Cytotoxic lesion of corpus callosum associated with cerebral venous sinus thrombosis.

Introduction - Cytotoxic lesion of corpus callosum (CLOCC) is a rare but increasingly recognized entity. It is associated with various etiologies including drug-related toxicity, epilepsy, infections, metabolic disturbances, and many other conditions. Here we present a rare case of CLOCC due to cerebral venous sinus thrombosis (CVST) in the postpartum period.Case report - A 25-year-old female was admitted due to persistent headaches on the third day of her postpartum period. The neurological examination did not reveal any focal neurological signs. She was diagnosed with CVST and splenium of co

Optimising gravimetric method interpretation using population-specific blood densities among Indonesian pregnant women: A cross-sectional study.

BACKGROUND: The diagnosis of postpartum haemorrhage (PPH) depends heavily on accurate and timely blood loss assessment, yet no gold standard assessment method is available. Gravimetric methods are recommended for low-resource settings, but they require an accurate blood density value for interpretation. Currently, there are no data on the blood density of pregnant or labour women. OBJECTIVE: This study measured the blood density of Indonesian pregnant women to improve the reliability of the gravimetric results in estimating postpartum blood loss. METHODS: This cross-sectional study was condu

Postoperative venous thromboembolism following cesarean delivery: prevalence, pathophysiology, diagnosis, treatment, and prevention.

Venous thromboembolism contributes to approximately 10% of pregnancy-associated deaths in the United States, with the highest incidence rate in the immediate postpartum period. Cesarean deliveries are associated with 4-fold relative risk of venous thromboembolism vs vaginal delivery. While most patients have some evidence of thrombosis in the pelvic veins after cesarean delivery, only a small proportion of patients (2.6-4.3 per 1000 deliveries) develop a clinically relevant venous thromboembolism. We outline the normal physiologic process of postcesarean hemostasis and the pathways by which ab

Epidemiology of venous thromboembolism during pregnancy and postpartum: results from the French prospective multicenter HEMOrrhage and venous THromboEmbolism in PostPartum study.

BACKGROUND: Venous thromboembolism (VTE) prevention in obstetrics remains a major public health concern. Precise data on the timing of VTE events and which women are at risk are necessary to improve thromboprophylaxis strategies. OBJECTIVES: Primary objective was to determine the incidence of VTE during postpartum. Secondary objectives were: (i) to assess VTE incidence during pregnancy; (ii) to describe VTE risk factors; (iii) to evaluate VTE incidence according to risk stratification and thromboprophylaxis. METHODS: From June 2015 to January 2019, the prospective cohort "HEMOTHEPP" study in

Executive summary of the consensus document for recommendations on the diagnosis of venous thromboembolism in pregnancy. Recommendations from the Spanish Society of Internal Medicine, the Spanish Society of Medical Radiology, the Spanish Society of Vascular Interventional Radiology, and the Spanish Society of Gynecology and Obstetrics 2025.

INTRODUCTION: Pregnancy and the postpartum period increase the risk of venous thromboembolism (VTE). In pregnant women, diagnosing VTE is challenging due to the similarity between normal pregnancy symptoms and those of VTE, the physiological elevation of D-dimer levels, and the altered validity of diagnostic scoring systems. In 2019, Pregnancy-Adapted YEARS Algorithm for diagnosis of suspected pulmonary embolism was published, providing evidence on the safety and effectiveness of this diagnostic algorithm. METHODS AND RESULTS: This consensus document, developed by the Spanish Societies of Int

Variation in outcome reporting in studies on the prevention and management of pregnancy-associated venous thromboembolism: a scoping review.

BACKGROUND: Pregnancy-associated venous thromboembolism (pVTE) is a notable cause of maternal morbidity and mortality. Yet, robust comparisons across studies are hampered by inconsistencies in outcome reporting. OBJECTIVES: This study determined the state of outcome reporting in studies addressing prevention and management of pVTE. METHODS: In this scoping review, we searched 7 databases including Medline, Embase, the Cochrane databases and 3 international trial registries. Clinical trials, registrations, and observational studies (prospective and retrospective) focusing on prevention or man

Recognition and Management of Pregnancy-Associated Venous Thromboembolism.

Pregnancy-associated venous thromboembolism is a leading cause of maternal mortality, particularly in the postpartum period. Thrombotic risk is heightened by physiological adaptations of pregnancy, including hypercoagulability, venous stasis, and vascular compression. Clinical signs and symptoms of pregnancy-associated venous thromboembolism such as edema and shortness of breath are frequently normal findings in pregnancy. Given the overlap of pathology and physiology, there is a low threshold for evaluation in the emergency department. Computed tomography is the gold standard for diagnosing p

Venous Thromboembolism in Pregnancy: Pathophysiology, Diagnostic Algorithms, and Therapeutic Approaches.

