A Case Report on Meningitis Complicated by Acute Ischemic Stroke and Failed Thrombolysis.

Ischemic stroke is one of the rare but grave complications of meningitis with poor outcomes and long-term sequelae, and the role of thrombolysis with infection-related stroke remains uncertain. This case report documents a 52-year-old man presented with fever, headache, vomiting, and altered sensorium, and on examination he had neck rigidity. He was treated empirically for bacterial meningitis with antibiotics and dexamethasone, showing initial improvement. But on the 9th day of admission, he developed acute right upper motor neuron hemiparesis, dysarthria, and left-sided facial palsy. Urgent

Surgical treatment of apical petrositis through the anterior transpetrosal approach: A case report.

BACKGROUND: Apical petrositis is a rare but serious infection, often accompanied by osteomyelitis, that may cause cranial neuropathies and typically responds to antibiotics. However, surgical intervention may be required when medical therapy fails or when complications such as trigeminal neuralgia develop. CASE DESCRIPTION: We report a rare case of apical petrositis that was successfully treated using the anterior transpetrosal approach. The patient was a 74-year-old woman who presented with pain in the distribution of the right mandibular division of the trigeminal nerve. A cystic lesion was

Clinical Outcomes of First-Time and Redo Mitral Valve Replacement Using MITRIS RESILIA Bioprosthesis.

BACKGROUND: The MITRIS RESILIA bioprosthetic valve incorporates anticalcification technology and is designed to facilitate implantation in fragile mitral annuli. However, clinical data on its safety and hemodynamic performance remain limited. METHODS: We conducted a retrospective multicenter study of 140 patients (mean age, 75 ± 8 years; 44% male) who underwent first-time (n = 100) or redo (n = 40) mitral valve replacement (MVR) with a MITRIS RESILIA valve between 2021 and 2022. The primary indication for first-time MVR was mitral regurgitation, followed by mitral stenosis, whereas that for r

Sedation-Assisted, Non-sedated, and Escalated Removal of Pediatric ENT Foreign Bodies in Emergency Settings: A Systematic Review.

Pediatric ear, nose, and throat (ENT) foreign bodies are among the most common procedural challenges in emergency medicine. Substantial uncertainty exists regarding the optimal approach to removal, including sedation-assisted versus non-sedated techniques, and the appropriate thresholds for escalation to otolaryngology or the operating room. A systematic literature search was conducted in PubMed, the Cochrane Library, Web of Science, and Scopus (search date: June 2026). Studies were included if they reported outcomes of ENT foreign body removal in pediatric or predominantly pediatric emergency

Multi-complex pharmacophore mapping for the repurposing of FDA drugs as HTLV-1 protease inhibitors: Toward novel anti-HTLV-1 therapeutics.

Human T-cell Leukemia Virus Type 1 (HTLV-1) protease (PR) is a critical enzyme involved in viral maturation and represents a key target for the development of antiviral therapeutics. Despite significant efforts to design novel inhibitors, many candidates have failed due to poor pharmacokinetic and safety profiles. In the present context, drug repurposing of FDA-approved compounds offers a rapid and cost-efficient strategy for identifying potential therapeutics. Thus, in this study, a multi-complex pharmacophore mapping approach was employed to explore the structural proteome of the HTLV-1 PR s

Severe Pulmonary Cryptococcosis in an Immunocompetent Patient Presenting With Acute Respiratory Failure.

Pulmonary cryptococcosis may present with nonspecific respiratory symptoms and, in patients with underlying reactive airway disease, can act as a trigger for refractory asthma exacerbation leading to acute respiratory failure, even in immunocompetent individuals. A 60-year-old immunocompetent male, with a negative HIV test, no chronic immunosuppressive therapy, and a remote brief corticosteroid course well below the threshold for clinically significant immunosuppression, presented with approximately two months of progressive dyspnea and cough after multiple treatment courses for presumed asthm

Analysis of failure factors in vital pulp therapy and construction of a "three-circle intersection" framework: a retrospective study of 103 cases.

This retrospective study investigated the key clinical factors influencing the success of vital pulp therapy (VPT) by analyzing failed cases and evaluated the adjunctive role of a semiconductor laser in cases with different pulp exposure sizes. We included 103 VPT cases on mature permanent teeth with caries-induced pulp exposure, dividing them into four groups based on laser use and exposure size (< 0.5 mm or ≥ 0.5 mm). We graded factors including preoperative pulp vitality, intraoperative hemostasis time, and restoration quality. Treatment outcomes were assessed clinically and radiographicall

Clinical efficacy of immunoglobulin in pediatric patients with severe respiratory syncytial virus pneumonia: a retrospective clinical analysis.

BACKGROUND: The effectiveness of intravenous immunoglobulin (IVIG) in the treatment of severe respiratory syncytial virus pneumonia (SRSVP) remains a topic of ongoing debate. This study aimed to evaluate the association of IVIG on the duration of hospitalization and the use of corticosteroids in pediatric cases of SRSVP. METHODS: To minimize confounding by indication, a 1:1 propensity score matching (PSM) was performed with a caliper width of 0.02. Multivariate logistic regression analysis was conducted for the entire cohort (n = 157), while generalized estimating equations (GEE) /generalized

METTL3 promotes human coronavirus replication through an interferon-independent mechanism.

