Emergent simultaneous cesarean section and maternal cardiac surgery.

BACKGROUD: Management of pregnant patients with cardiovascular disease is a challenge, and limited experiences exist to date in the literature. Particularly, valve surgery is still at high risk, with a consistent rate of preterm birth, postnatal physical or mental disabilities, and fetal mortality. CASE PRESENTATION: A 38-year-old woman, who was 37 weeks pregnant, was admitted to the emergency department with severe dyspnea and hypertension. A transthoracic echocardiography revealed a 4 cm mobile mass in the left atrium, attached to the interatrial septum by a stalk; the mass protruded into t

Postpartum Rapid Cardiopulmonary Deterioration in an Obese Mother and Congenital Pneumonia in the Neonate: A Dual Case Report.

The postpartum period demands close monitoring due to rapid hemodynamic and hormonal changes that can unmask subclinical conditions or exacerbate mild illnesses, particularly in individuals with comorbidities such as obesity. We report a case of a 31-year-old woman at 36 weeks of gestation with obesity and hypothyroidism, who presented with mild respiratory symptoms and had grade I diastolic dysfunction. Although outpatient treatment was initiated for a suspected respiratory infection, multidisciplinary evaluation led to the decision for cesarean delivery under spinal anesthesia. A late preter

Pulmonary aspiration during pregnancy or immediately postpartum in the UK: A population-based case-control study.

BACKGROUND: Pulmonary aspiration of gastric contents is the most frequent cause of death associated with complications of airway management during general anaesthesia. Pregnancy increases aspiration risk owing to factors including delayed gastric emptying and increased intragastric pressure. We describe the incidence, risk factors, management, and outcomes of maternal pulmonary aspiration in pregnancy in the UK. METHODS: We conducted a population-based surveillance and case-control study. Between September 2013 and August 2016, all UK consultant-led obstetric units prospectively identified ca

Oncologic Surgery for Lower Gastrointestinal Tumors during Pregnancy: A Literature Review.

BACKGROUND: Colorectal cancer (CRC) during pregnancy can be a challenging situation due to the spatial confinement of the tumor with the growing uterus carrying the fetus. It is one of the more common tumor entities occurring in pregnant women, and therefore, it has to be taken into account if "patients describe suspicious" symptoms. SUMMARY: Diagnosis and treatment are complex and require a specialized multidisciplinary team of visceral oncologists with expertise in colorectal surgery, gastrointestinal oncologists, gynecologists, obstetricians, and neonatologists to coordinate the optimal tr

Pregnancy outcomes in women with the highest cardiovascular risk (mWHO class IV): A single-centre study.

INTRODUCTION: The modified World Health Organization (mWHO) classification of maternal risk is used to estimate morbidity and mortality in pregnant women with cardiovascular disease. Despite guidelines advising against pregnancy in the highest risk category (mWHO Class IV), successful outcomes have been achieved in select managed cases with specialised multidisciplinary care. This single-centre study examines pregnancy outcomes in this high-risk group. METHODS: We conducted a retrospective cohort study of 32 pregnancies in 30 women classified as mWHO Class IV between 2008 and 2022. Maternal d

Pulmonary complications following postpartum tubal ligation with laryngeal mask vs. endotracheal intubation: a retrospective cohort study (2012-2024).

BACKGROUND: General anesthesia with endotracheal intubation is standard in obstetric patients due to the risk of aspiration. However, endotracheal intubation carries procedure-related risks, prompting interest in laryngeal masks as alternative or rescue devices. Evidence supporting their use in postpartum tubal ligation remains limited. METHODS: A retrospective cohort study was conducted at a high-risk obstetric center, including patients who underwent postpartum tubal ligation between 2012 and 2024. Patients were grouped according to the airway device used: laryngeal mask or endotracheal int

The obstetric anesthetic management of fibrosing mediastinitis: a case series and review of the literature (1990-2025).

BACKGROUND: Fibrosing mediastinitis is a rare and potentially deadly disease characterized by immune mediated fibrosis. In the United States, it most commonly arises as a post-infectious sequela of Histoplasma capsulatum infection, leading to progressive mediastinal compression that can complicate pregnancy. This case series seeks to provide guidance on the anesthetic considerations and management of patients with fibrosing mediastinitis in the peripartum period. METHODS: We conducted a retrospective review to identify patients with fibrosing mediastinitis who received pregnancy care at Mayo

Frequency and Management of Maternal Peripartum Cardiac Arrest during Anesthetic Care: A Multicenter Retrospective Cohort Analysis.

BACKGROUND: Previous studies describe maternal cardiac arrest as a rare and often preventable event, but details of clinical care are limited. This study sought to delineate the frequency, risk factors, etiology, and management of peripartum, perianesthetic maternal cardiac arrest in a large, contemporary U.S. cohort. METHODS: In this observational cohort study using the Multicenter Perioperative Outcomes Group database, the authors identified anesthetic records for delivery and up to 7 days postpartum for patients age 15 to 44 yr between 2015 and 2022. A screening algorithm using administrat

Anesthetic Management and Outcomes of Pregnant Women With Ebstein's Anomaly: Prospective Report.

Fetomaternal outcomes and anesthetic management of pregnancy have rarely been described in a prospective cohort of women with Ebstein's anomaly. Therefore, due to the scarcity of data, diagnostic approaches and obstetric and anesthetic management plans are still evolving. In this prospective observational study, we describe the anesthetic management and fetomaternal outcomes of seven women with different grades of unrepaired Ebstein's anomaly. All women were in NYHA (New York Heart Association) Class II, none had cyanosis or cardiac decompensation at the time of admission during pregnancy. Pre

The Management of Postpartum Cardiorespiratory Failure in a Patient with COVID-19 and Sickle Cell Trait Requiring Extraorporeal Membrane Oxygenation Support and Airflight Transportation.

Acute cardiovascular disorders are incriminated in up to 33% of maternal deaths, and the presence of sickle cell anemia (SCA) aggravates the risk of peripartum complications. Herein, we present a 24-year-old Caribbean woman with known SCA who developed a vaso-occlusive crisis at 36 weeks of gestation that required emergency Cesarean section. In the early postpartum period, she experienced fever with rapid onset of acute respiratory distress in the context of COVID-19 infection that required tracheal intubation and mechanical ventilatory support with broad-spectrum antibiotics and blood exchang

Obstetric management of a parturient with severe idiopathic scoliosis.

Scoliosis complicates pregnancy due to increased operative deliveries, restrictive pulmonary disease and anaesthetic difficulties. This is a case of a primigravid with severe scoliosis who underwent primary caesarean section under spinal block with isobaric anaesthetic combined with intravenous sedation postdelivery of the baby. This case highlights the importance of a multidisciplinary approach in the management of parturient with severe scoliosis, from the preconception period until the postpartum stage.