Venous thromboembolism (VTE), comprising deep venous thrombosis (DVT) and pulmonary embolism (PE), are frequently observed during pregnancy and may cause maternal death. During pregnancy, several physiological and anatomical changes occur that enhance thrombotic risk. In addition, this risk might be significantly increased by several patient-specific risk factors. Therefore, VTE risk assessment in all pregnant women is essential for early identification of those at increased risk so as to obtain a timely diagnosis and plan a correct therapeutic approach. Unfortunately, clinical examinations ma

Risk Factors Associated with Delivery Hospitalization Pulmonary Embolism.

OBJECTIVE: The objectives of this study were to analyze trends in obstetric pulmonary embolism (PE) and to determine how clinical risk factors have contributed to changes in PE risk over the study period. STUDY DESIGN: The 2000-2021 National Inpatient Sample was used for this cross-sectional analysis. Delivery hospitalizations of patients aged 15 to 54 years were identified. Trends in pulmonary embolism and associated risk factors were estimated with joinpoint regression, which calculated trends as the average annual percent change (AAPC) with 95% CIs. Risk factors analyzed included chronic c

Venous thromboembolism prophylaxis in pregnant women with inflammatory bowel disease: evidence gaps and future directions from the KASID Guidelines Task Force Team.

Venous thromboembolism (VTE) is a serious and potentially life-threatening complication in patients with inflammatory bowel disease (IBD), with the risk further increased during pregnancy and the postpartum period. Pregnancy induces a physiological hypercoagulable state, which, when combined with the chronic inflammatory and prothrombotic milieu of IBD, may result in synergistic thrombotic risk. Population-based cohort studies and meta-analyses consistently demonstrate an elevated risk of VTE during pregnancy and the postpartum period among women with IBD compared with those without, with risk

Cerebral venous sinus thrombosis in pregnancy and the postpartum period: epidemiology, management, and maternal mortality-a systematic review and meta-analysis.

BACKGROUND: Cerebral venous sinus thrombosis is a rare cerebrovascular disease of importance both during pregnancy and in the puerperium. However, the existing cohort literature relates to two distinct clinical populations: women with acute onset of pregnancy-/puerperium-associated cerebral venous thrombosis (CVST), and women with known previous CVST who were assessed in connection with a subsequent pregnancy. As these populations address distinct clinical questions and have distinct denominators and follow-up structures, this review has distinguished outcomes from the two populations and synt

Bilateral Ovarian Vein Thrombosis in a Non-Pregnant Woman: A Case Report.

Ovarian vein thrombosis (OVT) is a rare vascular disorder most commonly associated with pregnancy and the puerperium; however, it may also occur in non-pregnant women, in whom diagnosis can be challenging because of its rarity and nonspecific clinical presentation. We report the case of a 37-year-old non-pregnant woman with no significant past medical history who presented with a one-month history of undocumented fever associated with right lower quadrant abdominal pain and constitutional symptoms, including weight loss, night sweats, and fatigue. She had delivered nine months before presentat

Impact of anaemia severity on functional outcome in patients with cerebral venous thrombosis: a DOAC-CVT substudy.

INTRODUCTION: Anaemia is an established risk factor for poor outcome in intracerebral haemorrhage and ischaemic stroke. We examined whether anaemia predicts poor outcome in cerebral venous thrombosis (CVT). PATIENTS AND METHODS: We used data of the DOAC-CVT study, which was an international, prospective observational cohort study in adult patients with CVT that ran from January 2021 to January 2024. Anaemia at admission was defined according to World Health Organization criteria. Poor outcome was defined as modified Rankin Scale (mRS) 3-6 at 6-months. Binary logistic regression, adjusted for

Management of Post-Dural Puncture Headache in Postpartum Women: A Clinical and Therapeutic Update.

Post-dural puncture headache (PDPH) is a common complication of neuraxial anesthesia, particularly in the obstetric population, where its incidence is higher due to the frequent use of epidural techniques and the occurrence of accidental dural puncture. The condition arises from cerebrospinal fluid leakage following disruption of the dura mater, leading to intracranial hypotension, compensatory cerebral vasodilation, and traction of pain-sensitive structures, which collectively explain its characteristic orthostatic presentation. Pregnancy-related physiological changes, including increased int