N6-methyladenosine (m6A) is a pervasive mRNA modification that regulates RNA fate through effects on RNA-protein interactions, stability and translation. We previously showed that replication of human betacoronaviruses, OC43 (hCoV-OC43) and SARS-CoV-2, is sensitive to depletion or pharmacological inhibition of the m6A RNA methyltransferase METTL3, resulting in reduced viral RNA and protein accumulation. In other viral systems, such antiviral effects have been attributed to enhanced interferon (IFN) signalling and interferon-stimulated gene (ISG) induction. Here, using hCoV-OC43 we show a requi

Teprotumumab for Dysthyroid Optic Neuropathy: A Systematic Review and Single-Arm Meta-Analysis of Visual and Orbital Outcomes.

IMPORTANCE: Dysthyroid optic neuropathy (DON) is a sight-threatening complication of thyroid eye disease (TED). The pivotal teprotumumab trials excluded patients with DON, and the only prior treatment meta-analysis addressed intravenous glucocorticoids and orbital decompression, leaving the role of teprotumumab in DON unsynthesised. OBJECTIVE: To systematically review and, where possible, meta-analyse visual and orbital outcomes following teprotumumab treatment for DON. METHODS: PubMed, Ovid (MEDLINE/Embase) and Web of Science were searched from inception to 7 June 2026, following PRISMA 202

Trajectories of Suicidal Risk Impact Mood Regulation Differently in Patients With a Diagnosis of Bipolar Disorder.

BACKGROUND: Bipolar disorder (BD) carries a suicide risk 20 times higher than the general population, with up to 60% of patients attempting suicide. Current interventions have failed to reduce its incidence; static factors have shown limited predictive utility. Emerging evidence suggests dynamic monitoring approaches may offer complementary value. This study examined whether quantifiable differences in mood regulation patterns exist across the suicidality continuum among patients diagnosed with BD. METHOD: We analyzed daily self-reported mood, anxiety, and energy levels from 164 participants

Analytically accurate, clinically inaccurate: A diagnostic case of Pseudohyperparathyroidism.

Laboratory results are central to the diagnosis and management of endocrine disorders, but preanalytical factors may lead to misleading findings and inappropriate clinical interpretation. We report a 33-year-old kidney transplant recipient with a history of secondary hyperparathyroidism treated by parathyroidectomy several years earlier, who presented with an unexpectedly elevated parathyroid hormone (PTH) concentration exceeding the analytical measurement range (AMR) despite normal serum calcium and phosphorus. Given the marked biochemical discordance, a structured investigation was undertake

Extensive left atrial thrombus invading pulmonary veins and mitral valve in severe mitral stenosis.

Severe mitral stenosis is a valvular heart disease that obstructs blood flow from the left atrium to the left ventricle, leading to impaired ventricular filling, left atrial dilatation, atrial fibrillation, and thromboembolic events. We report the case of a female patient who presented with shortness of breath (NYHA Class-II) for six months. On examination, a low-pitched murmur was heard at the apex; other systemic examination findings were unremarkable. TTE/TEE revealed left atrial dilatation with LAVI of 120 ml/m2, a large left atrial thrombus, severe rheumatic mitral stenosis, and moderate

Physics-based data augmentation to enhance deep learning performance in tropical cyclone and storm surge modelling.

Accurate modelling of tropical cyclones (TCs) is essential for reliable storm surge simulations, as TCs provide the primary external forcing. While deep learning models have demonstrated effectiveness in TC modelling, a challenge remains regarding the limited diversity of TCs in reanalysis data used for training-specifically, the scarcity of extreme TC events. This study proposes a physics-based data augmentation method that utilizes a numerical weather prediction (NWP) model to physically generate TCs lying beyond the range of reanalysis data. Subsequently, focusing on small-sized yet intense

Therapeutic Potential of Gotu Kola (Centella asiatica), Phosphatidylcholine, and Taurine in Mood Disorders: A Systematic Scoping Review.

INTRODUCTION: Mood disorders are a leading cause of disability, with a substantial proportion of patients failing to respond adequately to first-line medications. Complementary interventions are thus being explored to augment standard treatment strategies. This review aimed to systematically map and describe the existing literature on the therapeutic potential of gotu kola (Centella asiatica), phosphatidylcholine, and taurine - the active nutraceutical constituents of Neurotain® Plus - in mood disorders and to assess whether the available evidence is sufficient to support subsequent meta-analy

Transcatheter Mitral Valve Replacement in Valve-in-Valve, Valve-in-ring, and Valve-in-Mitral Annular Calcification: A Case-based Review.

Transcatheter mitral valve replacement has emerged as a viable alternative to surgery in patients with failed mitral bioprostheses, annuloplasty rings, or native disease with mitral annular calcification. This case-based review explores the evolving transcatheter mitral valve replacement landscape through 3 illustrative cases, emphasizing key aspects of preprocedural planning, technical features, and postprocedural management. Valve-in-valve procedures remain the most predictable, with encouraging mid-term outcomes. In contrast, valve-in-ring and valve-in-mitral annular calcification procedure

Transcatheter Mitral Valve Replacement in Failed Mitral Bioprosthesis: Case Report.

Transcatheter valve-in-valve mitral valve replacement (ViV TMVR) is an established therapeutic alternative for selected patients with symptomatic degeneration of a surgical mitral bioprosthesis and high or prohibitive surgical risk. We present a 75-year-old woman with a failed Pericarbon More 29-mm mitral bioprosthesis and severe prosthetic mitral stenosis, which was resolved with transseptal transcatheter ViV TMVR. Baseline echocardiography showed mitral valve area 0.45 cm2 by pressure half-time, mean transmitral gradient 13 mmHg, preserved left ventricular systolic function, estimated systol
