Tofacitinib: New drug in the therapeutic armamentarium in ocular inflammation.

Tofacitinib, an oral Janus kinase (JAK) inhibitor, has established efficacy in the management of several autoimmune and inflammatory disorders, including rheumatoid arthritis, psoriatic arthritis, dermatitis, and ulcerative colitis. By inhibiting JAK phosphorylation, tofacitinib modulates cytokine-mediated inflammatory responses through interference with the JAK-STAT signaling pathway. In recent years, accumulating real-world evidence from India has suggested a beneficial role for tofacitinib in the management of ocular inflammatory diseases, particularly in patients with refractory disease wh

Reimagining dry eye disease management - a multimodal approach targeting the key pathophysiological drivers.

PURPOSE: Traditional dry eye disease (DED) diagnosis and subclassification often relies on limited metrics, failing to capture its complex heterogeneity. Aligning with the TFOS DEWS III framework, this study aimed to implement a novel, multimodal DED evaluation strategy, determining the prevalence of the pathophysiological DED drivers and identifying clinically relevant phenotypic clusters within a DED population. METHODS: A prospective, cross-sectional study was conducted at a university-based clinic. A total of 615 participants (aged 20-45) meeting TFOS DEWS II diagnostic criteria underwent

Failed ultrasound-guided hydrostatic reduction in pediatric intussusception: surgical-pathological characterization of failure phenotypes under a standardized protocol.

BACKGROUND: Ultrasound-guided hydrostatic reduction (UGHR) represents the first-line treatment for pediatric intussusception. However, treatment failure remains a persistent clinical challenge, driven largely by unrecognized pathological lead points (PLPs) before intervention. Large-cohort studies with complete surgical and pathological verification of all failed cases under a unified protocol remain scarce, leaving the true etiology and clinical phenotype of UGHR failure incompletely defined. PURPOSE: To define the distinct clinicopathological phenotype and surgical findings associated with

Acidic extracellular pH-induced PTH secretion in male mouse parathyroids is OGR1-dependent.

The role of extracellular acidity in regulating PTH secretion in cultured mouse parathyroid glands (PTGs) has not been studied to date, largely because of the technical difficulty of isolating mouse PTGs. We hypothesized that acidic extracellular pH directly stimulates PTH secretion through activation of a proton-sensing receptor, specifically ovarian cancer G protein-coupled receptor 1 (OGR1, also known as GPR68). To test this, we developed a method to reliably identify and isolate PTGs from male mice by administering 5-aminolevulinic acid, which induced selective fluorescence in these glands

Non-surgical treatment of essential tremor: What's in store?

Non-surgical therapy is the mainstay of treatment for Essential Tremor (ET). Traditional pharmacological treatments were found to provide variable tremor reduction, with limited tolerability and high discontinuation rates. This narrative review analyzed studies on adults with ET, focusing on current and emerging non-surgical therapies, with priority given to efficacy and safety. While ACP-711 is a newly investigated positive allosteric modulator (PAM) of GABA-A receptors, other PAMs (SAGE-324 and PRAX-114) have been withdrawn after failure to demonstrate statistically significant effects in cl

Targeting the BACE1-GSK-3β Signaling Axis in Alzheimer's Disease: From Molecular Crosstalk to Nanotechnology-Based Translational Strategies.

Recent evidence demonstrates that the interplay of amyloidogenesis and tauopathy plays a significant role in exacerbating neurodegeneration in Alzheimer's disease. This interplay of molecular pathways aids in the formation and accumulation of amyloid beta and neurofibrillary tangles extracellularly and intracellularly, respectively. These interconnected pathways highlight the need for the simultaneous inhibition of BACE1 and GSK-3β for the mitigation of disease progression. There are several drugs developed against BACE1 and GSK-3β separately, but they have limited therapeutic efficacy and fai

Case report: Maribavir as salvage therapy for multidrug-resistant CMV DNAemia in a non-transplant patient with ocular cicatricial pemphigoid.

Cytomegalovirus (CMV) is a significant cause of morbidity in immunocompromised patients, particularly those receiving chronic systemic therapy for autoimmune diseases such as ocular cicatricial pemphigoid (OCP). While the novel oral agent maribavir is approved for refractory CMV in post-transplant recipients, data regarding its efficacy in non-transplant populations remain scarce. We report the case of a 45-year-old female with OCP and comorbid Crohn's disease who developed multidrug resistant (MDR) CMV gastroenterocolitis. After failing sequential therapy with ganciclovir and foscarnet due to

The neurotoxic legacy of CAR-T cells: where do we stand?

