Heart Failure Occurring in the Perinatal Period: A Scientific Statement From the American Heart Association.

Heart failure in the perinatal period remains ambiguous in definition and management despite its recognition as a unique disease state. The true incidence and prevalence of heart failure or left ventricular systolic dysfunction during pregnancy and the postpartum period are unknown, although a prevalence as high as 1% to 2% has been reported in the general adult US population. Assessment of heart failure can be challenging in the pregnant or postpartum state, during which symptoms affecting physical function (eg, dyspnea, exercise intolerance, fatigue, and lower-extremity edema) are prevalent

Recognition of intrapartum uterine rupture, anesthetic management, and maternal-fetal outcomes: strategies for rapid response.

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes. RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T

Anesthetic Management of the Newly Diagnosed or Decompensating Cardio-Obstetric Patient.

Cardiovascular disease (CVD) is a leading cause of maternal mortality. Patients with newly diagnosed or decompensating CVD are at high risk of morbidity and mortality. Pregnancy heart teams (PHTs) allow for multidisciplinary management of the cardio-obstetric patient. Anesthesiologists are key members of the PHT. A cardio-obstetric patient's delivery plan should consider the pathophysiology of their lesion in the context of the hemodynamic changes that occur during pregnancy, labor, delivery, or termination, and the postpartum period. More research is needed to provide evidence-based guidance

Effect of Assisted Reproductive Technologies in Women With Cardiovascular Disease.

BACKGROUND: Assisted reproductive technologies (ART) include medical interventions used to address infertility. As access to ART has expanded globally, more women with underlying cardiovascular disease (CVD) are pursuing these treatments. However, data regarding the safety profile and pregnancy outcomes of ART in women with CVD remain limited. In this study we evaluated whether ART is associated with adverse maternal cardiac, obstetric, and fetal and neonatal outcomes among women with CVD. METHODS: This was a retrospective case-control study of consecutive pregnancies achieved through ART, re

Anesthetic management of a high-risk cesarean delivery in a patient with multivessel Takayasu arteritis.

Takayasu arteritis (TA) is a rare chronic inflammatory disease affecting large vessels that can result in stenosis, occlusion, and aneurysm formation. Although not a contraindication, TA poses significant anesthetic challenges in pregnancy. We describe the successful anesthetic management of a patient with TA complicated by aortic root dilation, abdominal aortic aneurysm, bilateral subclavian artery, and bilateral carotid artery stenosis undergoing elective cesarean delivery at 37 weeks. To minimize hemodynamic fluctuations, we used a slowly titrated dural puncture epidural technique along wit

Emergent simultaneous cesarean section and maternal cardiac surgery.

BACKGROUD: Management of pregnant patients with cardiovascular disease is a challenge, and limited experiences exist to date in the literature. Particularly, valve surgery is still at high risk, with a consistent rate of preterm birth, postnatal physical or mental disabilities, and fetal mortality. CASE PRESENTATION: A 38-year-old woman, who was 37 weeks pregnant, was admitted to the emergency department with severe dyspnea and hypertension. A transthoracic echocardiography revealed a 4 cm mobile mass in the left atrium, attached to the interatrial septum by a stalk; the mass protruded into t

Optimizing Psychological Health Across the Perinatal Period: An Update on Maternal Cardiovascular Health: A Scientific Statement From the American Heart Association.

Perinatal psychological health conditions (eg, perinatal depression, anxiety) are some of the leading causes of maternal mortality in the United States and are associated with adverse pregnancy outcomes, long-term cardiovascular outcomes, and intergenerational effects on offspring neurodevelopment. These risks underscore the importance of addressing maternal psychological health as a key determinant of perinatal cardiovascular health. Thus, it is vital to recognize the spectrum of perinatal psychological health and to provide guidance for both patients and clinicians on screening and managemen

Postpartum Rapid Cardiopulmonary Deterioration in an Obese Mother and Congenital Pneumonia in the Neonate: A Dual Case Report.

The postpartum period demands close monitoring due to rapid hemodynamic and hormonal changes that can unmask subclinical conditions or exacerbate mild illnesses, particularly in individuals with comorbidities such as obesity. We report a case of a 31-year-old woman at 36 weeks of gestation with obesity and hypothyroidism, who presented with mild respiratory symptoms and had grade I diastolic dysfunction. Although outpatient treatment was initiated for a suspected respiratory infection, multidisciplinary evaluation led to the decision for cesarean delivery under spinal anesthesia. A late preter

Pregnancy outcomes in women with the highest cardiovascular risk (mWHO class IV): A single-centre study.

