The effect of psychiatric comorbidities on in-hospital outcomes and mortality following isolated traumatic brain injury: a retrospective cohort analysis study.

PURPOSE: The impact of preexisting psychiatric illness on outcomes following traumatic brain injury (TBI), particularly mortality and in-hospital complications, remains poorly understood. This study aimed to evaluate the influence of psychiatric comorbidities on the clinical outcomes of patients with isolated blunt TBI. METHODS: We conducted a retrospective analysis using data from the American College of Surgeons Trauma Quality Programs database (2014-2016). Patients aged 18 years or older with isolated blunt TBI were included. Participants were categorized based on the presence or absence o

Arterial Embolization for the Internal Hemorrhoids Management: A Systematic Review.

BACKGROUND AND AIMS: Hemorrhoids are common vascular structures that can become symptomatic when prolapsed, representing the most prevalent benign anorectal condition worldwide. Despite affecting up to 11% of the population with symptoms including pain, bleeding, and discomfort, hemorrhoids remain underdiagnosed and often inappropriately managed. This systematic review aimed to evaluate the efficacy, clinical outcomes, and morbidity of endovascular artery embolization in hemorrhoids. METHOD: Up to August 2022, PubMed, Scopus, Web of Science, and Embase were searched comprehensively. All studi

Influence of Uterine Balloon Tamponade Prevalence on Uterine Artery Embolization in the Management of Postpartum Hemorrhage.

Objectives: The aim of our retrospective cohort study was to assess the influence of uterine balloon tamponade (UBT) prevalence on uterine artery embolization (UAE) in management of postpartum hemorrhage (PPH). Methods: This retrospective cohort study analyzed, as the final cohort, women with PPH who were transferred from other hospitals or clinics and underwent UAE at our university hospital. Initial UAE success was defined as achieving hemostasis with the first UAE procedure. The use rates of UAE, UAE success rates, and UAE procedural times were compared between the pre-UBT period (January 2

Complete Recovery From Amniotic Fluid Embolism With Cardiac Point-of-Care Ultrasound (POCUS) and Rotational Thromboelastometry (ROTEM)-Guided Resuscitation During Cesarean Section: A Case Report.

Amniotic fluid embolism (AFE) is a rare but severe obstetric emergency, with mortality approaching 50% within the first hour. The diagnosis of AFE is clinical and requires a high index of suspicion, along with the consideration of multiple differential diagnoses in peripartum patients presenting with sudden cardiovascular collapse. We report the case of a 38-week pregnant woman who, following cesarean section due to stationary labor, suffered a cardiac arrest during placental extraction. Following the return of spontaneous circulation, she exhibited bilateral decerebrate posturing and a severe

Renal Artery Pseudoaneurysm in Pregnancy Managed Successfully With Endovascular Embolisation.

Renal artery pseudoaneurysm is a rare but potentially life-threatening vascular condition, particularly when it occurs during pregnancy. Delayed recognition may lead to catastrophic haemorrhage with significant maternal and foetal morbidity. We present a case of a pregnant woman diagnosed with a renal artery pseudoaneurysm during the antenatal period who was successfully managed with endovascular embolisation. Prompt imaging, coordinated multidisciplinary care, and minimally invasive intervention enabled stabilisation of the condition and continuation of pregnancy, resulting in favourable mate

NEWS-Guided Management of Pulmonary Embolism in Pregnancy With Percutaneous Thrombectomy.

BACKGROUND: Pregnancy-associated pulmonary embolism (PE) represents a rare but life-threatening condition, with limited evidence to guide management. Reperfusion therapy must be carefully balanced against an increased risk of maternal and fetal bleeding complications. CASE SUMMARY: We report a case of a 38-year-old woman at 30 weeks' gestation who presented with acute hypoxemia and was diagnosed with central PE complicated by right ventricular dysfunction. The patient's deterioration, objectively captured by the National Early Warning Score (NEWS), guided a multidisciplinary team decision tow

Cesarean scar pregnancy as a precursor lesion for placenta accreta spectrum: evidence and implications.

BACKGROUND: Cesarean scar pregnancy (CSP) is a form of abnormal implantation in which the gestational sac embeds within the myometrial defect of a previous cesarean incision. Its incidence is rising in parallel with increasing cesarean delivery rates. Emerging evidence indicates that CSP and placenta accreta spectrum (PAS) share common histopathological and sonographic characteristics, supporting the concept that CSP represents an early phenotype within the PAS. OBJECTIVE: To synthesize current evidence on the diagnosis, classification and management of CSP and to clarify the biological and c

Transradial versus transfemoral access in uterine artery embolization for fibroids: a systematic review and meta-analysis.

