Patent foramen ovale (PFO) is a common anatomical variant that can promote paradoxical embolic events. As pregnancy and the postpartum period are responsible for a hypercoagulable state, the risk for paradoxical embolism might be increased in the presence of a PFO. An updated systematic review was performed to evaluate the available data regarding the risk of adverse pregnancy outcomes in women with reported PFO-related complications during pregnancy or postpartum. A comprehensive literature search was performed using MEDLINE, Scopus, and Web of Science electronic databases from their inceptio

PURPOSE: The Placenta Accreta Spectrum (PAS) poses a significant obstetric challenge, often leading to life-threatening hemorrhage during delivery. Intraoperative Cell Salvage (IOCS) is a promising but insufficiently studied blood conservation approach.
PATIENTS AND METHODS: This retrospective cohort investigation employed propensity score matching to analyze 102 PAS cases diagnosed through combined preoperative MRI and ultrasound at a tertiary referral center between 2018 and 2022, comparing outcomes between the IOCS (n=53) and non-IOCS (n=49) groups, while utilizing inverse probability weig

Amniotic fluid embolism (AFE) is a rare, yet life-threatening obstetric emergency characterised by sudden collapse of the mother due to circulatory and respiratory failure, often accompanied by coagulopathy. It accounts for a significant proportion of peripartum cardiac arrests and maternal deaths, with an incidence of 2-8 per 100,000 deliveries. The specific pathophysiology behind AFE remains unclear. However, one hypothesis states that amniotic fluid or fetal debris enters the maternal circulation, triggering a severe inflammatory and immunologic response. Diagnosis of AFE is primarily clini

Israel has one of the lowest maternal mortality rates globally, at 2.6 per 100,000 births. Most deaths occur between the late second trimester and the postpartum period, primarily due to obstetric complications such as postpartum hemorrhage, sepsis, and amniotic fluid embolism. Unlike the U.S.A., where leading causes include violence and substance abuse, maternal deaths in Israel are predominantly obstetric. A major challenge is distinguishing between preventable and non-preventable deaths. Research suggests that more than 60% of maternal deaths could be avoided with better diagnosis, timely i

Disseminated intravascular coagulation (DIC) in the postpartum period is a rare but potentially life-threatening complication arising from various obstetric conditions, including postpartum hemorrhage (PPH), placental abruption, intrauterine fetal demise (IUFD), and amniotic fluid embolism. This review explores pathophysiology, risk factors, diagnostic challenges, and management strategies of postpartum DIC. The delicate balance of hemostasis during pregnancy predisposes women to thromboembolic events, which, when disrupted, may lead to rapid consumption of coagulation factors and subsequent c

BACKGROUND: Amniotic fluid embolism (AFE) is a life-threatening condition characterized by the entry of amniotic fluid into the maternal circulation and triggering a cytokine storm. The incidence of AFE varies across different populations and studies, ranging from 1.9 to 6.1 cases per 100,000 deliveries. It can be classified into two clinical types: the cardiopulmonary collapse type (or classic type) and the disseminated intravascular coagulopathy (DIC) type involving atonic bleeding and rapid deterioration of coagulopathy. Here we present the cases of two multiparous women who experienced DIC

Amniotic fluid embolism (AFE) is a rare but life-threatening obstetric emergency marked by respiratory distress, severe hypotension, and coagulopathy with haemorrhage. This case involves a 45-year-old nulliparous woman with a dichorionic diamniotic twin pregnancy via in vitro fertilisation (IVF) with a donor egg. At 36 weeks' gestation, the patient developed sudden respiratory distress following the detection of persistent maternal tachycardia. Cardiotocography (CTG) revealed foetal distress, necessitating an emergency caesarean section, which resulted in the delivery of two live neonates. Her

