Ecological divergence of Paracoccidioides brasiliensis lineages and P. lutzii with SDG-relevant implications.

The ecological differentiation of Paracoccidioides brasiliensis lineages (S1, PS3, S1a/b) and Paracoccidioides lutzii across South America was analysed using environmental, climatic, edaphic, and anthropogenic data. Occurrences were linked to elevation, urbanisation, Köppen-Geiger climate classes, and bioclimatic variables. S1 exhibited the broadest ecological range, spanning mid-elevation uplands to lowland Amazonia and multiple climate zones (Af, Am, Aw, Cfa). PS2, PS3 and P. lutzii were mainly restricted to mid-elevation environments. PS2 was associated with slightly higher elevations; PS3

A Novel Molecular Assay for Point-of-Care Diagnosis of Paracoccidioidomycosis from Clinical Samples Using Recombinase Polymerase Amplification Coupled with a Lateral Flow Assay.

Paracoccidioidomycosis (PCM) is a systemic mycosis endemic to Latin America. Its diagnosis is often delayed due to the limited availability of accessible and rapid molecular tools in endemic settings. In this study, we developed and conducted a preliminary evaluation of a recombinase polymerase amplification assay coupled with a lateral flow readout (RPA-LFA) for genus-level detection of Paracoccidioides spp. using reference strains and DNA extracted from clinical specimens. The assay targets the ITS1-5.8S-ITS2 ribosomal region using dual-labeled primers, which enable visual detection on LFA s

Novel antifungal agents for treating endemic mycoses: hope on the horizon?

PURPOSE OF REVIEW: Current treatments for endemic mycoses are limited by toxicity, drug-drug interactions, the need for therapeutic drug monitoring, and suboptimal penetration to sequestered sites. At the same time, several new antifungal strategies and agents are nearing clinical availability and may change practice. RECENT FINDINGS: Current literature highlights limitations of standard therapy for histoplasmosis, coccidioidomycosis, blastomycosis, paracoccidioidomycosis, talaromycosis, sporotrichosis, and emergomycosis, which often rely on amphotericin B-based induction regimens followed by

Photo Quiz: Globose Yeast-Like Forms With Multiple Peripheral Buds in Urine and Cervical Cytology.

A 26-year-old immunocompetent woman with a history of noninvasive, low-grade papillary urothelial carcinoma presented with persistent upper abdominal pain. Urine cytology, performed for bladder tumor surveillance, revealed globose, yeast-like forms (10-30 μm) with thick walls and multiple narrowly attached peripheral buds in a "ship wheel" configuration-morphology classically associated with Paracoccidioides brasiliensis. Three months later, morphologically identical organisms were identified on routine cervical Papanicolaou (Pap) test, confirmed by PAS and GMS stains on cell block material. N

Regional Heterogeneity and Temporal Trends of Infant-Juvenile Paracoccidioidomycosis in Argentina: A 15-Year Comparative Study.

Paracoccidioidomycosis (PCM) is a neglected systemic mycosis in Latin America. This study aims to characterize the epidemiology and clinical profiles of infant PCM cases (0 to 14 years old) over a 15-year period (2008-2023) in the two primary endemic regions of Argentina, the northeast (NEA) and the northwest (NWA) areas. Results reveal striking regional heterogeneity. Distribution showed clear disparities, 71.1% of cases were recorded in NWA, while 28.9% in the NEA area. The NWA region exhibited a constant temporal occurrence, whereas the NEA showed episodic occurrence/outbreaks potentially t

Acidic Stress Induces Proteomic Reprogramming and Virulence-Associated Adaptation in Paracoccidioides brasiliensis.

Paracoccidioidomycosis (PCM) is a neglected systemic mycosis whose etiologic agents must adapt to acidic host niches such as phagolysosomes. Here, we used quantitative liquid chromatography-tandem mass spectrometry (LC-MS/MS)-based proteomics to define the response of Paracoccidioides brasiliensis to acidic stress (pH 4.5) versus control pH (6.5) after 5 and 24 h. We identified and quantified 4374 proteins, including 327 and 722 differentially abundant proteins at 5 and 24 h, respectively, revealing time-dependent proteomic reprogramming. Enrichment analyses highlighted proteolysis, protein me

Epistatic Interactions Among Inflammasome Gene Variants Are Associated with Reduced Risk of Paracoccidioidomycosis.

BACKGROUND: Paracoccidioidomycosis (PCM) is an endemic systemic mycosis in Latin America with marked clinical heterogeneity, suggesting a role for host immunogenetic factors. The NLRP3 inflammasome and IL-1β signaling are important in antifungal immunity, but human genetic evidence remains limited. We evaluated whether single-nucleotide variants (SNVs) in inflammasome-related genes (NLRP1, NLRP3, CARD8, CASP1, and IL1B) are associated with PCM susceptibility, clinical form, and disease severity, including gene-gene interactions. METHODS: We conducted a case-control study including 346 individ

Aspartic Protease Inhibition Induces Proteomic Remodeling in Paracoccidioides brasiliensis.