BACKGROUND: Uterine artery embolization (UAE) is a well-established, minimally invasive treatment for symptomatic uterine fibroids. Traditionally performed through transfemoral access (TFA), there has been growing interest in the transradial approach (TRA) due to its reported benefits in other interventional procedures. This systematic review and meta-analysis aim to compare TRA and TFA in UAE for uterine fibroids. METHODS: A systematic search of PubMed, Ovid MEDLINE, and Google Scholar was conducted from their date of inception to April 2025. Data were synthesized and analyzed using Review M

Minimally Invasive Treatment of Uterine Myoma in a Patient With Thrombocytopenia: A Case of Uterine Artery Embolization Followed by Laparoscopic Surgery.

Uterine fibroids are noncancerous tumors that affect women of reproductive age. Traditional surgical interventions for uterine fibroids require larger incisions and may require longer hospital stays and recovery times than minimally invasive therapies, such as laparoscopic myomectomy, hysteroscopic myomectomy, and uterine artery embolization (UAE). Preoperative UAE blocks the blood supply to fibroids and reduces intraoperative blood loss. This report describes the successful treatment of a patient with uterine fibroids and thrombocytopenia by UAE followed by laparoscopic surgery. The patient p

Two Uncomplicated Vaginal Deliveries in a Woman With Scimitar Syndrome: A Case Report.

Scimitar syndrome is a rare, complex congenital cardiopulmonary venous anomaly characterized by anomalous drainage of the right pulmonary vein into the inferior vena cava, hypoplasia of the right lung, and dextroposition of the heart. Only a few reports mentioned perinatal outcomes in women with scimitar syndrome. We report two uncomplicated deliveries in a woman with scimitar syndrome, which was diagnosed and surgically palliated at 15 and 19 years of age, respectively. Having had dyspnea on exertion (New York Heart Association (NYHA) Class II), recurrent pneumonia, and hemoptysis, she underw

Multidisciplinary Management of Massive Pulmonary Embolism in Twin Pregnancy Using Percutaneous Thrombectomy: Maternal-Fetal Survival.

Massive pulmonary embolism (PE) during pregnancy is a major cause of maternal mortality, with increased risk in twin gestations. Hemodynamic instability in the setting of contraindication to systemic thrombolysis poses a critical therapeutic challenge, requiring a multidisciplinary approach to ensure maternal stabilization and fetal viability. A 32-year-old woman at 29 weeks of twin pregnancy was admitted with bilateral massive PE and obstructive shock. Due to the high obstetric hemorrhagic risk associated with systemic thrombolysis, percutaneous mechanical thrombectomy was performed. The proc

Paired-operator simulation training for intrauterine tamponade balloon placement: A feasibility study with integrated TeamSTEPPS assessment.

BACKGROUND: Postpartum haemorrhage remains the leading cause of maternal mortality worldwide. Intrauterine tamponade balloons are increasingly used, but reported failure rates show considerable variability across studies. Individual studies showed substantial heterogeneity, with failure rates ranging from 4% to 39% depending on clinical context. This feasibility study evaluated paired-operator simulation training combining technical skills with TeamSTEPPS non-technical competencies. METHODS: This was an ancillary analysis of the PARTUM training session. Sixteen European obstetricians from thr

Intra-uterine use of a Foley balloon catheter to tamponade an actively bleeding post-traumatic uterine arteriovenous shunt.

Arteriovenous shunts (AV-shunts) whilst rare, can complicate intrauterine surgical procedures leading to severe vaginal bleeding. We present the case of a 29-year-old woman who experienced sudden, torrential vaginal bleeding 10 days following a hysteroscopic resection and curettage of a placental remnant after vaginal delivery. Ultrasonographic evaluation with power doppler demonstrated a post-traumatic AV-shunt. Tamponading the shunt for 36 hours utilising an intra-uterine inflated Foley catheter successfully controlled the bleeding and led to complete resolution of the AV-shunt. Accurately d

Pseudoaneurysm of the Internal Pudendal Artery Causing Delayed Vaginal Hemorrhage After Robotic-Assisted Total Hysterectomy: A Case Report.

Pseudoaneurysm is a rare but potentially life-threatening cause of delayed postoperative hemorrhage. Although pseudoaneurysms are well described after obstetric and gynecologic procedures, involvement of the internal pudendal artery after hysterectomy is extremely rare. A 50-year-old nulligravid woman underwent robot-assisted total hysterectomy with bilateral salpingectomy for symptomatic uterine leiomyomas. During transvaginal specimen retrieval, vaginal wall lacerations occurred bilaterally and were sutured intraoperatively. The postoperative course was uneventful, and the patient was discha

Management of intermediate- and high-risk pulmonary embolism-a case of a pregnant patient.

Acute pulmonary embolism (PE) has a heterogeneous clinical presentation, varying from mild chest symptoms to shock or cardiac arrest. Optimal management of severe acute PE requires assessment of multiple parameters to confirm the diagnosis and assess the patient's prognosis, preferably by EXPERT-PE care/Pulmonary Embolism Response Teams (PERTs), integrating multidisciplinary expertise to optimize PE management. While management of severe acute PE already can be challenging, this particularly applies to certain patient subgroups, in particular pregnant women. As a result of ruling out pregnant
