Description of Postoperative Void Patterns After Scheduled Cesarean Delivery Without Use of Routine Intraoperative Urinary Catheterization in a Large-Volume Safety Net Hospital.

BACKGROUND AND OBJECTIVES: While urinary catheters are widely utilized during cesarean delivery, little evidence exists to support the practice, and it may be associated with increased risk of urinary tract infections and unnecessary intervention. In this study we aim to describe postoperative voiding patterns and assess the prevalence of complications in patients undergoing scheduled cesarean delivery without an indwelling intraoperative urinary catheter.  Materials and methods: This is a prospective observational cohort of patients undergoing scheduled cesarean delivery at an urban safety-ne

Pregnancy Following Clam Cystoplasty and Mitrofanoff Procedure in a Patient With Fowler's Syndrome and Detrusor Overactivity.

Fowler's syndrome causes urinary retention due to failure of the urethral sphincter to relax. Management aims for complete bladder emptying, typically via intermittent self-catheterization. If conservative treatment fails, detrusor overactivity with Fowler's syndrome may be managed surgically using clam cystoplasty and the Mitrofanoff procedure to increase bladder capacity and reduce pressure. We report a rare case of pregnancy in a woman with Fowler's syndrome and detrusor overactivity, who previously underwent clam cystoplasty and Mitrofanoff procedure. She had a continent, functional abdomi

Vascularized Composite Allotransplantation of the Uterus: A Systematic Review of Eligibility Criteria.

BACKGROUND: Uterus transplantation (UTx) is the sole clinical treatment for patients with a diagnosis of uterine infertility factor to experience gestation and delivery. Following UTx, candidates who have been largely healthy must agree to extensively interface with the healthcare system and practice strict adherence to an immunosuppressive regimen to protect the allograft until delivery of a live birth. Aside from the risks associated with immunosuppression, UTx recipients often face complicated pregnancies, with a possibility of pregnancy loss and allograft failure. Therefore, appropriate re

Use of Antibiotic Eluting Calcium Sulfate Beads in High-Risk Primary Total Knee Arthroplasty.

Local delivery of high-dose antibiotics via absorbable calcium sulfate beads has been investigated as a treatment of prosthetic joint infection (PJI). We investigate this strategy as a prophylactic measure for high-risk patients undergoing primary total knee arthroplasty (TKA). A retrospective review of a single-surgeon consecutive series of primary TKA patients with identified risk factors for PJI development was performed. These patients were treated with calcium sulfate beads containing 1 g of vancomycin and 1.2 g of tobramycin per 10 cc, with 10 cc placed intraarticularly. Outcomes include

Body site-specific micro- and lactobiota in genitourinary infections during pregnancy.

BACKGROUND: Immunological, metabolic, and hormonal changes occur during pregnancy, which can affect the composition and function of the microbiota in diverse body sites. These changes can influence pregnancy outcomes and the health of the mother and baby. We aimed to characterize the microbiota of pregnant women across various body sites and examine the clinical and population-specific factors associated with its composition. The role of lactobacilli in genitourinary infections during pregnancy was analyzed as well. MATERIAL AND METHODS: One hundred and five pregnant women with gestational ag

Postnatal outcomes and surgical management of prenatally detected unilateral congenital anomalies of the kidney and urinary tract.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) represent 15-20% of prenatally diagnosed congenital anomalies, often presenting unilaterally. This study aimed to describe sonographic features of fetuses with unilateral renal anomalies and evaluate their postnatal outcomes. Additionally, we assessed whether specific prenatal ultrasound findings predicted postnatal complications. METHODS: This was a retrospective, observational study including singleton pregnancies referred to our center from 2008 to 2023 for unilateral renal anomalies identified in second or third trim

Sociodemographic Profile and Clinical Outcomes of Gestational Diabetes Mellitus: A Regional Hospital-Based Cross-Sectional Study From Mauritius.

INTRODUCTION: Mauritius is among the countries with the highest disease burden globally in diabetes mellitus (DM), ranking twelfth in terms of disease impact. Though data is available for DM in Mauritius, there is a lack of data and research on gestational DM (GDM) in Mauritius. This study aimed to bridge this gap to determine the sociodemographic details of GDM patients in the Mauritian context, the prevalence of GDM in Mauritius, identify the risk factors predisposing Mauritian women to GDM, and assess maternal and fetal/neonatal complications of GDM in Mauritius. METHODS: A cross-sectional

Economic and clinical outcomes of cesarean deliveries with skin closure using 2-octyl cyanoacrylate plus polymer mesh tape versus conventional smooth sutures plus waterproof wound dressings.

OBJECTIVE: Cesarean delivery is the most common major operating room procedure performed in the United States. Wound closure after cesarean delivery includes suturing for uterine closure and to close the fascial-and sometimes subcutaneous-tissue layer followed by skin closure. Optimal skin closure is critical as it affects the risk of both cesarean wound infection and dehiscence. To our knowledge, however, no clinical or real-world studies comparing 2OPMT with conventional sutures for skin closure following cesarean delivery have been published to date. We sought to compare the economic and cl

QT Prolongation and Torsades De Pointes Due to Undiagnosed Sheehan Syndrome: A Rare Cause of Lethal Arrhythmia.