INTRODUCTION: The modified World Health Organization (mWHO) classification of maternal risk is used to estimate morbidity and mortality in pregnant women with cardiovascular disease. Despite guidelines advising against pregnancy in the highest risk category (mWHO Class IV), successful outcomes have been achieved in select managed cases with specialised multidisciplinary care. This single-centre study examines pregnancy outcomes in this high-risk group. METHODS: We conducted a retrospective cohort study of 32 pregnancies in 30 women classified as mWHO Class IV between 2008 and 2022. Maternal d

The obstetric anesthetic management of fibrosing mediastinitis: a case series and review of the literature (1990-2025).

BACKGROUND: Fibrosing mediastinitis is a rare and potentially deadly disease characterized by immune mediated fibrosis. In the United States, it most commonly arises as a post-infectious sequela of Histoplasma capsulatum infection, leading to progressive mediastinal compression that can complicate pregnancy. This case series seeks to provide guidance on the anesthetic considerations and management of patients with fibrosing mediastinitis in the peripartum period. METHODS: We conducted a retrospective review to identify patients with fibrosing mediastinitis who received pregnancy care at Mayo

Pregnancy in Patients With McArdle's Disease.

The influence of hereditary disorders of metabolism in pregnancy can be unclear, probably due to their rarity. McArdle's disease does not have an impact on fertility; therefore, some cases of pregnancy have been reported in patients with McArdle's disease. This article reviews published cases of pregnant women with McArdle's disease, focusing on the period of pregnancy, delivery, and postpartum. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), the authors conducted a literature search of PubMed, Web of Science, and Dialnet, as well as the journal Ata P

Anesthetic Management and Outcomes of Pregnant Women With Ebstein's Anomaly: Prospective Report.

Fetomaternal outcomes and anesthetic management of pregnancy have rarely been described in a prospective cohort of women with Ebstein's anomaly. Therefore, due to the scarcity of data, diagnostic approaches and obstetric and anesthetic management plans are still evolving. In this prospective observational study, we describe the anesthetic management and fetomaternal outcomes of seven women with different grades of unrepaired Ebstein's anomaly. All women were in NYHA (New York Heart Association) Class II, none had cyanosis or cardiac decompensation at the time of admission during pregnancy. Pre

The Management of Postpartum Cardiorespiratory Failure in a Patient with COVID-19 and Sickle Cell Trait Requiring Extraorporeal Membrane Oxygenation Support and Airflight Transportation.

Acute cardiovascular disorders are incriminated in up to 33% of maternal deaths, and the presence of sickle cell anemia (SCA) aggravates the risk of peripartum complications. Herein, we present a 24-year-old Caribbean woman with known SCA who developed a vaso-occlusive crisis at 36 weeks of gestation that required emergency Cesarean section. In the early postpartum period, she experienced fever with rapid onset of acute respiratory distress in the context of COVID-19 infection that required tracheal intubation and mechanical ventilatory support with broad-spectrum antibiotics and blood exchang

Postpartum pheochromocytoma crisis associated with cardiogenic shock and Sheehan's syndrome.

We report a case of a woman in her mid-30s who developed severe cardiac shock hours after giving birth to her second child with the need for extracorporeal haemodynamic support. Initially, postpartum cardiomyopathy was suspected, and high-urgency heart transplantation was considered. However, the endocrine work-up and imaging revealed pheochromocytoma as the cause for acute heart failure that was completely reversible. Notably, the patient also developed Sheehan's syndrome with pituitary necrosis and sustained hypopituitarism, most likely as a consequence of the haemodynamic failure during phe

Pregnancy Arrhythmias: Management in the Emergency Department and Critical Care.

Pregnancy is closely associated with an elevated risk of arrhythmias, constituting the predominant cardiovascular complication during this period. Pregnancy may induce the exacerbation of previously controlled arrhythmias and, in some instances, arrhythmias may present for the first time in pregnancy. The most important proarrhythmic mechanisms during pregnancy are the atrial and ventricular stretching, coupled with increased sympathetic activity. Notably, arrhythmias, particularly those originating in the ventricles, heighten the likelihood of syncope, increasing the potential for sudden card
