BACKGROUND: The diagnostic evaluation of pulmonary embolism (PE) in pregnancy is challenging, as the physiologic changes of pregnancy can mimic several PE symptoms. Acute respiratory infections introduce additional diagnostic complexity by producing systemic inflammation, altering vital signs, and, in some cases, elevating D-dimer levels. As a case in point, we examined how SARS-CoV-2 infection affected presentation, pretest probability, and diagnostic testing in pregnant patients with possible PE.
METHODS: We performed a retrospective cohort study across 21 community medical centers from Oct

OBJECTIVE: Complications of operative hysteroscopy (OH) are rare and usually minor. However, in rare cases, certain complications such as gas embolism (GE) can be life-threatening. The objective of this study was to analyze the circumstances and outcomes of GE cases occurring during OH.
MATERIALS AND METHODS: This retrospective descriptive observational study included all cases of GE during OH reported between 2016 and 2023 in the experience feedback (REX) database of Gynerisq, an organization accredited in risk management in Gynecology and Obstetrics by the French National Authority for Heal

In forensic practice, sudden maternal deaths following obstetric procedures require meticulous postmortem investigation to establish the cause of death. Although pregnancy termination is generally considered a safe procedure, rare but potentially fatal complications may occur. Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome and venous air embolism are individually uncommon but life-threatening conditions. A 23-year-old woman (gravida 2, para 1) at 9 weeks and 6 days of gestation underwent an uncomplicated elective pregnancy termination following normal preoperative l

The use of extracorporeal membrane oxygenation (ECMO) to support the pregnant patient and fetus requires a complex decision-making process. Peripartum ECMO requires coordinated and informed decision-making between an interdisciplinary team of experts, incorporating the unique considerations and, at times, competing physiologic priorities of the pregnant patient. It is often confounded by a scarcity of local relevant experience engendered by its rare occurrence. No event has made the need for an organized approach to the utilization of ECMO in pregnant patients more pressing than the COVID pand

Pulmonary embolism (PE), a potentially life-threatening condition, results from obstruction of the pulmonary arteries by thromboemboli. It typically presents with cough, dyspnea, pleuritic-type chest pain, hemoptysis, or hemodynamic instability. However, in patients with Parkinson's disease (PD) or elderly patients, it may present with recurrent falls. Falls as a primary presenting symptom of PE are rare and underrecognized but should be considered in patients with PD, especially if the falls are new-onset, unexplained, or associated with signs of hemodynamic compromise. We report a case of an

BACKGROUND: Placenta accreta spectrum (PAS) is a disorder of placental-uterine attachment often linked to placenta previa and prior cesarean delivery and characterized by hemorrhage with placental separation. Standard treatment for PAS in the United States is cesarean hysterectomy. Venous air embolism is a cesarean delivery complication defined as air trapped in the venous system.
CASE: We report a coronary artery venous air embolism during a cesarean hysterectomy managed with a surgical intermission, with mortality risk assessment and temporary conservative management as a bridge to hemodyna

BACKGROUND: Which advanced imaging modality-computed tomography pulmonary angiography (CTPA) or lung scintigraphy-is optimal to evaluate for suspected pulmonary embolism (PE) in pregnancy is debated. Practice patterns, prevalence of indeterminate imaging, and clinician response in contemporary community hospitals are not well understood.
METHODS: This retrospective cohort study included gravid adults in the emergency department (ED) and Labor and Delivery units (LDUs) who underwent advanced PE imaging 10/1/2021-3/31/2023. We excluded patients with COVID-19. Physician preferences were compared

Decompression illness (DCI) is a well-known medical emergency among scuba divers, aviators, astronauts, and compressed-air workers. It includes two diseases: decompression sickness (DCS) and arterial gas embolism (AGE). Vision changes are one of the symptoms that can happen in patients with DCI, but acute complete loss of vision is extremely rare. We report a 28-year-old male patient, a novice scuba diver who presented to our hospital with complete loss of vision while he was ascending from 18 m depth. The diagnosis of AGE was made depending on the history, clinical examination, and radiologic

INTRODUCTION: Neonatal resuscitation may require urgent umbilical venous catheter (UVC) placement. Complications can be observed with umbilical venous catheterization, especially in a stressful context. Inspired by the aeronautic environment, medical routine checklists, also called "cognitive aids," secure the equipment and environment for the patients once they are admitted to the operating room. We hypothesized that reading a cognitive aid for UVC placement in the delivery room during neonatal resuscitation simulation scenarios can (a) improve the performance in reducing catheterization dura

