Maternal peripartum complications continue to be a significant problem in the United States, even among previously healthy women. The problems include peripartum bleeding, infection, hypertension, and thromboembolic disease. Primary care physicians are often called upon to treat these conditions. An awareness of the approach to diagnosis and management helps to ensure optimal outcome.

We describe three cases in which severe hypoxemia occurred during cesarean section. The first and second patients showed low oxygen saturation (91, 90%, respectively) on their arrival at the operation room, and hypoxemia deteriorated rapidly after the induction of anesthesia. During surgery low oxygen saturation, ranging from 90 to 95%, persisted until the beginning of spontaneous respiration. OBJECTIVE: Our goal was to develop the most comprehensive database possible to counsel patients about selective termination for fetal abnormalities, because no one center has sufficient data to assess much more than crude loss rates.
STUDY DESIGN: A total of 183 completed cases of selective termination from 9 centers in 4 countries were combined (169 twins, 11 triplets, 3 quadruplets). Variables included indications, methods, (potassium chloride, exsanguination, air embolus), gestational age at procedure, pregnancies lost (< or = 24 weeks), gestational age at delivery, and neonatal outcome.

A Holter monitor was used to record ST segment changes during cesarean section in 170 consecutive healthy parturients starting 2 h before and ending 3 h after surgery. Lumbar epidural anesthesia (LEA, n = 120) or subarachnoid anesthesia (SA, n = 50) was used. Transthoracic 2-D echocardiograms were obtained in 30 patients from the LEA group. ECG changes suggestive of myocardial ischemia are common during cesarean delivery under regional anesthesia. Air embolism may occur following criminal abortion, vaginal douching, powder insufflation as treatment for vaginal infections, and orogenital sex. The patient reported in this work deteriorated following pulmonary barotrauma. Diagnosis was made 16 h after the appearance of neurological signs. She was transferred immediately to the hyperbaric unit. The speed and completeness of recovery are directly related to the prompt diagnosis and commencement of therapy. The effect of position, horizontal versus 5 degrees reverse Trendelenburg's, on the incidence of venous emboli during Caesarean section was evaluated in 207 patients. Venous emboli were diagnosed using precordial ultrasonic Doppler monitoring. An early experience of camp laparoscopic sterilization in Gujarat State, India, resulted in 22 deaths among 106,500 women undergoing the operation during 1979 and 1980. This is a report of a 39-year-old parturient who had a haemodynamically compromising venous air embolism during a repeat Caesarean section under lumbar epidural anaesthesia. The embolism occurred immediately after surgical incision during surgery in the superficial subcutaneous tissues. The diagnosis was made using intraoperative precordial ultrasonic Doppler monitoring which allowed early and successful treatment.

Recent reports have indicated that subclinical venous air embolism (VAE) may occur in 50-65% of patients during cesarean delivery, with a potential for life-threatening embolic events. We attempted to confirm this incidence using precordial Doppler monitoring and to examine the occurrence of correlative signs thought to accompany embolic events. We investigated the occurrence of gas embolism during Caesarean section using a Doppler ultrasound probe and found that it occurs between uterine incision and delivery. Embolism is less common during general anaesthesia than has been reported during regional anaesthesia. Both ruptured membranes and a protracted uterine incision to delivery interval predispose to embolism.

A lethal paradoxical air embolism (PLE) occurred in a 21-year-old secundigravida during general anaesthesia for cervical cerclage on account of cervical insufficiency. Twenty-two selective terminations in multiple gestations were performed by a number of different methods. In 17 dichorionic pregnancies there was a successful delivery in surviving singletons or twins. Twin pregnancies pose particular problems in both prenatal diagnosis and obstetric management. We present 19 twin pregnancies that underwent fetal blood sampling (FBS). The indications were mostly similar to those for singleton pregnancies, with both fetuses being sampled.
