The predictive value of risk assessment models for venous thromboembolism on gynaecological cancer patients.

BACKGROUND: Venous thromboembolism (VTE) is a multifactorial disease. There are two main clinical entities that are associated with morbidity and mortality, deep vein thrombosis and pulmonary embolism. Our study aimed to compare the three risk assessment models (RAMs), Khorana, Caprini and Padua in terms of predicting VTE in gynaecologic oncology patients. METHODS: Patients were retrospectively scored according to Caprini, Padua and Khorana scoring models to assess the risk for VTE. Accuracy analysis of risk assessment models was performed using sensitivity, specificity, positive and negative

Uterine Arteriovenous Malformation Presenting with Amenorrhea-A Rare Case Report and Literature Review.

Background and Clinical Significance: Uterine arteriovenous malformation (AVM) is a rare but potentially life-threatening condition due to the risk of severe acute hemorrhage. Unlike AVMs in other anatomical locations, which are predominantly congenital, uterine AVMs are often acquired, typically developing after uterine procedures such as dilatation and curettage (D&C), cesarean section, or pregnancy-related events. Women commonly present with abnormal bleeding disorders. Case Presentation: We are presenting the case of a 41-year-old Caucasian woman with secondary amenorrhea three months afte

Postpartum Aortic-Origin Uterine Artery Pseudoaneurysm: Retroperitoneal Hematoma Treated With Coils.

We report a rare case of massive left abdominopelvic hematoma (~16.6 × 13.9 × 14.5 cm) from a 1.3 cm pseudoaneurysm on an aberrant uterine artery from the anterior abdominal aorta, arising between the superior mesenteric artery and inferior mesenteric artery, supplying the lateral uterus in a woman in the postpartum period. The hematoma manifested on postpartum day two with signs of hypovolemia and a painful, throbbing lump in the left lower abdomen. Computed tomography (CT) angiography revealed a large retroperitoneal hematoma and a pseudoaneurysm associated with the anomalous artery, which a

Early application of modified A-OK protocol for amniotic fluid embolism: Case series report.

RATIONALE: Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric emergency with high mortality, characterized by sudden cardiovascular collapse, hypoxia, and coagulopathy. Current management is primarily supportive, but emerging case reports suggest a potential benefit from a novel regimen combining atropine, ondansetron, and ketorolac (A-OK). However, due to AFE's extreme rarity, large-scale studies are lacking. PATIENT CONCERNS: Three women with suspected AFE presented with acute hemodynamic instability, hypoxia, or coagulopathy during or shortly after delivery. DIAGNOSES: AFE

A Hybrid Strategy Using Uterine Artery Embolization Followed by Hysteroscopic Morcellation for Vascular Retained Products of Conception After Spontaneous Miscarriage: Two Case Reports.

Background/Objectives: Retained products of conception (RPOC) are typically managed using dilation, curettage, or hysteroscopic resection. However, when the retained tissue is hypervascular, there is a significant risk of hemorrhage, particularly in cases of spontaneous miscarriage, in which vascular RPOC is rarely reported. Uterine artery embolization (UAE) is an established method for controlling acute bleeding. However, using mechanical hysteroscopic morcellation after UAE has not been fully explored. Methods: We report two cases of reproductive-aged women who developed vascular RPOC after

Amniotic Fluid Embolism Complicated with Hepatic Rupture: A Case Report.

BACKGROUND: Amniotic fluid embolism (AFE) is a rare but highly fatal complication during delivery. It occurs when amniotic fluid suddenly enters the maternal circulation, causing acute multi-organ failure. Prompt recognition and management are crucial for improving survival rates. CASE PRESENTATION: We reported a 37-year-old woman at 39 weeks gestation who developed AFE during labor induction. The patient presented with sudden cyanosis, dyspnea, and restlessness, followed by cardiac arrest during emergency cesarean section. Postoperative refractory postpartum hemorrhage (RPPH) necessitated mu

Impact of Uterine Artery Embolization on Subsequent Fertility Outcomes: A Meta-Analysis of 20 Years of Clinical Evidence.

Background: Uterine fibroids affect 70-80% of women by age 50, often impairing fertility through mechanical distortion and altered endometrial receptivity. Uterine artery embolization (UAE) is a minimally invasive alternative to surgery, though its impact on future fertility remains debated. This meta-analysis aimed to evaluate pregnancy rates, time to conception, and fertility-related complications following UAE in women with symptomatic fibroids. Methods: A systematic search was performed across PubMed/MEDLINE, Embase, Cochrane Library, Web of Science, and other databases (January 2005-March

Cardiac Critical Care of the Cardio-Obstetric Patient.

Cardiovascular disease (CVD) is the leading cause of pregnancy-related mortality in the United States, with an incidence that has increased from 7.2 to 32.9 fatalities per 100,000 live births in the last 3 decades. This trend underscores the potential for an increase in the volume of admissions to cardiac intensive care units (CICUs) in the peripartum period. While congestive heart failure remains at the forefront of maternal morbidity, other life-threatening conditions include myocardial infarction (MI), hypertensive emergencies, fatal arrhythmias such as ventricular fibrillation, venous thro

Successful treatment of subinvolution of the placenta site with uterine artery embolisation, sparing the patient's fertility: A case report.

Subinvolution of the placental site can lead to severe post-partum haemorrhage, though it is a rare cause of the condition. Subinvolution of the placental site is an abnormal persistence of widely dilated uteroplacental spiral arteries in the absence of retained products of conception, and is associated with an increased risk of maternal morbidity and mortality. This report presents a case of an uneventful caesarean section that was followed by multiple presentations of major secondary post-partum haemorrhage, with a subsequent diagnosis of subinvolution of the placental site on histopathology

Analysis of immune-related biomarkers in amniotic fluid embolism by sequencing data and bioinformatics.

BACKGROUND: Amniotic fluid embolism (AFE) represents an uncommon yet life-threatening obstetric emergency characterized by its sudden onset and significant contribution to maternal death. Immune dysregulation has been implicated as a key pathological mechanism in AFE progression. This investigation employed through comprehensive bioinformatics approaches transcriptomic profiling of clinical specimens to systematically identify immune-associated genetic alterations that can distinguish AFE - induced pulmonary embolism from normal controls. METHODS: We conducted RNA sequencing on clinical speci

Fat Embolism Syndrome: Evolving Perspectives on Diagnosis and Care.

Fat embolism syndrome (FES) is a rare clinical entity. Fat embolization in patients with trauma and long-bone fractures occurs in a majority of patients, and only a few patients develop the triad of skin petechia, respiratory distress, and neurological disturbances, which are termed FES. The exact incidence is not known, as there are no definitive criteria. All English-language publications about FES were searched and reviewed using Google Scholar, PubMed, various databases, conference proceedings, and abstracts, which were included in this study. FES is particularly underdiagnosed and underre

Massive Pulmonary Embolism in the Postpartum Period as the First Manifestation of Antiphospholipid Syndrome: A Case Report.

Pulmonary embolism (PE) in the postpartum period is a rare but potentially fatal complication. Physiologic hypercoagulability combined with obstetric comorbidities increases thrombotic risk, while underlying autoimmune causes such as antiphospholipid syndrome (APS) may further amplify this risk. We report the case of a 28-year-old woman, four weeks post-cesarean delivery for preeclampsia and intrauterine growth restriction, who presented with acute dyspnea, chest pain, and hypoxemia. Initial non-contrast chest CT suggested pneumonia, and she was treated with intravenous antibiotics; however, p
