BACKGROUND: Optimising management of second-trimester medical abortion is important, as complications increase with gestational age. We aimed to compare a 24-h interval with a 48-h interval between mifepristone intake and misoprostol administration in in-hospital, second-trimester medical abortion for effectiveness and acceptability.
METHODS: This open-label, randomised, controlled, non-inferiority trial was conducted at nine hospitals in India, Sweden, Thailand, and Viet Nam among adults undergoing medical abortion for a singleton viable pregnancy at a gestation of between 9 weeks and 20 wee

Amniotic fluid embolism (AFE) is a rare, life-threatening obstetric event associated with extremely high maternal and perinatal morbidity and mortality with no known definitive pathophysiologic pathway. Current recommended management strategies are mainly supportive and include high-quality cardiopulmonary resuscitation and prompt delivery of the potentially viable fetus to improve neonatal survival and maternal resuscitation. More recently, case reports have described success in the resuscitation of patients with presumed AFE with a protocol that includes intravenous administration of atropin

The use of extracorporeal membrane oxygenation (ECMO) to support the pregnant patient and fetus requires a complex decision-making process. Peripartum ECMO requires coordinated and informed decision-making between an interdisciplinary team of experts, incorporating the unique considerations and, at times, competing physiologic priorities of the pregnant patient. It is often confounded by a scarcity of local relevant experience engendered by its rare occurrence. No event has made the need for an organized approach to the utilization of ECMO in pregnant patients more pressing than the COVID pand

Postpartum hemorrhage (PPH) remains a leading contributor in maternal illness and death worldwide, and its incidence continues to rise. This has led to an increased reliance on allogeneic blood transfusions (ABT), which are limited in supply and carry significant health risks. Cell salvage (CS), which collects and transfuses a patient's own blood, has been proposed as a useful adjunct or alternative. This narrative review aims to evaluate and summarize the current literature on obstetric CS (OCS) in the management of PPH, with a focus on efficacy, safety, cost-effectiveness, and future researc

The maternal mortality rate remains approximately 4 per 100 000 deliveries. Between January 2010 and July 2024, 629 maternal deaths were reported, of which 590 were reviewed. The Maternal Safety Proposal summarizes these cases. Deaths from obstetric hemorrhage decreased from 28% in 2010 to 7% in 2019 but rose to approximately 20% in 2022 and dropped to 10% in 2023. In the past 4 years, suicide has surpassed obstetric hemorrhage as a leading cause of death. In 2023, intracranial hemorrhage/infarction became the leading cause, with six cases reported. Cardiopulmonary collapse from amniotic fluid

BACKGROUND: Amniotic-fluid embolism (AFE) is a rare occurrence but life-threatening disease that can occur in process of delivery and postnatal women. The topic of cesarean section surgery and its associated risk factors has been neglected due to the infrequent occurrence of AFE. However, AFE significantly contribute to maternal morbidity and mortality.
PURPOSES: We aim to examine the correlation between clinical issues, prenatal complications, comorbidities, medical perioperative complications, and cesarean section surgery of AFE in the US, utilizing a Nationwide Inpatient Sample (NIS) datab

INTRODUCTION: Amniotic Fluid Embolism (AFE) is a rare but catastrophic obstetric emergency characterized by the sudden entry of amniotic fluid or fetal debris into the maternal circulation. This triggers acute cardiopulmonary collapse, disseminated intravascular coagulation (DIC), and multi-organ failure. Despite its low incidence, AFE remains a significant contributor to maternal mortality worldwide. The pathophysiology is poorly understood, involving immune-mediated anaphylactoid reactions and mechanical vascular obstruction.

BACKGROUND: Previous studies describe maternal cardiac arrest as a rare and often preventable event, but details of clinical care are limited. This study sought to delineate the frequency, risk factors, etiology, and management of peripartum, perianesthetic maternal cardiac arrest in a large, contemporary U.S. cohort.
METHODS: In this observational cohort study using the Multicenter Perioperative Outcomes Group database, the authors identified anesthetic records for delivery and up to 7 days postpartum for patients age 15 to 44 yr between 2015 and 2022. A screening algorithm using administrat

Amniotic fluid embolism is one of the leading causes of direct maternal deaths worldwide. It cannot be predicted. Pregnant women who have undergone a cesarean section, a multiple pregnancy, a placental implantation disorder, polyhydramnion, the need for drug-induced labor and a maternal age ≥ 35 years have a significantly increased risk of developing a peripartum amniotic fluid embolism. The diagnostics of amniotic fluid embolism are purely clinical and include a triad of sudden onset of hypoxia and hypotension, followed by coagulopathy. A causal treatment is not yet known. Treatment must be i

Amniotic fluid embolism (AFE) is a rare, sudden, and catastrophic complication of pregnancy that can result in cardiopulmonary arrest, potentially leading to death. The pathophysiology of an AFE includes an inflammatory and coagulopathic response due to fetal materials entering maternal circulation with the hallmark triad of symptoms: acute respiratory distress, cardiovascular collapse, and coagulopathy. Management of AFE should include high-quality cardiopulmonary resuscitation, immediate delivery of the fetus if applicable, early intubation to provide adequate oxygenation and ventilation, fl

Amniotic embolism remains the 3rd leading cause of maternal death in France, with 21 maternal deaths over the 2016-2018 triennium. The women who died were more likely to be obese (25%), to benefit from induction of labor (71%) and be cared in a maternity hospital <1500 deliveries/year (45%), compared with the reference population (ENP 2016). The symptom occurred mainly during labor (95%) and the course was rapid, with a symptom-to-fatality interval of 4hours 45minutes (min: 25minutes - max: 8 days). Preventability was proposed for 35% of the deaths assessed, with areas for improvement identifi

PURPOSE OF REVIEW: The worldwide leading cause of maternal death is severe maternal hemorrhage. Maternal hemorrhage can be profound leading to an entire loss of blood volume. In the past two decades, Patient Blood Management has evolved to improve patient's care and safety. In surgeries with increased blood loss exceeding 500 ml, the use of cell salvage is strongly recommended in order to preserve the patient's own blood volume and to minimize the need for allogeneic red blood cell (RBC) transfusion. In this review, recent evidence and controversies of the use of cell salvage in obstetrics are