Chimeric antigen receptor T (CAR-T) cell therapy has transformed outcomes for relapsed/refractory B-cell malignancies and is increasingly reshaping the therapeutic landscape of autoimmune disorders and solid tumors, offering curative potential where options were previously limited. Its broader deployment is, however, constrained by immune-mediated toxicities, chiefly cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS). ICANS spans a heterogeneous spectrum from mild aphasia and tremor to seizures, cerebral edema, coma, and death, and remains diffic

Case report: failed restoration of left ventricular function after myosplint® placement followed by successful HeartMate 3™ LVAD implantation.

BACKGROUND: Mechanical restoration devices such as the Myosplint® were developed in the early 2000s to reshape left ventricular geometry and delay disease progression in patients with dilated cardiomyopathy. Despite initial safety and feasibility, long-term clinical efficacy remained limited. Today, patients with prior Myosplint® implantation may still present with end-stage heart failure requiring long-term mechanical circulatory support. However, device removal during HeartMate 3™ LVAD implantation may pose technical challenges due to altered myocardial anatomy. CASE SUMMARY: We report the

Nd: YAG laser rescue of sulcus-placed glaucoma drainage device tube occlusion: a two-case series.

BACKGROUND: Tube occlusion after glaucoma drainage device (GDD) implantation is a recognized complication typically requiring surgical revision. The Aurolab Aqueous Drainage Implant (AADI) is a cost-effective device increasingly used in low- and middle-income countries, yet, to our knowledge, laser management of AADI tube blockage has not been reported. CASE PRESENTATION: Two consecutive cases of postoperative AADI tube obstruction were managed with Nd: YAG laser. Case 1 was an 11-year-old one-eyed girl with primary congenital glaucoma and a failed trabeculotomy-trabeculectomy who developed i

Idiopathic Non-Caseating Granulomatous Lesion of the Pancreas Mimicking a Neuroendocrine Tumor: A Diagnostic Challenge.

INTRODUCTION: Pancreatic granulomas are uncommon and typically classified based on the presence or absence of caseation. Caseating granulomas are most often associated with tuberculosis, while non-caseating granulomas are usually secondary to systemic conditions such as sarcoidosis. Idiopathic cases are rare and can pose a significant diagnostic challenge, particularly when mimicking malignancy. We present a unique case initially suspected to be a neuroendocrine tumor, ultimately diagnosed as an idiopathic non-caseating pancreatic granuloma. CASE PRESENTATION: A 75-year-old female was evaluat

Post-endoscopic neurosurgical Candida albicans meningoencephalitis diagnosed by targeted next-generation sequencing: a case report.

Candida albicans (C. albicans) meningoencephalitis is a rare but diagnostically challenging infection that usually occurs in immunocompromised individuals, although life-threatening disease can also develop in immunocompetent patients. The absence of specific clinical features and the limited sensitivity of conventional microbiological methods frequently lead to diagnostic delays. We describe a 79-year-old woman with no known immunodeficiency who presented with symptoms initially suggestive of bacterial meningitis and failed to improve after empirical ceftriaxone, meropenem, and vancomycin the

When Mitral Repair Fails: Understanding Recurrence, Risk Factors, and Treatment Choices.

Background: Reintervention after mitral valve repair represents a relevant clinical challenge, yet the mechanisms and timing of repair failure remain incompletely defined. Understanding how the interval between index repair and reoperation affects failure mechanisms and the feasibility of repeat repair may help refine surgical strategies. Methods: We retrospectively analyzed 194 patients undergoing repeat mitral valve surgery between 2010 and 2025 after prior repair. Median age was 70 years and 61.3% were male. Patients were stratified by time to reoperation: 0-5 years (n = 91), 6-10 years (n

Glaucoma drainage devices as a novel treatment option in the management of recurrent iris cysts.

IntroductionEpithelial inclusion cysts of the iris are a rare complication following ocular surgery or trauma and typically exhibit a more aggressive course than other types of iris cysts. Their management remains challenging due to their tendency for rapid growth, which can lead to obscuration of the visual axis, elevated intraocular pressure, corneal decompensation, and secondary glaucoma or uveitis. Particularly in pediatric and young adult patients, the high proliferative capacity of epithelial cells increases the risk of recurrence. Current treatment modalities, including Nd:YAG laser cys
