Blastomycosis of the esophagus presenting with gastrointestinal bleeding.

A patient with abdominal discomfort and hematemesis was found to have lower esophageal inflammation on endoscopy. A biopsy specimen from this area showed yeast forms of Blastomyces dermatitidis with a polymorphonuclear infiltrate and granuloma formation. Extensive evaluation showed no other bleeding site. Sputum samples obtained earlier also showed B. dermatitidis. Treatment with amphotericin B led to resolution of bleeding and eradication of the fungus on repeat biopsy and sputum samples. An esophageal stricture subsequently developed and required dilation. Seven previously reported cases of

Granuloma formation and killing functions of granuloma in congenitally athymic nude mice infected with Blastomyces dermatitidis and Paracoccidioides brasiliensis.

We did this experiment to clarify the mechanism of granuloma formation and the killing functions of granuloma in nude mice against Blastomyces dermatitidis and Paracoccidioides brasiliensis infections. B. dermatitidis A-295 and P. brasiliensis B-1183 were the cultures used. Congenitally athymic nude (nu/nu) mice and their heterozygous (nu/+) littermates of BALB/c background were the test animals. From culture A-295, 0.1% and 1% cell suspensions (wet weight) were prepared and from culture B-1183 0.2% and 2% cells suspensions were prepared. Ten nu/+ and 10 nu/nu mice were allotted to each of fou

Blastomycosis.

Blastomycosis is a fungal disease with an endemic area identical to that of histoplasmosis in the United States. Disease states range from a subclinical pulmonary illness to a rapidly progressive and fatal disease. Cutaneous lesions are common although the lung is the portal of entry for blastomyces. The organisms are easily demonstrated with potassium hydroxide preparations of fresh sputum, pus from skin lesions, or other biologic material. Skin and serologic tests are unreliable, largely because of cross-reactivity with antigens of histoplasmosis. Because of the ever present potential for mi

Cultural and histopathologic confirmation of canine blastomycosis diagnosed by an agar-gel immunodiffusion test.

Sixteen sera from 17 dogs with blastomycosis produced a precipitin band identical with the diagnostically significant precipitin A band formed by a Blastomyces dermatitidis reference antiserum and a soluble B dermatitidis yeast-form antigen in the agar-gel immunodiffusion test (94% sensitivity). The other serum from a dog with histopathologic demonstration of B dermatitidis in pulmonic tissues produced an unrelated precipitin band. Sixteen of the 17 diagnoses made by the detection of precipitin A were confirmed by isolation and culture of B dermatitidis or by histopathologic demonstration of t

Light and electron microscopic observations of blastomyces dermatitidis in sputum.

Freshly expectorated sputum from a 52-year-old man with extensive pulmonary blastomycosis was examined by light and electron microscopy. Wet preparations of the sputum, stained with 1% aqueous cresyl violet, revealed numerous thick-walled, spherical blastospores and occasional budding forms. Ultrastructurally, the blastospores revealed a double-layered cell wall, the peripheral portion of which consisted of a thick, densely staining lamellar material of variable thickness, suggestive of the glycocalyx of bacterial cells. Mitochrondria and other organelles were easily recognized, but distinct n

Pathogenesis of paracoccidioidomycosis: a model based on the study of 46 patients.

Forty-six Paracoccidioidomycosis patients were studied with emphasis on lung pathology. It was found that the greatest clinical involvement of the reticuloendothelial system occurred in younger individuals. On the other hand, the frequency of tegumentary lesions was low in young patients and increased with age. Lung involvement was nearly always demonstrated when searched for and showed no relationship to the patient's age. In the young patients the disease was acute while in the older individuals its course was chronic. The findings from this study permitted formulation of a model for the pat

A rapid slide flocculation test for the diagnosis of American trypanosomiasis using Trypanosoma cruzi fragments preserved by lyophilization. Comparison with hemagglutination, immunofluorescence, and complement fixation tests.

A slide flocculation test for Chagas' disease is described, which uses a lyophilized, stable antigen obtained by formalin and ultrasonic treatment of culture forms of Trypanosoma cruzi. The test was compared with other tests for the serodiagnosis of American trypanosomiasis and showed a high sensitivity, positive results being obtained in every case of acute or chronic Chagas' disease. In sera from blood donors and from normal individuals with negative T. cruzi antigen complement fixation tests a specificity of 96% was found. False positive flocculation tests were seen, especially in cases of

Nonbacterial pus-forming diseases of the skin.

The formation of pus as a result of an inflammatory response to a bacterial infection is well known. Not so well appreciated, however, is the fact that many other nonbacterial agents such as certain fungi, viruses and parasites may provoke pus formation in the skin. Also heat, topical applications, systemically administered drugs and some injected materials can do likewise. Numerous skin diseases of unknown etiology such as pustular acne vulgaris, pustular psoriasis and pustular dermatitis herpetiformis can have bacteriologically sterile pustules. The importance of considering nonbacterial cau
