions for acceptance of a future maternal GBS vaccine: Perspectives of pregnant women, lactating women, and community members in Kenya.

Group B streptococcus (GBS) is a leading global cause of neonatal sepsis and meningitis, stillbirth, and puerperal sepsis. While intrapartum antibiotic prophylaxis (IAP) is a currently available GBS disease prevention strategy, IAP is programmatically complex to implement, precluding use in low- and middle-income countries. In Kenya, 2% of stillbirths are attributable to GBS infection. Two maternal GBS vaccines are in late-stage clinical development. However, licensure of a maternal GBS vaccine does not translate into reduction of disease. We conducted 28 in-depth interviews with pregnant peop

Maternal high-care and intensive care units in low- and middle-income countries.

Despite notable advancements in minimizing maternal mortality during recent decades, a pronounced disparity persists between high-income nations and low-to middle-income countries (LMICs), particularly in intensive and high-care for pregnant and postpartum individuals. This divergence is multifactorial and influenced by factors such as the availability and accessibility of community-based maternity healthcare services, the quality of preventive care, timeliness in accessing hospital or critical care, resource availability, and facilities equipped for advanced interventions. Complications from
