Postpartum stroke-disseminated intravascular coagulation associated with amniotic fluid embolism.

Postpartum stroke associated with amniotic fluid embolism (AFE) and disseminated intravascular coagulation (DIC) is an extremely rare life-threatening complication. Here, we report the clinical course and functional recovery of a postpartum woman who experienced ischemic stroke due to DIC following an AFE. Intensive and comprehensive rehabilitation therapies including physical, cognitive, and language interventions yielded significant improvements in motor function, balance, communication, and daily living activities. This case highlights the importance of structured rehabilitation to maximize

Amniotic Fluid Embolism: A Case Report.

In obstetrics, an amniotic fluid embolism (AFE) is a rare and highly devastating event. This case highlights the presentation of an amniotic fluid embolism and its treatments. A 35-year-old G1P0 became pulseless shortly after her cesarean section and was successfully resuscitated with high-quality CPR, fluid administration, and a regimen of atropine, ondansetron, and ketorolac. She was under general anesthesia for her cesarean due to non-reassuring heart tones and remained intubated when transferred to the intensive care unit. Due to the rapid responsiveness of treatment, she had a favorable o

A Prospective Randomized Controlled Simulation Study to Test a Proof-of-Concept Real-Time Periprocedural Decision Support System to Aid in Timely Diagnosis and Intervention on Patient Deterioration.

OBJECTIVES: The recent pilot simulation study by Greenberg and colleagues introduced a proof-of-concept audio-visual support system for anesthesiology professional management of perioperative patient deterioration. The results demonstrated that real-time decision support for providers may enhance diagnostic accuracy/efficiency. In collaboration with Greenberg and colleagues, the authors from Massachusetts General Brigham (MGB) similarly developed a realistic operating room simulation environment at Massachusetts General Hospital (MGH) Learning Center. Here, the ability of anesthesiology traine

Forensic evidence from a 30-year study of non-thrombotic pulmonary embolism.

Non-thrombotic pulmonary embolism (NTPE) represents a heterogeneous and underrecognized group of conditions characterized by the obstruction of the pulmonary vasculature by non-thrombotic material, often associated with complex pathogenetic mechanisms and non-specific clinical presentations. Despite their potential to cause sudden and unexpected death, comprehensive forensic studies on NTPE remain limited. This study presents a 30-year retrospective forensic analysis (1993-2024) of autopsy cases performed at the Institute of Legal Medicine of Milan, aimed at identifying and characterizing deat

Pregnancy-associated extracorporeal life support: insights from a nationwide dual-cohort analysis.

BACKGROUND: Extracorporeal life support (ECLS) is used as rescue therapy for refractory cardiopulmonary failure. Population-level evidence on prognosis and long-term sequelae in patients requiring ECLS during pregnancy or the peripartum period remains limited. METHODS: We performed a nationwide retrospective dual-cohort study using linked Taiwanese databases (2001-22). In the delivery cohort, 247 ECLS pregnancies were compared with 3 970 948 non-ECLS pregnancies. In the ECLS cohort, 247 pregnancy-associated ECLS cases were compared with 2112 non-pregnant women of reproductive age receiving EC

Forensic Pathology Involvement in Maternal Mortality Review Committees.

The objective of this paper is to outline the historical processes within the United States (U.S.) for gathering maternal death information, to highlight changes over time in how the data are reported, and to focus on current efforts to gather granular and accurate detail through the use of state-level Maternal Mortality Review Committees (MMRC). This paper will discuss the history of MMRCs, including the initiation of the first MMRC and the current state of MMRCs across the U.S. It will discuss the modern MMRC, including its mission, design, oversight, professional composition, objectives, an

Society for Maternal-Fetal Medicine Special Statement: Checklist for initial management of amniotic fluid embolism-Updated 2026.

Amniotic fluid embolism is a rare syndrome characterized by sudden cardiorespiratory collapse during labor or soon after delivery. Because of its rarity, many obstetrical providers have no experience in managing amniotic fluid embolism and may therefore benefit from a cognitive aid such as a checklist. In 2021, the Society for Maternal-Fetal Medicine published a sample checklist for the initial management of amniotic fluid embolism based on standard management guidelines. In this article, we present an updated checklist with revisions to align the terminology with advanced cardiac life support

Quantitative Evaluation of Coagulability in Obstetric DIC Using TEG6s for New Japanese Diagnostic Criteria of Obstetrical Disseminated Intravascular Coagulation.

Obstetric disseminated intravascular coagulation (DIC) onset is indicated by a bleeding tendency; however, its quantitative evaluation is difficult. Here, we quantitatively evaluated coagulability using the blood viscoelasticity test, TEG6s, for determining the threshold at which bleeding tendency occurs based on coagulation function test results and bleeding volume. This retrospective, observational study included 33 cases with potential obstetric DIC. For the coagulation blood test, we evaluated fibrinogen, fibrin/fibrinogen degradation products (FDP), platelet count, prothrombin time (PT),

Successful management of amniotic fluid embolism with cardiac arrest and liver rupture: a case report.

Amniotic fluid embolism (AFE) is an unpredictable and critical obstetric emergency. Although infrequent, it carries a significant risk of mortality. AFE is typically characterized by abrupt circulatory and respiratory failures, along with disseminated intravascular coagulation (DIC). Another rare complication during pregnancy is spontaneous liver rupture, which poses a serious danger to both the mother and the fetus. AFE generally manifests suddenly during labor, delivery, or shortly after childbirth and is believed to be triggered by the entry of amniotic fluid components into the mother's bl

Trends in Pregnancy-Related Mortality in the United States: Pregnancy Mortality Surveillance System, 2005-2024.