BACKGROUND: Pregnant and postpartum women face an increasing risk of severe complications due to pathophysiological changes, advanced maternal age, obesity, and reproductive technologies. Globally, maternal mortality remains a critical concern, with numerous critically ill obstetric patients requiring intensive care each year. However, comprehensive nationwide data on obstetric intensive care unit (ICU) admissions in Japan remain scarce. This study aimed to provide a detailed analysis of the clinical characteristics, causes of ICU admission, interventions, and outcomes of obstetric patients in

BACKGROUND: Amniotic fluid embolism is a rare potential complication in the peripartum period.
CASE SUMMARY: A 27-year-old nulliparous woman presented for elective induction at 41 weeks' gestation. Due to nonreassuring fetal heart tracing, the patient underwent emergent caesarean section complicated by pulseless electrical activity arrest secondary to suspected amniotic fluid embolism after fetus delivery. Despite return of spontaneous circulation, the patient remained in cardiogenic shock with right ventricular failure. The patient was cannulated for venoarterial extracorporeal membrane oxyg

BACKGROUND: Pregnancies at high risk for adverse health outcomes for mother and offspring often require long-term antenatal hospitalization and/or frequent outpatient visits. We have developed a telemonitoring home care service for high-risk pregnancies that has been integrated into the Electronic Patient Journal System of our department. We will compare clinical safety, patient-reported outcome measures, and use of healthcare resources compared to standard practice for hospital admissions and/or outpatient visits.
METHOD: The home monitoring of high-risk pregnancies study is an ongoing obser

Amniotic fluid embolism (AFE) is a critical obstetric complication characterized by the entry of amniotic fluid and its components into maternal circulation during parturition, leading to acute cardiopulmonary failure, disseminated intravascular coagulation (DIC), and anaphylactic shock. Affected patients typically exhibit abrupt onset, rapid progression, and exceedingly high mortality. Early recognition and prompt intervention are pivotal in AFE management. We present a case of AFE-induced cardiac arrest in a 35-year-old primigravida who developed acute cardiopulmonary collapse during vaginal

RATIONALE: Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric emergency with high mortality, characterized by sudden cardiovascular collapse, hypoxia, and coagulopathy. Current management is primarily supportive, but emerging case reports suggest a potential benefit from a novel regimen combining atropine, ondansetron, and ketorolac (A-OK). However, due to AFE's extreme rarity, large-scale studies are lacking.
PATIENT CONCERNS: Three women with suspected AFE presented with acute hemodynamic instability, hypoxia, or coagulopathy during or shortly after delivery.
DIAGNOSES: AFE

BACKGROUND: Amniotic fluid embolism (AFE) represents an uncommon yet life-threatening obstetric emergency characterized by its sudden onset and significant contribution to maternal death. Immune dysregulation has been implicated as a key pathological mechanism in AFE progression. This investigation employed through comprehensive bioinformatics approaches transcriptomic profiling of clinical specimens to systematically identify immune-associated genetic alterations that can distinguish AFE - induced pulmonary embolism from normal controls.
METHODS: We conducted RNA sequencing on clinical speci

BACKGROUND: Amniotic fluid embolism (AFE) with cardiac arrest is an obstetric emergency with an exceedingly high maternal mortality. Early application of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) may provide essential cardiopulmonary support, but its clinical value warrants further investigation.
CASE REPORT: This report describes the successful management of a 36-year-old parturient with advanced maternal age who sustained sudden cardiac arrest due to AFE during the second stage of labor. Immediately following arrest, the patient received high-quality CPR and achieved retur

Disseminated intravascular coagulation (DIC) is a rare, life-threatening complication of pregnancy in which a hypercoagulable state develops secondary to the contact of tissue factor with circulating blood and causes widespread abnormal clotting. This pathological process disrupts normal blood flow, which can cause multi-organ dysfunction and is associated with very high maternal morbidity and mortality. Occurring in two phases, DIC begins with overactive clotting, which leads to the formation of blood clots within the vessels, and ultimately, the overactive clotting process uses up the body's