Paracoccidioidomycosis (PCM) is a major systemic mycosis in Latin America caused by Paracoccidioides brasiliensis, yet the contribution of aspartic proteases to fungal physiology and pathogenicity remains poorly understood. Here, we employed data-independent acquisition (DIA)-based quantitative proteomics to investigate the impact of pepstatin A-mediated inhibition of aspartic proteases on the P. brasiliensis proteome. High-coverage analysis identified up to 21,156 peptides and approximately 3000 proteins per condition, revealing progressive and time-dependent proteomic remodeling after inhibi

Ileocecal Paracoccidioidomycosis Presenting as a Pseudotumoral Lesion in a Pediatric Patient: A Histopathological Diagnostic Challenge.

Ileocecal pseudotumoral lesions in pediatric patients pose a significant diagnostic challenge, particularly in endemic regions where infectious and neoplastic conditions overlap. Gastrointestinal paracoccidioidomycosis (PCM) is rare and may closely mimic intestinal tuberculosis or malignancy, leading to potential misdiagnosis. We report a 14-year-old girl presenting with constitutional symptoms and imaging findings of ileocecal wall thickening with mesenteric lymphadenopathy. Endoscopy revealed ulceroinfiltrative lesions suggestive of neoplasia. Histopathological evaluation demonstrated granul

CTLA-4 blockade improves antifungal treatment outcomes in a murine model of pulmonary paracoccidioidomycosis.

Paracoccidioidomycosis (PCM) is a systemic fungal infection endemic in the Americas, with the highest incidence in Brazil. Current treatment relies on antifungal agents such as amphotericin B (AmB), itraconazole, and fluconazole. However, long-term therapy frequently leads to treatment discontinuation and/or sequelae due to drug toxicity, chronic inflammation, and fibrosis, which can severely impair organ function and quality of life. Accordingly, immunomodulatory therapies have emerged, including the use of monoclonal antibodies against the checkpoint molecule CTLA-4. Previously, we demonstra

Comparative Analysis of Oral and Oropharyngeal Mucosal Lesions of American Tegumentary Leishmaniasis and Other Infectious Granulomatous Diseases and Squamous Cell Carcinoma.

American tegumentary leishmaniasis (ATL) and other infectious granulomatous diseases (IGDs) may present with oral/oropharyngeal mucosal lesions (OOPML). IGD-OOPML can result from fungal, parasitic, or bacterial infections, and squamous cell carcinoma (SCC) represents the main differential diagnosis. ATL, other IGD, and SCC share overlapping clinical and epidemiological features, making diagnostic suspicion challenging. This study compared sociodemographic and clinical characteristics among ATL, other IGD, and SCC. Descriptive, comparative, and multivariable logistic regression analyses were pe

Identification, selection and testing of new antigens for tegumentary Leishmaniasis serological diagnosis.

Tegumentary Leishmaniasis (TL), caused by several Leishmania species, presents a broad spectrum of clinical manifestations. This diversity complicates early and accurate diagnosis, which is crucial for effective treatment a preventing chronic or disfiguring outcomes. Conventional diagnostic methods often have significant limitations related to sample collection, technical execution, and interpretation of results. This study employed an immunoproteomic strategy to identify novel antigens, aiming to improve serological diagnostic methods for TL. Candidate peptides were selected using in silico l

Disseminated paracoccidioidomycosis with a pulmonary-spine fistula: suggestive of contiguous spread.

Paracoccidioidomycosis (PCM) rarely involves the spine. We report a 55-year-old man with disseminated PCM presenting with thoracic spinal cord compression and an unprecedented pulmonary-spine fistula identified intraoperatively. Diagnosis was confirmed by histopathology and mycology. The patient underwent surgical decompression and stabilization followed by itraconazole, achieving complete neurological recovery. This case highlights a unique complication of PCM and supports combined surgical and antifungal management in cases with neurological compromise.

Urine detection of Histoplasma antigens among symptomatic people living with HIV (PLWH) in Mato Grosso do Sul, Brazil: A comparative study of EIA and lateral flow assay.

Histoplasma spp., thermally dimorphic fungus acquired from environmental exposure, is associated with high mortality in people with advanced HIV disease. Early diagnosis remains challenging because conventional mycological methods have limited sensitivity, reinforcing the importance of urinary antigen detection. We conducted a cross-sectional study between 2022 and 2023 at a tertiary referral hospital in Campo Grande, Mato Grosso do Sul, Brazil, to assess the agreement between two urinary Histoplasma antigen detection platforms and to evaluate Histoplasma antigenuria among symptomatic people l
