Managing the Critically Ill Postpartum Patient in the Emergency Department.

This review article prepares emergency physicians to recognize and manage critical illness in postpartum patients, a population at heightened risk of preventable morbidity and mortality in North America amid persistent racial/geographic disparities and shrinking obstetric access. It highlights key physiologic changes after delivery that complicate patient management. The article focuses on 3 immediate postpartum emergencies (postpartum hemorrhage, postpartum preeclampsia, and amniotic fluid embolism) and 3 subacute (pulmonary embolism, peripartum cardiomyopathy, and spontaneous coronary artery

The effect of psychiatric comorbidities on in-hospital outcomes and mortality following isolated traumatic brain injury: a retrospective cohort analysis study.

PURPOSE: The impact of preexisting psychiatric illness on outcomes following traumatic brain injury (TBI), particularly mortality and in-hospital complications, remains poorly understood. This study aimed to evaluate the influence of psychiatric comorbidities on the clinical outcomes of patients with isolated blunt TBI. METHODS: We conducted a retrospective analysis using data from the American College of Surgeons Trauma Quality Programs database (2014-2016). Patients aged 18 years or older with isolated blunt TBI were included. Participants were categorized based on the presence or absence o

Rapid detection of fibrinolytic activation in postpartum hemorrhage and acute obstetric coagulopathy using a novel assay.

Postpartum hemorrhage (PPH) remains the leading cause of pregnancy-related mortality worldwide. Regardless of the initiating cause, continued bleeding may progress to a systemic coagulopathy. This coagulopathy may be complicated further by profound fibrinolytic activation that progresses to systemic hyperfibrinolysis, a condition that we have termed acute obstetric coagulopathy (AOC). Patients with placental abruption or amniotic fluid embolism are among those at highest risk for AOC. In response to the unmet need for a rapid method to detect fibrinolytic activation in this scenario, we develo

Arterial Embolization for the Internal Hemorrhoids Management: A Systematic Review.

BACKGROUND AND AIMS: Hemorrhoids are common vascular structures that can become symptomatic when prolapsed, representing the most prevalent benign anorectal condition worldwide. Despite affecting up to 11% of the population with symptoms including pain, bleeding, and discomfort, hemorrhoids remain underdiagnosed and often inappropriately managed. This systematic review aimed to evaluate the efficacy, clinical outcomes, and morbidity of endovascular artery embolization in hemorrhoids. METHOD: Up to August 2022, PubMed, Scopus, Web of Science, and Embase were searched comprehensively. All studi

Ranked Severe Maternal Morbidity Index for Population-Level Surveillance by Mode of Delivery.

OBJECTIVE: To determine which severe maternal morbidity indicators identify the most in-hospital mortality during delivery hospitalization by delivery mode. MATERIALS AND METHODS: We obtained data from the 1993-2015 Healthcare Cost and Utilization Project's National Inpatient Sample. Separate analyses were conducted for cesarean and vaginal deliveries. We ranked 22 severe maternal morbidity indicators by their overall population-attributable fraction of in-hospital mortality. RESULTS: We identified 87,864,173 delivery hospitalizations; 27.9% were cesarean deliveries and 72.1% were vaginal de

Abnormal Uterine Bleeding Among Oral Anticoagulant Users.

OBJECTIVE: To estimate the real-world incidence of abnormal uterine bleeding after oral anticoagulant initiation with a large national claims database, stratified by agent and age group. METHODS: We conducted a nested matched case-control study using the MarketScan Research Databases (2008-2022). Women 18 years of age or older initiating oral anticoagulants (warfarin, apixaban, rivaroxaban, dabigatran, and edoxaban) for deep venous thrombosis, pulmonary embolism, or atrial fibrillation or flutter were matched 1:5 to anticoagulant nonusers. The primary outcome was abnormal uterine bleeding (AU

Atypical amniotic fluid embolism presenting with isolated coagulopathy: a case report.

BACKGROUND: Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric complication characterised by the sudden entry of amniotic fluid or fetal components into the maternal circulation, triggering severe cardiorespiratory collapse, coagulopathy, or both. While most patients exhibit abrupt hypotension and hypoxia, a coagulopathy-dominant subtype has increasingly been recognised but remains easily overlooked. Its insidious onset and absence of overt cardiopulmonary signs often delay recognition and appropriate intervention. CASE PRESENTATION: We report a 30-year-old Chinese woman (gravi

Incidence, Outcomes, and Management of In-Hospital Maternal Cardiac Arrest.

Maternal cardiac arrest is a catastrophic event, with data regarding outcomes and feasibility of timely resuscitative hysterotomy remaining scarce. Our retrospective cohort study identified 31 pregnant or postpartum patients with in-hospital cardiac arrest at a tertiary care center over 15.5 years, an incidence of 2 per 10,000 deliveries (95% CI, 1.4-2.8/10,000 deliveries). Return of spontaneous circulation (ROSC) occurred in 87.1% of the patients (95% CI, 75.2-98.8%) within a median of 2 minutes, and 77.4% (95% CI, 62.6-92.1%) survived until discharge. Eleven instances of cardiac arrest (35.5

Influence of Uterine Balloon Tamponade Prevalence on Uterine Artery Embolization in the Management of Postpartum Hemorrhage.