We report a case of long QT syndrome (LQTS) associated with torsades de pointes (TdP) in a 57-year-old female patient transferred to our cardiac care unit/intensive care unit with septic shock secondary to urinary tract infection (UTI) from an adjacent hospital. The patient had no notable medications or electrolyte abnormalities that could have contributed to acquired LQTS, with normal potassium and only slightly reduced calcium (1.06 mmol/L) and magnesium (1.8 mg/dL). After detailed history taking and exploration, the patient was diagnosed with Sheehan syndrome secondary to a complicated deli

Contribution of medico-administrative databases to health vigilance: example of post-oocyte retrieval infections.

STUDY QUESTION: Can we monitor post-oocyte retrieval infections in the French national health data system to complement the French ART vigilance system? SUMMARY ANSWER: Medico-administrative databases provide a more comprehensive view of post-oocyte retrieval infections and can be used to detect abnormal increases in frequency and outlier ART centers as a complementary tool to the ART vigilance system. WHAT IS KNOWN ALREADY: The various studies of ART complications are reassuring, showing relatively low overall complication rates. Nonetheless, the European Union has set up a vigilance system

Prevalence, risk factors, and antimicrobial resistance of endemic healthcare-associated infections in Africa: a systematic review and meta-analysis.

BACKGROUND: Healthcare-associated infections (HCAI) place a significant burden on healthcare systems globally. This systematic review and meta-analysis aimed to investigate the prevalence, risk factors, and aetiologic agents of endemic HCAI in Africa. METHODS: MEDLINE/PubMed, CINAHL, and Global Health databases (EBSCOhost interface) were searched for studies published in English and French describing HCAI in Africa from 2010 to 2022. We extracted data on prevalence of HCAI, risk factors, aetiologic agents, and associated antimicrobial resistance patterns. We used random-effects models to esti

Protocol for a parallel cluster randomized trial of a participatory tailored approach to reduce overuse of antibiotics at hospital discharge: the ROAD home trial.

BACKGROUND: Antibiotic overuse at hospital discharge is common, costly, and harmful. While discharge-specific antibiotic stewardship interventions are effective, they are resource-intensive and often infeasible for hospitals with resource constraints. This weakness impacts generalizability of stewardship interventions and has health equity implications as not all patients have access to the benefits of stewardship based on where they receive care. There may be different pathways to improve discharge antibiotic prescribing that vary widely in feasibility. Supporting hospitals in selecting inter

Novel case of combination antibiotic therapy for treatment of a complicated polymicrobial urinary tract infection with one organism harboring a metallo-β-lactamase (MBL) in a pregnant patient.

Carbapenem resistance due to metallo-beta-lactamases (MBLs) is a global phenomenon and an important challenge for antibiotic therapy (Boyd et al., 2020 [1]). While previous reports have demonstrated both in vitro and in vivo synergy using the combination of ceftazidime-avibactam and aztreonam against Stenotrophomonas maltophilia, an MBL-harboring organism, this treatment strategy has not been reported during pregnancy (Mojic et al., 2017 [2], [3], Mojica et al., 2016 [4], Alexander et al., 2020 [5]). We describe a 33-year-old pregnant female with polymicrobial, bilateral pyelonephritis caused

Autologous platelet-rich plasma for assisted reproduction.

BACKGROUND: Autologous platelet-rich plasma (PRP) consists of plasma and a concentrate of platelets extracted from fresh whole blood of the person being treated. Research has suggested that intrauterine or intraovarian infusion/injection of PRP before embryo transfer may improve endometrial receptivity and response to ovarian stimulation in women undergoing assisted reproduction. We compared these interventions to standard treatment, placebo, or other interventions (mechanical or pharmacological). OBJECTIVES: To assess the effectiveness and safety of intrauterine and intraovarian infusion/inj

Acute febrile illness in pregnancy: Spectrum of disease and impact on maternal and foetal outcome.

Acute febrile illness (AFI) in pregnancy is a neglected cause of maternal and foetal mortality and morbidity in low-and middle-income countries. This prospective cohort studied antenatal and postpartum women admitted with acute fever to a tertiary care university teaching hospital from July 2014 to March 2015 for aetiology, maternal and foetal complications, and the impact on maternal mortality ratio (MMR) and perinatal mortality rate. Among the 180 women admitted with AFI, urinary tract infection 54(30%) was the commonest cause, followed by airborne infections (67; 37.2%), peripartum or wound

Liposome-encapsulated aprotinin biodistribution in mice: Side-by-side comparison with free drug formulation.

Injuries of the respiratory system caused by viral infections (e.g., by influenza virus, respiratory syncytial virus, metapneumovirus, or coronavirus) can lead to long-term complications or even life-threatening conditions. The challenges of treatment of such diseases have become particularly pronounced during the recent pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). One promising drug is the anti-fibrinolytic and anti-inflammatory protease inhibitor aprotinin, which has demonstrated considerable inhibition of the replication of some viruses. Encapsulation of