We present two critical cases of life-threatening postpartum hemorrhage (PPH) due to amniotic fluid embolism (AFE) complicated by disseminated intravascular coagulopathy (DIC). These cases are the first to show the effectiveness of resuscitative endovascular balloon occlusion of the aorta (REBOA) for patient survival. In Case 1, the patient, experiencing critical conditions with severe PPH and DIC despite conventional treatments, including rapid blood transfusion and fibrinogen concentrate, was air-transferred to our hospital, where REBOA was promptly employed before hysterectomy was completed

Amniotic fluid embolism (AFE) is a rare but potentially catastrophic complication during pregnancy. The most common presenting symptoms of AFE are cardiac arrest, hemodynamic instability, and profound hemorrhage. Here, we present a case of a 28-year-old female with a past medical history of anemia, gestational diabetes, hypothyroidism, anxiety, and depression, presenting for induction of labor for cholestasis of pregnancy. The labor course was complicated by sudden cardiac arrest and resuscitation, followed by an emergent bedside peri-cardiac arrest cesarean section. Subsequently, the patient

Hysteroscopic procedures complicated by air embolism (AE) are exceptionally rare occurrences in clinical practice, and there have been no previously reported cases of AE associated with hysteroscopic dilation and curettage. While the overall incidence of this complication is low, the consequences can be devastating. During early pregnancy, the unique physiological changes, such as elevated hormonal levels and increased uterine blood supply, significantly heighten the risk of AE development. Therefore, the prompt recognition of early signs and symptoms, coupled with the implementation of timely

OBJECTIVE: To compare the risk of severe maternal morbidity (SMM) from the delivery admission to 42 days' postdischarge among persons with sickle cell disease (SCD) to those without SCD.
STUDY DESIGN: This retrospective cohort study included deliveries ≥20 weeks' gestation at an urban safety net hospital in Atlanta, GA from 2011 to 2019. The exposure was SCD diagnosis. The outcome was a composite of SMM from the delivery admission to 42 days' postdischarge. SMM indicators as defined by the Centers for Disease Control and Prevention were identified using the International Classification of Dis

IMPORTANCE: Little is known about the association between sickle cell disease (SCD) and severe maternal morbidity (SMM).
OBJECTIVE: To examine the association of SCD with racial disparities in SMM and with SMM among Black individuals.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study was a retrospective population-based investigation of individuals with and without SCD in 5 states (California [2008-2018], Michigan [2008-2020], Missouri [2008-2014], Pennsylvania [2008-2014], and South Carolina [2008-2020]) delivering a fetal death or live birth. Data were analyzed between July and December

PURPOSE: Fatal neonatal pneumothorax is often described in the literature as a consequence of prematurity and respiratory distress syndrome, pulmonary congenital anomalies, artificial ventilation and cardiopulmonary resuscitation.
METHODS: Forensically clinical commentaries of the letal complications in perinatal malpractices.
RESULTS: After an orderly term delivery in a 31.- year old tercipara, the newborn developed a clinical sign of meconium aspiration syndrome, for which he was primarily resuscitated with an APGAR score of 3,5,6. In the neonatology department, one hour after delivery, se

Compared to operative hysteroscopy, diagnostic hysteroscopy rarely leads to issues. However, one very uncommon yet potentially fatal complication is air embolism, with an incidence rate of three in 17,000 cases. This report describes an unexpected complication discovered during diagnostic hysteroscopy surgery. In the course of routine infertility testing, a 29-year-old woman underwent a diagnostic hysteroscopy under general anesthesia. Intraoperatively, her end-tidal carbon dioxide (EtCO2) levels decreased, oxygen saturation dropped, and heart rate increased, leading the anesthesiologists and

INTRODUCTION: Uterine myomas are the most common benign uterine tumors. Hysteroscopic myomectomy has grown as a standard minimally invasive surgical procedure, but this technique is not free from complications.
CASE: An hysteroscopic myomectomy was performed on a 38 years-old woman. During the awakening after the procedure, she presented focal neurological deficits, thus arterial blood gas test and total body computerized tomography (CT) scan were urgently carried out. They revealed a very high carboxyhemoglobin level and abdominal venous air embolism. The patient stayed in Trendelenburg posi