Objectives: The aim of our retrospective cohort study was to assess the influence of uterine balloon tamponade (UBT) prevalence on uterine artery embolization (UAE) in management of postpartum hemorrhage (PPH). Methods: This retrospective cohort study analyzed, as the final cohort, women with PPH who were transferred from other hospitals or clinics and underwent UAE at our university hospital. Initial UAE success was defined as achieving hemostasis with the first UAE procedure. The use rates of UAE, UAE success rates, and UAE procedural times were compared between the pre-UBT period (January 2

Complete Recovery From Amniotic Fluid Embolism With Cardiac Point-of-Care Ultrasound (POCUS) and Rotational Thromboelastometry (ROTEM)-Guided Resuscitation During Cesarean Section: A Case Report.

Amniotic fluid embolism (AFE) is a rare but severe obstetric emergency, with mortality approaching 50% within the first hour. The diagnosis of AFE is clinical and requires a high index of suspicion, along with the consideration of multiple differential diagnoses in peripartum patients presenting with sudden cardiovascular collapse. We report the case of a 38-week pregnant woman who, following cesarean section due to stationary labor, suffered a cardiac arrest during placental extraction. Following the return of spontaneous circulation, she exhibited bilateral decerebrate posturing and a severe

Renal Artery Pseudoaneurysm in Pregnancy Managed Successfully With Endovascular Embolisation.

Renal artery pseudoaneurysm is a rare but potentially life-threatening vascular condition, particularly when it occurs during pregnancy. Delayed recognition may lead to catastrophic haemorrhage with significant maternal and foetal morbidity. We present a case of a pregnant woman diagnosed with a renal artery pseudoaneurysm during the antenatal period who was successfully managed with endovascular embolisation. Prompt imaging, coordinated multidisciplinary care, and minimally invasive intervention enabled stabilisation of the condition and continuation of pregnancy, resulting in favourable mate

Extracorporeal cardiopulmonary resuscitation for maternal cardiac arrest secondary to amniotic fluid embolism during external cephalic version: a case report.

Amniotic fluid embolism (AFE) is a rare complication of pregnancy with high morbidity and mortality. A healthy 33-year-old G4P2012 woman at 39 + 2 weeks presented for an external cephalic version (ECV). During ECV attempts, the patient experienced sudden onset cardiac arrest. Emergent resuscitative delivery was performed followed by emergent application of extracorporeal cardiopulmonary resuscitation via peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) due to profound maternal hemodynamic instability. The patient developed postpartum hemorrhage and disseminated intravascu

Acute Postpartum Respiratory Distress With Right Ventricular Dysfunction Complicating Into Postpartum Cardiomyopathy: A Case of Suspected Atypical Amniotic Fluid Embolism.

Amniotic fluid embolism (AFE) is a devastating and unpredictable obstetric emergency typically characterized by sudden cardiopulmonary collapse and coagulopathy during or shortly after delivery. It is triggered by an anaphylactoid-histamine-mediated systemic response, evidently with no available confirmatory tests in living patients. The diagnosis is primarily clinical, supported by imaging and exclusion of mimics.

NEWS-Guided Management of Pulmonary Embolism in Pregnancy With Percutaneous Thrombectomy.

BACKGROUND: Pregnancy-associated pulmonary embolism (PE) represents a rare but life-threatening condition, with limited evidence to guide management. Reperfusion therapy must be carefully balanced against an increased risk of maternal and fetal bleeding complications. CASE SUMMARY: We report a case of a 38-year-old woman at 30 weeks' gestation who presented with acute hypoxemia and was diagnosed with central PE complicated by right ventricular dysfunction. The patient's deterioration, objectively captured by the National Early Warning Score (NEWS), guided a multidisciplinary team decision tow

Assessment of coagulation markers and decision curve analysis for predicting transfusions and hemostatic interventions in severe postpartum hemorrhage: A nationwide observational study in Japan.

BACKGROUND: Postpartum hemorrhage (PPH) remains a major cause of direct obstetric mortality. We aimed to predict pregnant women with severe PPH, defined as cases requiring hemostatic interventions-such as transcatheter arterial embolization, hysterectomy, bimanual uterine compression, or intrauterine balloon tamponade-or blood transfusions. METHODS: This retrospective observational study utilized a large-scale database covering approximately 600 hospitals in Japan from 2014 to 2023. Women of ≥18 years of age, emergency hospitalized for placental abruption, amniotic fluid embolism, or peripart

Cesarean scar pregnancy as a precursor lesion for placenta accreta spectrum: evidence and implications.

BACKGROUND: Cesarean scar pregnancy (CSP) is a form of abnormal implantation in which the gestational sac embeds within the myometrial defect of a previous cesarean incision. Its incidence is rising in parallel with increasing cesarean delivery rates. Emerging evidence indicates that CSP and placenta accreta spectrum (PAS) share common histopathological and sonographic characteristics, supporting the concept that CSP represents an early phenotype within the PAS. OBJECTIVE: To synthesize current evidence on the diagnosis, classification and management of CSP and to clarify the biological and c

Transradial versus transfemoral access in uterine artery embolization for fibroids: a systematic review and meta-analysis.

BACKGROUND: Uterine artery embolization (UAE) is a well-established, minimally invasive treatment for symptomatic uterine fibroids. Traditionally performed through transfemoral access (TFA), there has been growing interest in the transradial approach (TRA) due to its reported benefits in other interventional procedures. This systematic review and meta-analysis aim to compare TRA and TFA in UAE for uterine fibroids. METHODS: A systematic search of PubMed, Ovid MEDLINE, and Google Scholar was conducted from their date of inception to April 2025. Data were synthesized and analyzed using Review M
