Heart-shaped tension-reducing suturing versus conventional interrupted suturing alleviates perineal pain, accelerates resolution of suture reactions, and improves scar quality in primiparous women undergoing perineal repair.

OBJECTIVE: To evaluate the efficacy of the heart-shaped tension-reducing suture (HSTRS) versus interrupted suture (IS) in scar quality and postoperative pain relief in primiparous women. METHODS: In this study, 142 women were allocated to HSTRS (n = 61) or IS (n = 81). Pain score was assessed via NRS at multiple timepoints; suture reactions were graded (0-III), and scar quality was evaluated using VSS, OSAS, and PSAS at 42 days, 90 days, and 180 days. RESULTS: The HSTRS group showed significantly lower pain scores (P < 0.0001), faster resolution of suture reactions, and superior scar outcome

Ultrasonography in disaster medicine: a narrative review.

This narrative review aimed to critically appraise the diagnostic accuracy, clinical impact, and operational feasibility of ultraportable point-of-care ultrasonography (POCUS) across the four domains of disaster medicine. A literature search was conducted following the SANRA and PRISMA-trAIce guidelines. Records were identified through an AI-assisted literature-discovery tool (the Undermind Deep-Search platform, interrogating the Semantic Scholar corpus) run on 15 June 2025, rather than through an independent structured multi-database search. Candidate references were screened manually against

Cervical lacerations are more frequently observed in nulliparas with obstetric anal sphincter injuries: evidence from a large cohort.

PURPOSE: Cervical lacerations, though infrequent, can lead to significant postpartum bleeding and maternal morbidity if undiagnosed. Obstetric anal sphincter injuries (OASIS) represent a more widely studied complication of vaginal delivery. However, the potential co-occurrence of OASIS and cervical lacerations has not been examined. METHODS: A retrospective cohort study was conducted at a single university-affiliated medical center from January 2018 to July 2024. The study included term singleton cephalic pregnancies that reached full cervical dilatation, including vaginal deliveries and intr

Vaginal natural orifice transluminal endoscopic myomectomy.

BACKGROUND: Vaginal natural orifice transluminal endoscopic surgery (vNOTES) avoids abdominal incisions and is increasingly used for benign gynecologic surgery, but evidence for fertility-sparing myomectomy remains limited. METHODS: We searched PubMed, MEDLINE, EMBASE and Scopus (January 1, 2000 - July 31, 2025) for comparative studies (vNOTES vs. multi-port or laparoendoscopic single-site [LESS] myomectomy) and single-arm vNOTES cohorts. Two reviewers screened/extracted data. Risk of bias was assessed using Newcastle-Ottawa (comparative) and Joanna Briggs Institute (JBI; case series). Random

Platelet-Rich Plasma in Postpartum Wound and Scar Repair: A Narrative Review of Clinical Evidence, Patient-Centered Outcomes, and Translational Challenges.

Platelet-rich plasma (PRP) is an autologous blood-derived product containing concentrated platelets and platelet-associated mediators involved in hemostasis, inflammation, angiogenesis, epithelialization, fibroblast activity, and extracellular matrix remodeling. In obstetric practice, cesarean abdominal wounds, perineal injury, pelvic floor trauma, and uterine scars represent distinct repair settings with different clinical consequences. This narrative review evaluates current evidence for PRP in postpartum wound and scar repair, with emphasis on patient-centered outcomes and the distinction b

Autologous cytokine-induced NK cells as candidate cellular senolytics: evidence, obstacles, and the experiments still needed.

Cellular senescence and chronic low-grade inflammation occupy a central, mechanistically coupled position among the hallmarks of aging. Natural killer (NK) cells decline in per-cell cytotoxicity with age, and lower NK cell cytotoxicity has been associated with higher cancer incidence and infection-related mortality in older adults. NK cells are also endogenous effectors of senescent-cell clearance, engaging stress-induced NKG2D and DNAM-1 ligands and depending on perforin-mediated cytolysis. These literatures meet at a hypothesis: that autologous cytokine-induced NK cells, manufactured from a

Applying machine-learning models to successful vaginal birth after two cesareans.

BACKGROUND: Vaginal birth after two cesarean deliveries (VBAC2) is increasingly recognized as a reasonable and, in selected cases, preferable option. Current evidence suggests that successful and safe VBAC2 can be achieved, particularly among carefully selected candidates, including grand multiparous women. However, the factors that reliably predict success and reduce maternal and neonatal risk in this population remain insufficiently defined. METHODS: This retrospective observational study included 541 women with a history of two previous cesarean deliveries who attempted vaginal birth betwe

Safety and Efficacy of [177Lu]Lu-DOTATATE in Neuroendocrine Tumors at a Tertiary Cancer Center in Jordan: A Multinational Arab Cohort.

The outbreak of coronavirus named COVID-19, initially identified in Wuhan, China in December 2019, has spread rapidly at the global scale. Most countries have rapidly stopped almost all activities including industry, services and transportation of goods and people, thus decreasing air pollution in an unprecedented way, and providing a unique opportunity to study air pollutants. While satellite data have provided visual evidence for the global reduction in air pollution such as nitrogen dioxide (NO2) worldwide, precise and quantitative information is missing at the local scale. Here we studied

A comparison in maternal and neonatal outcomes between mild fundal pressure and vacuum extraction for shortening the second stage of labor.

OBJECTIVE: Mild fundal pressure (MFP) and vacuum extraction (VE) are interventions used during the second stage of labor when clinical intervention is required. While VE is well established with standardized training and predictable risks, MFP remains controversial with limited comparative data. This study aimed to compare maternal and neonatal outcomes of MFP versus VE in women requiring intervention during the second stage of labor. METHODS: This is a retrospective cohort study of women with singleton term pregnancies requiring intervention during the second stage of labor at a tertiary med

Foxp3+ Tregs Promote M2 Macrophage Polarization via Sirt1-ERK1/2- STAT3 Pathway in Ovarian Cancer Progression.

INTRODUCTION: Epithelial Ovarian Cancer (EOC) is a highly aggressive gynecological malignancy with a high mortality rate primarily due to late-stage diagnosis and metastatic dissemination. Regulatory T cells (Tregs) have emerged as critical mediators of immune evasion, yet the role of Foxp3+ Tregs in modulating Tumor-Associated Macrophage (TAM) polarization and the underlying molecular mechanisms in EOC remains unclear. METHODS: An orthotopic EOC mouse model and in vitro co-culture systems were employed to investigate the effects of Foxp3+ Tregs on TAM polarization. Quantitative Real-Time PCR