OBJECTIVE: To describe Pregnancy Mortality Surveillance System (PMSS)-identified underlying causes of pregnancy-related deaths and changes over time in the overall and cause-specific pregnancy-related mortality ratios in the United States, 2005-2024. METHODS: For this descriptive study, all pregnancy-related deaths were identified in the Centers for Disease Control and Prevention's PMSS from 2005 to 2024. The PMSS has nine mutually exclusive underlying cause-of-death categories: hemorrhage, infection or sepsis, amniotic fluid embolism, thrombotic pulmonary or other embolisms, hypertensive dis

Peripartum Vascular Calamities: Placental Abruption and Amniotic Fluid Emboli.

Amniotic fluid embolism (AFE) and placental abruption represent two of the most catastrophic obstetric emergencies, sharing a common pathway of severe coagulopathy while differing fundamentally in their underlying pathophysiology. AFE is characterized by sudden cardiovascular collapse, respiratory distress, and disseminated intravascular coagulation (DIC), likely triggered by an anaphylactoid-like reaction to fetal antigens entering maternal circulation. The condition manifests with both consumptive coagulopathy and hyperfibrinolysis, with platelet counts frequently falling below 50,000/μL and

From salvage hysterectomy to damage control: uterine artery ligation with tamponade for AFE with refractory hemorrhage in a 10-year analysis.

BACKGROUND: Amniotic fluid embolism (AFE) is a leading cause of maternal mortality and peripartum hysterectomy. Damage-control surgery offers a theoretical path to uterine preservation, but evidence in AFE is lacking. OBJECTIVE: To evaluate a paradigm shift from radical hysterectomy to a uterus-preserving damage-control strategy for AFE with refractory postpartum hemorrhage. METHOD: In this single-center retrospective analysis (2015-2025), patients were categorized into a Radical Surgery Era (2015-2019, n = 5) and a Damage Control Era (2020-2025, n = 6). The latter received a standardized pr

Presumed Amniotic Fluid Embolism Complicated by Disseminated Intravascular Coagulation and Refractory Postpartum Hemorrhage: A Case Report and Narrative Review.

Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric emergency characterized by sudden cardiorespiratory collapse, disseminated intravascular coagulation (DIC), and a high case-fatality rate. Because no confirmatory test exists, the diagnosis remains clinical and one of exclusion. We report a case of presumed AFE in a 39-year-old primigravida with uterine myomas, obesity, and chronic hypertension who underwent an elective primary cesarean delivery under spinal anesthesia. During manipulation of the placenta, the patient developed abrupt cardiovascular collapse requiring cardiopul

Intraoperative Collapse During First-Trimester Abortion: Hyperleukocytosis Crisis, Thrombotic Pulmonary Embolism, or Amniotic Fluid Embolism?

BACKGROUND: Hyperleukocytosis in patients with hematologic malignancies poses significant perioperative risks, including leukostasis, coagulopathy, tumor lysis syndrome and hyperinflammatory syndrome. Additionally, pregnancy and hyperleukocytosis predispose patients to a hypercoagulable state, increasing the risk of thrombosis. On the other hand, amniotic fluid embolism (AFE) is a rare but serious complication of pregnancy. Although distinct in etiology, thrombotic pulmonary embolism (PE), AFE, and hyperleukocytosis crisis can present with a strikingly similar clinical picture of pulmonary hyp

Polyarginine Peptide R11-Actin Interaction Induces a Domino Effect on Cytoskeleton Remodeling to Suppress Bladder Cancer Metastasis.

BACKGROUND: While socioeconomic status (SES) indicators such as educational attainment and employment are among the major drivers of health and illness, the health returns of SES indicators may differ across racial groups. Built on Marginalization related Diminished Returns framework (MDRs) that refers to weaker health effects of SES indicators for marginalized and minoritized groups than non-Hispanic White people, we conducted this study with two aims: First to test the association between educational attainment and employment with cardiometabolic diseases (CMDs), and second, to test racial v

Amniotic Fluid Embolism With Maternal and Neonatal Survival in the Setting of Previously Undiagnosed Placenta Accreta Spectrum 3D: A Case Report.

Amniotic fluid embolism (AFE) is a rare and life-threatening obstetric emergency characterized by sudden cardiopulmonary collapse and coagulopathy. Diagnosis remains clinical, and one of exclusion, as presentation may overlap with other causes of maternal cardiovascular collapse and hemorrhage. Here we present a 34-year-old woman, gravida 7 para 2133, at 33 weeks and one day gestation with known complete placenta previa who presented with vaginal bleeding and painful uterine contractions. Ultrasound performed two weeks prior demonstrated complete placenta previa without sonographic evidence of

Vaginal Perforations After Fecal Management System Use in Critically Ill Patients: A Case Series.

Fecal incontinence is a relatively common condition in patients admitted to the intensive care unit (ICU). Fecal management systems (FMS) were developed to divert stool away from the skin. While they have benefits, rare but serious complications can occur. This case series discusses two cases of vaginal perforations following FMS use in critically ill patients. An 87-year-old female was admitted twice to the ICU for infective endocarditis and bacterial pneumonia. During her second ICU stay, an FMS device was in place for up to 19 days, after which a vaginal perforation with stool leakage from
